We experienced 3 cases of sporotrichosis confined to the face. They were lymphatic type, pustular type and fixed ulcerative type of cutaneous sporotrichosis respectively. Two types of them except the lymphatic type were very difficult to be diagnosed correctly with clinical features. So, we reviewed the types of sporotrichosis and the problems in establishing the diagnosis. 3 cases are as follows: Case l. A 57 years old female second hand dealer had a ulceration measuring 2-2cm in diameter on the right dorsum of the nose after injuring with sharp rusty iron material. Soon after ulceration healed, 5 beaded nodules measuring 0.5-1cm in diameter newly developed on the right side of the maxillary area. Case II. A 55 years old female house wife had a suppurated ulcer with serous exudate, which looked like impetigenous lesions, on the left mandibular area, without any apparent trauma history. Case III. A 52 years old female had a rice grain sized papule, which was grown slowly in size, on the left maxillary area without any apparent trauma history. Skin graft was performed after excision of the nodule, because it was diagnosed as skin cancer at a local clinic. A large ulceration with necrosis of the tissues developed on the very site of skin graft performed, accompanied by a bean sized nodule below the ulceration.

PURPOSE: Cerebral contusion is often associated with delayed edema, ischemia, demyelination, and secondary impairment in the neurological function. Early prediction of the outcome for patients with diverse critical illnesses has long been a concern of intensive care unit physicians. Recently, the systemic release of host-derived inflammatory mediators has been used for prognosis assessment in patients with multiple infections, burn injury, and systemic infection. Experimental investigations have revealed the importance of free radicals and calcium currents in cellular damage. Also experimental injuries have caused an early production of cytokines. This study was undertaken to analyze the production of interleukin-6 in the brain and in plasma following a brain contusion. METHODS: Sixty male Sprague-Dawley (SD) rats, each weighing 300-350 g were used in an experimental group following brain contusion, and 18 SD rats were used as a control group following a sham operation. Intracerebral IL-6 and plasma IL-6 were measured by using the ELISA method with a rat IL-6 kit at 3, 6, 24, 48, and 72 hours after the brain contusion. After contusion, the brains were fixed by perfusion via the carotid artery with 40% formaldehyde, glacial acetic acid, and 100% ethanol (1:1:8) at a flow rate of 25 ml/min and stained with hematoxylin and eosin for histologic examination. RESULTS: Brain IL-6 levels increased to reach a maximum of 160.23 pg/ml, at 6 hour after brain trauma. Plasma IL-6 levels increased to 70.02 pg/ml at 3 hour following brain contusion. CONCLUSION: The elevated brain IL-6 level in the injured rat does not seem to reflect a systemic inflammation. Although plasma IL-6 is detected in the sham-operated and the traumatized rats, the levels are too low to account for the increase observed in the brain cortex. This finding shows that the increase in brain IL-6 is related to the dynamics of brain contusion.

Rheumatoid arthritis is a kind of collagen diease and involves many joints and extra-articular systems-heart, respiratory system, central nervous system, skin and reticuloendothelial system. The peripheral corneal ulcer is a rare chracteristic ocular finding in rheumatoid arthritis. Generally this ulcer is combined with minimal inflammation and vascularization. The local corticosteroid therapy is of no value and may be harmful. Collagenase inhibitor, hydrophylic contact lens. heparin therapy, limbal conjunctivectomy have been tried for treatment of this ulcer. The authors experienced a case of peripheral corneal ulcer in rheumatoid arthritis which responded to systemic corticosteroid therapy.

His presentation, characteristic magnetic resonance image findings, treatment, and 2-year follow-up result are described. The patient about this rare neoplasm of the skull base are also reviewed.

Pseudomonas scleral abscess is characterized by acute onset and rapid progression to scleral perforation, and has a subsequent risk of endophthalmitis. After control of infection with appropriate antibiotics, graft surgery should be performed for reinforcing the defected sclera. We performed a scleral homograft on the eye with large necrotic sclera caused by Pseudomonas aeruginosa infection after pterygium excisIon. And the results were remarkably good.

Granular corneal dystrophy is primary heredofamilial dystrophy of the cornea, which is characterized by the presence of plemorphic milk-white spots in the superficial stroma in the central cornea. This corneal dystrophy was first ,described by Arthur Groenouw in 1890. Since then. many other types of familial corneal dystrophies have been discribed by morphological characteristics. In 1938 BUcklers classified the corneal dystrophy into 3 types; granular, macular and lattice dystrophy, whose nemenclature is now generally accepted. Now we report 2 cases of granular corneal dystrophies which involved 2 sisters in a family.

189-192 Six male patients with typical symptoms of Behcet's syndrome were subjected to a clinical examination. One of these was a Chinese patient. The age when the disease started was between 20 to 39 years. All of them became blind but no one was dead.

Myoclonus epilepsy and ragged-red fiber (MERRF) syndrome is one of the common etiologies of progressive myoclonus epilepsy. The clinical features of MERRF syndrome are myoclonus, seizure, dementia, ataxia, neuropathy, myopathy, deafness, and lipouta. The patients with MERRF syndrome have a point mutation in mitochondrial DNA at 8344 or 8356 nucleotide. We are reporting a patient who developed myoclonus and seizure at the age of eighteen. He later showed cerebellar ataxia, peripheral neuropathy, and cognitive dysfunction. Skeletal muscle biopsy failed to demonstrate ragged-red fibers. He was diagnosed as MERRF syndrome by the mitochondrial DNA analysis. He had 86% mutant mitochondrial genomes (A-)G(8%) mutation) in leukocytes, and his asymptomatic mother had 66%. The absence of ragged-red fibers does not rule out the possibility of MERRF syndrome. Demonstration of mitochondrial DNA mutation is the most convincing method for establishing the diagnosis of MERRF.
