Sexual impotence from positioning on the fracture table is rare complication. We are reporting a case of pudendal nerve neurapraxia associated with pressure from the perineal post of the fracture table in 42 years old male with hip fracture. Historical reviews and suggestion for prophylactic measures are also reported in this paper.

3.Monostotic Fibrous Dysplasia of the Cervical Spine: A Case Report

Fibrous dysplasis is a condition characterized by the metastatic replacement of the medullary component of predominantly long bone with fibrous tissues. This change is responsible for the characteristic features, including bony abnormalities, pathologic fractures and pain. The monostotic form of the fibrous dysplasia occurs more freguent than the polyostotic form. The monostotic fibrous dysplasia does not exhibit the laboratory abnormality, extra-skeletal manifestation or the endocrinopathies associated writh polyostotic disease. Monostotic fibrous dysplasia occurs most frequently in the metaphysis of long bones and the ribs. Only the seven previous cases of vertebral involvement (three cervical, one thoracic, three lumbar spine) in monostotic fibrous dysplsia have been reported. Authors experienced a case of monosotic fibrous dysplasia of the seventh cervical vertebrate, so we report a case with the review of the seven previous reported cases of the monosotic fibrous dysplasia of the spine.

4.Irreducible Dorsal Dislocation of First Metatarsophalangeal Joint by Closed Method: Report of a Case

Traumatic dislocation of the first metatarso-phalangeal joint is rare injury. The mechanism of this injury is hyperextension of the great toe, causing displacement of the proximal phalanx on the dorsum of the first metatarsal head and neck. The complex soft tissue anatomy surrounding this joint interferes with the closed reduction of the dislocation. This paper describes a patient with complete dorsal dislocation of the first metatarsophalangeal joint treated at the Paik hospital which cannot be reduced by closed manipulation.

6.Effects of Steroids and vitamin D Derivatives on Differentiation and Proliferation of Cultrured Osteoblast Cells.

This study was designed to evaluate of the effects ofB-estradiol, progesterone, dexamethasone, hydrocortisone, calcitriol and alfacalcidol on the cell proliferation and differentiation in the rat osteoblast-like osteosarcoma cells ROS 17/2.8. The proliferation of ROS 17/2.8 cell treated with 1 M B-estradiol, 1 M progesterone, 1 M hydrocortisone, 1 M dexamethasone, 10 (-8) M calcitriol and 10 (-8) M alfacalcidol was decreased by 19%, 56%, 60%, 64%, 28% and 38% of control, respetively. Production of TGF-Bl by ROS 17/2.8 cells treated with B-estradiol, progesterone, hydrocortisone, dexamethasone, calcitriol and alfacalcidol was elevated by 30%, 18%, 60%, 48%, 405% and 452% of control, respectively. Alkaline phosphatase activity of ROS 17/2.8 cell treated with hydrocortisone, dexamethasone, calcitriol and alfacalcidol was increased by 3.4 times, 12.9 times, 10.0 times and 5.1 times of control, respectively, but with B-estradiol and progesterone decreased by 39% and 10% of control respectively. Northern blot experiments demonstrated that calcitriol and alfacalcidol increased mRNA levels of both osteocalcin and osteopontin.

9.A Clinical study of the High Tibial Osteotomy in Osteoarthritic Knees with Varus Deformity

During the period of 5 years and 5 months, from January 1979 to May 1984, we treated 27 cases(17 patients) of osteoarthritis with genu varum deformity by high tibial osteotomy. The results were as follows: l. In the 27 cases of the high tibial osteotomy, 24 cases were female and 3 cases were male. 2. Of the etiology of the osteoarthritis with genu varum, 14 female patients had primary one and 3 male patient knees were related with infection(2 patients) and trauma (1 patient). 3. Preoperative varus angles, ranging from 0° to 20°, 6.4° in an average and postoperative valgus angles, ranging from 5° to 15°, 8° in an averge. 4. In a review of the high tibial osteotomy, good and fair results were noted in 26 cases (96.3%) of the knees after average 2 years of follow-up. 5. The high tibial osteotomy is found to be useful method in the treatrnent of the osteoarthritic knee with varus deformity.

10.A Clinical Study of Open Tibia Fractures

The treatment of open tibia fracture is challenging to Orthopedic Surgeon with its high infection rate and other complications. We had wound culture of open tibia fracture at emergency room in all cases, and treated using various External Fixators. The author analized the 134 cases of the open tibia fractures from March 1979 to February 1987. The average follow-up was one year. The results were as follws : 1. Type I open fracture was 23 cases, Type II 58 cases, Type IIIA 33, Type IIIB and Type IIIC was 10 cases each. The most frequent type of fracture was Type II, 58 cases by Gustilo classification. 2. The union time of Type I fracture was 6.5 months in average, and that of Type II and Type III was 9.5 months, 11.2 months. 3. Of those 52 cases whose initial culture was positive, 12 cases developed infection. The low infection rate would probably be due to combined administration of cephalosporin and firm application of external fixator to severe open tibia fractures.
