Studies were conducted in rivers in Kyungpook Province, Korea from July 1970 to September 1974 to determine the stations where Parafossarulus manchouricus, the intermediate snail host of Clonorchis sinensis, are abundant, and the infection rates of these snails with the cercaria of Clonorchis sinensis. Snail collections were made during four consecutive summers(July to September in each year). Ten snail habitats were found. The population density of the snails per square meter of the river bottom ranged from 25 to 250. The highest population of snails was encountered at the Dong-chon station. Cercaria studies by means of the immersing and crushing technics indicated surprisingly low rate of Clonorchis sinensis infection among the snails examined. The proportion of infected snails in Kyungpook Province was 0.8 per thousand of all snails examined. Results indicated that the Parafossarulus manchouricus exists in limited areas within the rivers under study and the infection of the snail with the cercaria of Clonorchis sinensis is very low.

2.Two Cases of Hyperlippoproteinemia (Type II).

Two cases of hyperlipoproteinemia is reported, Hyperlipoproteinemia is characterized by various kinds of cutaneous xanthomatosis associated with or without arteriosclerotic changes due to increasing lipoprotein fraction of the serum. Case I . 37-year old housewife had multiple, pea to nut sized, various shaped, cutaneous and subcutaneous nodular masses has been developed last 3 years. Some of her family member have similar symptoms. Serum cholesterol level was 790 mg% and serum triglyceride level was 270 mg% after 14 hour fasting. Case II. 52-year old housewife was suffered from pea sized, multiple, subcutaneous nodular and cutaneous eruptions last 2 years. There was no family histories. Serum cholesterol level was 895 mg% and serum triglyceride level was 350 mg% after 14 hour fasting. Typical Touton giant cells were found in biopsy specimens taken from cutaneous lesons. ECG, X-ray, glucose toleranc test and other laboratory examinations showed within normal limits. Tne agarose electrophoresis pattern of serum lipoprotein fractions are very similiar with that of hyperlipoproteinemia type II. These two patients were treated with nicotinic and 1. 0 gm and clofibrate 2.0 gm daily by mouth and restricted animal fatty foods. Total serum cholesterol level was markedly decreasea 2 months after treatment in case I.

3.The Comparison of the Ankle Arthrodesis According to Surgical Procedures.

The aim in arthrodesis of ankle is to gain a painless, normal walking gait of destroyed ankle joint caused by various origins. Despite more than 30 different fusion techniques have been described in the literature, but the results were not always satisfactory. Blair introduced tibial sliding graft, rectangular hone block donated at distal tibia, to the neck of talus. His method has advantages of a normal apperance of the foot, no shortening, and the weight bearing thrust on normal tissue. The purpose of this study was to compare the ankle arthrodesis according to surgical procedures & introduce the method which could produce excellent results for ankle arthrodesis. We reviewed 23 patients(24 cases) undergone ankle arthrodesis from Jan. 1989 to Dec. 1996 in Kyung Hee Medical Center. Male were twelve(13 cases) and female were eleven. Mean age was 42.6(l8-75)years. The Blair fusion method was used in ten cases. Iliac bone graft followed by internal fixation using Steinmann pin in 4 cases, using cancellous screw in I case, and using both Steinmann pin and staple in 1 case, while tibiotalar coaptation without bone graft followed by internal fixation using Steinmann pin in 3 cases, using staple in 1 case, and using cancellous screw in 1 case. In 3 cases, arthroscopic abrasion and cancellous screw was used. The mean duration of follow-up was 46(6-99) inonths. Time to union was average l4.6(8-23) weeks in Blair fusion and 17.5(8-40) weeks in other methods. Delayed union was observed in three cases, but no case was found in Blair fusion. Pain was ohserved in 5 cases, 3 in Blair fusion and 2 in other methods. Nonunion or pseudoarthrosis was not ohserved. On the clinical criteria for Boston Childrens Hospital ankle-scoring system, we ohserved nineteen excellent and five good cases. The difference in results according to fusion method was not ohserved. The results in tibiopedal motion using cineroentgenography were seven good and three fair cases in Blair, while two good, ten fair and two poor in other methods. By using Blair fusion, we expect early bony union and better clinical results.

4.Foveomacular Vitelliform Dystrophy, Adult Type.

Vitelliform macular dystrophy, adult type, is a type of pattern dystrophies of the pigment epithelium characterized by autosomal daminant inheritance, mid-life onset and small, round or oval, yellow deposits located at the level of the pigment epithelium. The authors report a case of this dystrophy which showed small yellow round lesion measuring 1/4-1/3 D.D. within the macula, normal electroretinogram, subnormal electrooculogram light-peak/dark-traugh ratio. typical irregular ring-like transmitted fluorescence surrounding the central non-fluorescent Iesion and leakage from perifoveal capillaries on fluorescent angiogram.

5.A Case of Osseous Choristoma Involving Both Eyes.

The authors experienced the bilateral osseous choristoma in 19 year-old female. Osseous choristoma is rare isolated bony nodule occurring in the conjunctival or episcleral connective tissues in eyes which are otherwise normal. They are present at birth and frequently have a history of increasing in size. There are no known associations with disorders of metabolism of with systemic osteogenic abnormalities.

7.Comparison of the SRK and SRKII Formulas with Revision of Constant A in Intraocular Lens Power Calculation.

The predictive accuracy of the SRK and SRKII formulas were compared in 113 pseudophakic eyes who had extracapsular cataract extraction with implantation of same type of posterior chamber IOL from Feburary 1990 to March 1991. These patients were folio we red up for at least 2 months postoperatively. Their best corrected visual acuities were above 0.5 and their astigmatisms were below 3 diopters by spectacles correction at the time of the last refraction. The results were as follows. 1. Overall, there were no significant differences in predictive accuracy between SRK and SRKII formulas. 2. The revised constant A for same type of implatnts, which constant A was 118.8. was 117.4 The SRK and SRKII formulas with revised constant A were more accurate than conventional formulas(p<0.001). 3. Our results suggested the constant A was to be revised appropriately according to the surgeon's individual regression analysis when using SRK or SRKII formulas for the calculation of intraocular lens power in Koreans.

8.A case of Equinus Deformity due to Mesenchymoma of Calf Muscle: A case Report

Joint deformity secondary to neoplastic involvement of the soft tissue has been well described and is easy to diagnose. Mesenchymema is a rare neoplasm which can arise at anyplace of the body and called as angiomyolipoma, lipomyohemangioma, fibromyxolipoma and etc. according to the composition of the various tissues. We experienoed a case of equinus deformity due to mesenchymoma of calf muscle. The patient was treated by excision and tendo calcaneus lengthening with satisfactory result.

9.Seasonal variation of snail population of Parafossarulus manchouricus and larval trematode infection in river Kumho, Kyungpook province, Korea.

Studies were conducted to determine the ecology of the snail, Parafossarulus manchouricus, the seasonal variation of the snail population, and the infestation rates of these snails with the cercariae of digenetic trematodes, in the snail habitat of river Kumho, in Dongchon of Taegu city, in Kyungpook Province, Korea, from 1975 to 1979. The earliest time the snails were found was early in April every year. At that time the water temperature ranged from 13 to 14 degrees C. The population density of snails ranged from 2 to 8 per square meter. The highest population density of snails was encountered in late June and the water temperature was between 24 and 26 degrees C. The snails disappeared in early and mid-November, when the water temperature dropped to 10-13 degrees C. The average snail population in June was 115.9 per square meter in 1975 but in 1976 dropped abruptly to 30.5 and remained at approximately the same level in 1977. It then decreased at considerably lower rates, with the average of 18.7 in 1978 and 14.5 in 1979. The snails collected in the habitat were examined for the presence of cercariae of digenetic trematodes. Of these, four species, Clonorchis sinensis, Cyathocotyle orientalis, Exorchis oviformis, and Loxogenes liberum were found. The most frequently liberated cercaria was L. liberum 43.3 per cent, followed by E. oviformis 6.2 per cent and C. orientalis 1.5 per cent. The least frequently liberated was C. sinensis 0.1 per cent. In the monthly liberation rates for larval trematodes, the higher rates were observed in June, July and August every year, and the cercarial rates for Clonorchis sinensis, Cyathocotyle orientails and Exorchis oviformis demonstrate a fairly consistently fluctuating downward trend over the five-year period, except for Loxogenes liberum. Summarizing the results, this study indicated that the population density of snails in the habitat and the infection of the snail with the cercariae of digenetic trematodes has decreased over the five-year period from 1975 to 1979.

10.Cheese-like Material in the Heart: An Autopsy Case Report of Severe Hypertriglyceridemia in Diabetic Ketoacidosis Patient.

Here we report an autopsy case of diabetic ketoacidosis (DKA) with severe hypertriglyceridemia (12,900 mg/dl). A 29-year-old woman with a history of type 1 diabetes was found dead at a motel. There was no injury on external inspection, but a lump of cheese-like material was noted in the heart at autopsy and peripheral blood plasma had a creamy appearance. After postmortem biochemical analysis, we made a diagnosis of DKA with severe hypertriglyceridemia and concluded that these unusual autopsy findings were caused by DKA and postmortem change. Uncontrolled diabetes often causes DKA and hypertriglyceridemia. To the best of our knowledge, this is the first report in Korea of DKA with severe hypertriglyceridemia diagnosed by autopsy.
