1.Studies on Blood Group Isoantigens in Uroepithelium -Determination of Isoantigens in Non-Malignant Epithelium of Tumor-Bearing Bladder-.

A and B blood type isoantigens were investigated in 17 cases surrounding bladder cancer by means of the specific red cell adherence test (SRCA). The tissue specimen were classified into positive, intermediate and negative groups according to SR CA reaction of the primary tumors. The histologically normal epithelium was SRCA-positive in 86.2 % of the specimens in the positive group and 70.0% in the negative, group; for hyperplastic epithelium it was 76.4 % and 50.0% and for the dysplastic epithelium it was 60.0% and 53.3 % respectively. The results may indicate that dedifferentiation expressed as the loss of blood group isoantigens has occurred in the histologically benign epithelium of the tumor-bearing bladders and that its occurrences is frequent in the bladder, associated with SRCA-negative tumors.

2.Operative Treatment of Chronic Unreduced Dislocation of the Shoulder.

We experienced four patients that had gone unreduced for at least eight weeks and evaluated the results of the operative treatment in these paitents. All four cases were anterior dislocation, and among them three cases were dislocated by direct trauma and one was dislocated during seizure attack. Pain was moderate in one case and mild in three cases. In the preoperative range of motion, the average passive tlexion at the scapular plane was 143 degrees (90 degrees - l70 degrees) and average external rotation at the side was 33 degrees(5 degrees-60 degrees), but the average active flexion was 103 degrees, active external rotation was 16 degrees and the internal rotation at the back was the sacral level except one. All four patients complained of severe limitation of daily living activity due to limitation of motion and weakness and could not occupy their jobs. In all four cases, three dimensional CT scan was performed to evaluate glenoid and humeral bone defect and for preoperative planning. Operation was performed at 8 weeks after initial dislocation in two cases, 1 year later in one case and 2 years later in one case. Open reduction was done and the defect of humeral head was filled with osteochondral allograft in two cases. Fractured humeral head fragment was reduced and fixated with non-ahsorhahle suture in one case and prosthetic head replacement was performed in the other. As a result, three patients had a mild or moderate pain postoperatively and moderate functional restrictions in the activities of daily living, and they were considered to be unsatifactory. Two cases were subluxated during the follow-up. These results show that the overall prognosis for the surgical treatment of the chronic unreduced dislocated shoulder is unpredictable and unacceptable.

7.The analysis of patient visit pattern in a university hospital before and after implementation of health care delivery system.

BACKGROUND: The implementation of health care delivery system(HCDS) in 1989 affected the visit pattern of tertiary care hospital. In this study we analyzed the effect of implementation of HCDS in a university hospital. METHODS: We analyzed the yearbook of a university hospital from 1986 to 1992. The clinical departments were divided into three groups of medical departments, surgical departments with application of HCDS and clinical departments without application of HCDS. RESULTS: The total outpatient visits increased after implementation of HCDS. The new outpatient visits decreased after implementation in the medical departments and surgical departments but re-visit patients increased in all clinical departments. The number of emergent patients increased at year 1889, but decreased in 1990, then little changed. Admission rate via emergency room decreased in surgical departments and clinical departments without application of HCDS. The patient from Pusan in which hospital is located was little changed but patients that from other areas increased. So the proportion of patients from Pusan decreased, and from other regions increased. The visit of patients with self pay decreased, but patients with medical insurance increased. CONCLUSIONS: In spite of increased total outpatient visits, the new outpatient visits and patient visits from near place decreased through implementation of HCDS. The new outpatient visits decreased in the medical departments and surgical departments in clinical departments with application of HCDS, but the new outpatient visits increased in clinical departments without application of HCDS.

9.Arthroscopic Treatment for Calcific Tendinitis of Origin of Long Head of Triceps.

A 55-year-old female experienced acute left shoulder pain without specific trauma. Radiography showed calcific deposits in the inferior part of the glenoid fossa. Magnetic resonance arthrography showed calcific deposits in the origin of the long head of triceps brachii muscle. Conservative treatment failed to resolve the symptoms; therefore, arthroscopic surgery was performed. The patient experienced immediate and dramatic pain relief, and normal shoulder motion was demonstrated 1 year after surgery. In conclusion, although rare, calcific tendinitis of the triceps brachii muscle, which causes shoulder pain, should be included in the differential diagnosis of acute shoulder pain. Arthroscopic surgery is a treatment option for chronic cases and those resistant to conservative treatment.
