2.Two Cases of partial Lipoatrophy.

Partial lipoatrophy is a relatively rare disease characterized by depression in certain areas of body due to absence of subcutaneous fat tissue. In the typical case, the face is affected first, and gradually spreads to the upper half of the body. Recently, Imamura et al reported an atypical form of partial lipoatrophy characterized by involvement of abdomen and neighboring regions and centrifugal enlargement of the lesions with re4dish and scaly change in the surrounding area. They proposed. the name lipodystrophia centrifugalis a,bdominalis infantilis for this disorder. The authors reported two cases of partial lipoatrophy sirnilar to the cases of Imamura et al. Case 1 was 5 year-old female with depressed Iesions on the lower abdomen, inguinal areas, and around the external genital and perineal areas. Eliopsy specimen showed absence of subcutaneous fat tissue. Case 2 was 5 year-old. female with depressed lesions on the left lower abdomen and inguinal region Riopsy specimen showed decrease in subcutaneous fat and inflammatory infiltrates in the dermis and subcutis.

3.A Case of porphyria Cutanea Tarda.

Porphyria is a rare metabolic disorder in this country and a few cases of acute intermittent porphyria has been reported. We observed a case of porphyria cutanea tarda associated with liver cirrhosis. The patient was 61-year-old farmer with heavy alcoholic habit. He had been suffered from skin fragility and photosensitivity for 3 years. His face color was slate blue and sclerdermoid appearance noted especially on the cheek. Bullae, which is healing slowly and followed by atrophic pigmented scars, were developed on the dorsum of hand and feet after receiving trivial trauma and massive alcohol intake. None of his family members has similar symptoms. Urine specimen showed port-wine coIor and fluoresced pinkish under the Wood's light. Serum iron level was markedly elevated (400 microgram%). The other abnormal findings of liver function test were BSP retention (16%/45min.), elevated SGOT (198 unit) and SGPT (80 unit) levels. Esophagram revealed suspicious of varices and liver scanning showed cirrhotic changes. Skin biopsy specimens taken from the cheek and dorsum of hand showed sclermoid changes and subepidermal bulla. Liver biopsy disclosed mild degree of cirrhotic changes.

4.Statistical Survey of Herpes Zoster and It's Treatment with Sublesional Injection of Predinisolone - procaine.

The statistical survey on 148 patients with herpes zoster was made. Among 148 patients, 78 were treated with sublesional injection of predinisolone-procaine for severe neuralgic pain. 1) The incidence of herpes zoster was increasing with age, reaching it's height at age of 41-60 group and declining thereafter (Fig. 1). 2) The ratio of severe neuralgic pain of herpes zoster to the total incidence of every age groups was increasing with age, being 23.5% in the youngest age group under 10 and 85.7% in the eldest age group between 71-80 (Table 1). 3) The ratio of zoster peetoralis to the total incidence was 70% and a case of generalized herpes zoster associated with lymphocytic leukemia was observed. 4) 78 cases of herpes zoster suffering from the severe neuralgic pain were treated with sublesional injection of predinisolone-proicaine. The effect was excellent, shortening it's eours and relieving pain. There were so complications of hematogenous dissemination, skin infection and atrophy after the injection, except one case of hematogenous dissemination in lymphocytic leukemia.

5.A Clinical Study of Androgenetic Alopecia (III).

BACKGROUND: Androgenetic alopecia is considered to be a genetically determined disorder influenced by age and androgen. The proportion of patients with androgenetic alopecia among the total number of patients with alopecia seems to be gradually increasing. OBJECTIVES: The purpose of this study is to evaluate the family history,clinical and endocrine status of the patients with androgenetic alopecia. METHODS: 1113 patients with androgenetic alopecia who had visited the Department of Dermatology, Yongsan Hospital, College of Medicine,Chung-Ang University during the 3 years (1995.1-1998. 12) have been examined. RESULTS: The results are summarized as follows 1) The incidence of androgenetic alopecia among the total number of alopecia patients was 64.5%, showing recent increment. 2) There were 855 male and 258 female patients being most prevalent in the third decade in both sexes and the patients younger than 30 years old with premature androgenetic alopecia,made up 70.3% of the male patients and 48.8% of the female patients with androgenetic alopecia. 3) While Norwood's type Iia was the most common and following type II, III vertex,and IV in the male AGA, Ludwig's type II was the most common in female AGA 4) There was a family history of baldness in 53.5% of first degree relatives in male patients and 51.6%in female patients. 5)Associated diseases were observed in 565(66.8%) of the male patients and 219 (84.8%) of the female patients:diseases associated with androgen such as seborrheic dermatitis and acne vulgaris occupied 39.1%. CONCLUSION: Based on our findings, those who want to treat androgenetic alopecia at the earlier ages are gradually increasing and it seems to be reasonable to believe that the age, genetic factors, localized effects of androgens on the scalp and the density and/or functional activity of androgen receptors may influence the pathogenesis of androgenetic alopecia.

7.Choroidoretinal Vascular Anastomoses After Chorioretinal Rupture.

Numerous cicatrizing and granulomatous diseases that simultaneously involve choroid, retina and their intervening Bruch's membrane may cause choroidoretinal vascular anastomoses. Anastomoses at capillary level and in neovascular granulomatous tissue are probably not uncommon under these and related circumstances, but choroidal connections to large-caliber segments of otherwise normal retinal arterioles and venules are unusual. We retrospectively studied 44 eyes of 44 patients with choroidal ruptures after blunt trauma. Of 44 choroidal ruptures, three patients had chorioretinal ruptures and choroidoretinal vascular anastomoses. This report describes three cases of large-caliber choroidoretinal anastomoses after chorioretinal ruptures.
8.A Case of Multiple Pilomatricoma.

The pilomatricoma (Calcifying Epithelioma of Malherbe) is rare benign tumor of adnexal keratinocytes. It is occuring usually as a single, asymptomatic, 0. 5 cm to 3. 0 cm sized, deep seated, firm nodule, covered by normal or pink skin. Multiple pilomatricoma is relatively rare, as 3. 5% of reported cases of pilomatricoma. A 21-year-old male patient had 7 subcutaneous nodules on the neck, both upper extremities and trunk for 9 years. We excised one of them, under the impression of calcinosis cutis, cysticercosis cutis or benign skin tumor. But histopathologic findings of the lesion showed the typical pattern of pilomatricoma with shadow cells, calcification and foreign body giant cells. Five of remainders were excised and they showed same histopathological findings of pilomatricoma. The authors presented herein one case of multiple pilomatricoma with the review of literature.

9.Atypical Case of Granuloma Annulare.

Granuloma annulare is a benign, usually self-limited chronic dermatasia of unknown cause, charaeterized by akin colored papules that may be arranaged in an annular configuraticn. Histopathologically, it is charicterized by camplete or incomplete foeal degeneration of collagen of chronic infliammatory cells. The authors obaerved an atypical case of granuloma annulare in a 4-year-old girl. She showed multiple papules and nodule on hands and feet. On inital examination, her skin lesions suggested histiacytosis, xanthanulama, rheumatoid nodule, or benign skin tumor. But the skin biopey revealed typica1 findings of granuloma awulare. Focal degeneration of colagen was confirmed by toluictine blue stain. Her skin lesions were almoatly cleared by skin biopsy and intralesional injectian of corticosteroid.

10.A Case of Pityriasis Lichenoides et Varioliformis Acuta.

The nail has been considered to be the best human model to evaluate the general body state at all ages. The present study was disigned to measure the nail growth rate and to define the correlation of them with other values of nail size. The subjects were 373 males and 812 females whose ages from R months to 79 years. The results were as follows: 1. The linear elongation of the nails were most rapid in the 14 to 15 year-old age group. It decreased steadily as the subjects got older and olr1er. 2. Sex differences in the nail growth rate were not statistically significant. R. Where were no statistically significant differences in the linear growth rate of nails between right and left hands. 4. The middle finger nail grew more quickly than others, while the thumb and little finger nails grew more slowly. 5. Linear growth of nails were not significantly correlated to the thickness and length of nails and the length of lunulae.
