5.Studies on Uric Acid Disorders in Patients With Upper Urinary Calculi.

We studied tire causative diseases and serum concentration and daily urinary excretion of uric acid in 115 cased of radiopaque upper urinary stone formers. And with the aim of establishing methods for the prevention of recurrent stone formation, allopurinol and/or sodium bicarbonate was administered to hyperuricemia and/or hyperuricosuria. The following results were obtained. 1. 25 of 115 cases having uric acid abnormality in serum and/or urine, it was some correlation on between hyperuricemia and/or hyperuricosuria and calcium formers. 2. 14 of 25 cases administering allopurinol and/or sodium bicarbonate after lithotomy had not recurrent stone formation for 1-2 years. But of 11 cases receiving medical treatment alone only 2 cases were spontaneously evacuated the stones, 3 cases were decreased in number of stones, 6 cases were not changed. 3. After administration of allopurinol serum uric acid, daily urinary uric acid daily urinary calcium and daily urinary inorganic phosphorus value were significantly decreased but serum calcium and inorganic phosphorus were not significantly changed. These results suggested that allopurinol was effective or prevention of recurrent stone formation, even through our studies were relatively short period and not many cases.

6.Experience of Marshall-Marchetti Operation in Stress Incontinence.

The stress incontinence is a disease commonly developing in multipara and occurs by deficiency of estrogen and relaxation of pelvic organ generally after menopause but it is possible to occur before menopause by change in the structure of pelvic organ. Herin, we report 1 cases of stress incontinence, one nuliparous in pelvic trauma and remaining 3 cases in multipara. The diagnosis of stress incontinence was made by history, Marshall-Marchetti test and chain cystography. All cases underwent Marshall-Marchetti operation. After the operation, symptoms were improved in all cases and they have had satisfactory in usual life for 6 months to 4 years.

9.A Clinical Analysis of the Breast Masses by Biopsy.

BACKGROUND : A discussion of the chronologic trends of operative indications of breast masses for proper management of breast masses is desirable. METHODS : A retrospective analysis of 882 patients with breast masses who underwent operations at Kangnam General Hospital from January 1986 to December 1997 was done. All the lesions were classified according to for age, size of the mass, and the pathologic result. Concurrently, the distributions of the different masses were retrospectively analyzed for each one-year time period. RESULTS : The most prevalent age group was the third decade (28.6%) followed by the fourth decade (25.3%) and the fifth decade (24.4%). The most common lesion was fibroadenoma (38.7%), followed by mammary dysplasia (22.6%) and malignancy (18.3%). In fibroadenoma, the most common pathologic type was intracanalicular (34.9%), and the juvenile type occurred in significantly younger patients compared to other types (19.2 vs 29.2 years of age). In mammary dysplasia, the most common pathologic type was fibrous (74.4%), and the epithelial hyperplasia and adenosis types were occurred in older patients compared to the other types (39.1 and 41.0 vs 33.5 and 35.4 years of age). As the chronologically changing pattern of the pathologic types, the number of mammary dysplasia had a decreasing tendency and fibroadenoma had a relatively increasing one, and the reason may be the effect of fine-needle aspiration cytology. CONCLUSIONS : The result of this study shows that the distribution of the disease in terms of age and frequency is almost the same as that of other reported results. The trend of the disease types in terms of the time period shows that the operation rate for fibroadenomas is increasing while the rate for mammary dysplasia is decreasing. This tendency might come from the decreasing use of surgical biopsy for mammary dysplasia.

10.A Case of Behcet's Disease with Ileal Ulcer Penetrated to the Duodenum.

Behcets syndrome is a multisystem disorder presenting with recurrent oral and genital ulcers as well as uveitis, Gastrointestinal involvement associated with ileal ulcer is rare, but its outcome may by fatal. We have recently experienced a 43-year-old man who had Behcets syndrome with ileal ulcer penetrated to 3rd portion of the duodenum. He had been updergone right hemicolectomy and ileocolostomy for ileal inflammation of unknown etiology 20 years ago. Thereafter he had had relapsing oral and genital ulcers, and abdominal pain intermittently. From 15 days ago before visiting our hospital, he had severe abdominal pain and dark blood-colored stool. The colonofiberscopic finding revealed a large, deep penetrating ulcer and multiple shallow ulcers on the ileum. Deal segmental resection, gastro-jejunostomy and jejunal patch at 3rd portion of the duodenum were performed with marked improvement.
