BACKGROUND: Female diffuse hair loss is a common baldness with a multifactorial origin. In recent studies, it has been suggested that reduced or depleted iron stores are a cause of female diffuse hair loss. OBJECTIVE: We performed this study in order to elucidate the pathogenesis of female diffuse hair loss and to evaluate the differences in serurn iron levels between normal females and those with diffuse hair loss in Korea. METHODS: Blood levels of hemoglobin, iron, TIBC, ferritin, testosterone, thyroid function, ANA and Zn were examined in 30 patients with female diffuse hair loss and 20 normal females. RESULTS: 1) Serum Zn, testosterone, T4, TSH of the patient group were not statistically significant compared to those of the control group (p>0.05). 2) TIBC values were significantly increased, but serum ferritin levels were signifieantly decreased in the patient group (p<0.05). There were no significant differences in hemoglobin, serum iron in both groups (p>0.05). 3) No significant differences between clinical severity and the values of serum Zn, testosterone, T4, TSH, TIBC, serum femtin, hemoglobin and serum iron were observed (p>0.05). CONCLUSION: These findings suggested that the variations of TIBC, serum femtin levels may play a role in the pathogenesis of female diffuse hair loss, but further studies will be needed for evaluation of this condition.

A case of a 21-year old female with Maffucci's syndrome is presented where the patient developed multiple cutaneous spindle-cell hemangioendotheliomas. She had multiple pea to walnut sized, non-tender, normal skin colored or slight bluish nodules and bony mass-like lesions on the left upper extremity and hand. A histological examination of the lesions showed that they were composed of irregularly dilated, thin walled cavernous blood spaces containing phleboliths and collapsed vascular spaces separated by spindled fibroblastic cells. Radiologically, the bony lesions showed radiolucent densities with calcified spots within the second proximal phalanx and metacarpal bone of the left hand, which were consistent with enchondromas.

Tuberculosis verrucosa cutis is caused by the external inoculation of Mycobacterium tuberculosis into the skin of a person who already has a significant degree of cell mediated immunity to this organism. This form of cutaneous tuberculosis is less common. A 29-year-old male had a solitary adult palm sized, verrucous surfaced scaly plaque for about 10 years. The lesion was excised about 10 years ago but gradually increased in size. At first we suspected a deep mycosis and treated him with anti-fungal medication for 3 months. However, the skin lesion did not improve. He was then treated with anti-tuberculosis medication for 9 months under the impression of tuberculosis verrucosa cutis. The skin lesion was almost healed and no recurrence has been observed for 8 months after the treatment.

Mandibular condylar fracture is common in mandibular fractures. Unlike other facial, skeletal fractures, most of mandibular condylar neck or head fractures are treated with closed reduction and subsequent functional therapy is essential for preventing complications including ankylosis, arthrosis and growth disturbance. From January, 2000 to September, 2002, we have treated 15 cases of mandibular condylar fractures with closed reduction by using functional appliance with bite block. Among these cases, we report a case of 14-year-old female with mandibular condylar neck fracture, resulted in good clinical and radiographic progress.

We investigated the 1,004 workers who worked in a automobile factory to study the epidemiologic characterist of dermatoses due to cutting oils. Among the workers, 667(66.4%) answered the questionnaire. They are belong to 5 departments of the factory-the Engine-Work(86), Power train Assembly(17). We measured the oil mist concentration in air of the departments and examined the workers who had dermatologic symptoms. The results were follows; 1) Oil mist concentration; Of all measured points(52), 9 points(17.2%) exceeded 5mg/m3-the time-weighed PEL- and one department had a upper confidence limit(95%) higher than 5mg/m3. 2) Dermatologists examined 213 workers. 172 of them complained any skin symptoms at that time-itching(32.5%), papule(21.6%), scale(15.7%), vesicle(12.5%) in order. The abnormal skin site found by dermatologist were palm(29.3%), finger & nail(24.6%), forearm(16.2%), back of hand(8.4%) in order. 3) As the result of physical examination, we found that 160 workers had skin diseases. Contact dermatitis was the most common; 69 workers had contact dermatitis alone(43.1%), 11 had contact dermatitis with acne(6.9%), 10 had contact dermatitis with folliculitis(6.3%), 1 had contact dermatitis with acne & folliculitis, and 1 had contact dermatitis with abnormal pigmentation. Others were folliculitis(9 workers, 5.6%), acne(8, 5.0%), folliculitis & acne(2, 1.2%), keratosis(1, 0.6%), abnormal pigmentation(1, 0.6%), and non-specific hand eczema(47, 29.3%). 4) The prevalence of any skin diseases was 34.0 per 100 in cutting oil users, and 13.3 per 100 in non-users. Especially, the prevalence of contact dermatitis was 23.0 per 100 in cutting oil users and 4.3 per 100 in non-users. 5) We tried patch test(standard series, oil series, organic solvents) on 49 patients to differentiate allergic contact dermatitis from irritant contact dermatitis and found 20 were positive. 6) In a multivariate analysis(independent=age, tenure, kinds of cutting oil), the risk of skin diseases was higher in the water-based cutting oil user and both oil user than non-user or neat oil user(odds ratio were 2.16 and 2.78, respectively). And the risk of contact dermatitis was much higher at the same groups(odds ratio were 5.16 and 6.82, respectively).

Regular hemodialysis has been established as a way of treatment for end stage renal failure. In adults, hemodialysis 5 hours each time, three times weekly with one square meter hemodialyzer is now widely accepted as a standard, and there are many reports on long term follow up studies. Quite a large difference are present, however, in our community mainly originated from patient's poor compliance in frequent dialysis with short interdialysis interval. The author analyzed 200 cases of chronic renal failure who have received hemodialysis treatments during the 5 year period from July 1976 to June 1981 at the hemodialysis unit of the hanyang University Hospital and the following results are obtained; 1. Actual one year survival rate was much higher in thrice(87.1%) than one(35.1%) or twice(54.1%) weekly dialysis. 2. Long term(more than 6 months) complications were also more frequent in once(85.7%) or twice(70.2%) weekly treatments than in thrice(22.2%). The predominant complications were congestive heart failure, pericarditis, and infections. 3. Those who recieved three times weekly dialysis had better rehabilitation grades than the patients groups of twice or once weekly treatment. 4. Of peridialysis distressful symptoms, nausea, vomiting dizziness were less frequent in the group three times a week dialysis. In contrast, headache, hypotension, muscle cramps, and weakness were not significantly related with frequency of hemodialysis. 5. Those who voided a large amount of urine output had better survival and less frequent requirement of blood transfusions. 6. Main causes of death were due to patient poor compliance, hyperkalemia and cerebrovascular acidents. 7. Economic problems were the major cause of dialysis interruption.

The normal criteria of the ankle joint laxity of the Korean may be different from that of the western. For the objective study, we designed two devices for the anterior drawer test and inversion stress test. Using the devices we radiologically examined the 120 ankle at neutral and 30 plantar flexed position. The following results were obtained; l. In the talar tilt angle, the minimun angle, the maximun angle and the mean were 1.0°, 12.0°, 3.7° respectively at the neutral position and 1.0°, 19.0°, 4.8° respectively at the 30° plantar flexed position. 2. With the Grace method, the minimum distance, the maximum distance and the mean were 3.Omm, 12.0mm, 5.2mm respectively at the neutral position and 3.0mm, 12.6mm, 6.1mm respectively at the 30 plantar flexed position. 3. With the Kelikian method, the minmum value, the maximum value and the mean were Omm, 7.5mm, 2.2mm respectively at the neutral position and Omm, 8.0mm, 2.5mm respectively at the 30 plantar flexed posion. 4. There was no difference in laxity for the age and sex.

Percutaneous biliary drainage is an effective new nonsurgical method for combined external/internal catheterdecompression of obstructed extrahepatic bile ducts. Molnar and Stockum, and more recently others, have reportedsucdess utilizing this approach to establish biliary drainage in patients with obstructive jaundice. Percutaneousbiliary drainage was performed in 9 patients with obstructive jaundice. Internal drainage was accomplished in 4patients (44.4%), and external drainage in 5 patients (55.6%). Major complications occurred in 2 patients; bileleakage, one; sepsis, one. Minor complications occurred in 7 patients; mild or high fever, 5 cases; transienthemobilia, 2 cases. The catheter management problems encoutered including pain, catheter dislodgement ormigration, lumen occulusion, and external bile leakage.

There are three different kinds of aphakic corrections as glasses, contact lens and intraocular lens. In this paper we analyse the 20 aphakic patients who had operations during the past four years in our hospital by the intracapsular cataract extraction technique. 10 patients(50%) are using aphakic glasses and 8 patients(40%) are contact lens wearers and remained 2 person are non-corrected monocular aphakics. Among the CL wearers most of them have occupations. Glass wearers are increased in number parallel with the increasing their ages. Most of the aphakic patients have good visual corrections and satisfied with their present corrective measures.
