2.A Case on Risperidone-Induced Tardive Dyskinesia.

OBJECTIVE: Base on clinical practice, the authors report a case of tardive dyskinesia arising during the course of treatment with resperidal. METHODS: This article was review and analysis of a case on risperidone-induced tardive dyskinea. RESULTS: Mrs K, a 51-year-old women with a 1-year history of schizophrenic disorder, gradually developed tardive dyskinetic movement of the mouth, lip, and tongue over a 4 month period(From July 1996 to June 1997) while taking risperidone. Initially she was treated with haloperidol and alprazolam. However, the haloperidol was subsequently discontinued because of EPS developed. From 11th March 1997, she was observed to have a severe form of tardive dyskinesia involving her tongue, lip, and mouth. After risperidone was withdrawn at 9th May 1997, her tardive dyskinetic movement was disappeared. CONCLUSIONS: This is, to our knowledge, the first report of the onset of tardive dyskinesia in a patient taking risperidone. However, additional controlled studies of specific questions are needed ; e.g., the dose-response curves for produce tardive dyskinesia and the mechanism of producing risperidone-induced tardive dyskinea and so on.

3.Treatment of Infectious Endophthalmitis.

Endophthalmitis is a rare but catastrophic complication of intraocular surqeries and penetrating injuries. In spite of advances in diagnosis and therapy, the overall prognosis for recovery of useful vision has been regarded as poor. We experienced eight cases of endophthalmitis and performed vitrectomy combined with intravitreal antibiotics. Four(50%) had a final isual acuty of 0.2 or better. Vitrectomy was performed in cases of advanced intraocular inflammation, and intraocular lens was removed for suspected fungal endophthalmitis or to facilitate performance of a vitrectomy. A sample of vitreous, diluted by the irrigating solution, was passed through a disposable membrane filter system and inoculated into the culture media. Modification of therapy after initial broad spectrum antibiotics was based on the culture results and clinical responses. Repetitive intraocular antibiotic injections or reoperation were performed in severe cases.

5.Effect of Nitric Oxide on the Viability of Bone Marrow - Derived Cultured Mast Cells.

It is well established that mast cell proliferation and maturation are regulated by two principle cytokines, IL-3 and the c-kit ligand stem cell factor (SCF). Previous reports have demonstrated that bone marrow-derived IL-3-dependent mast cells exhibit the characteristic apoptosis on removal of IL-3. To know how the number of mast cells is controlled, we observed the effects of nitric oxide (NO) on the murine bone marrow-derived cultured mast cells (BMCMC). Apoptosis was measured by the analysis of flow cytometric data and electrophoretic evidence of DNA fragmentation. Our data showed that sodiurn nitroprusside (SNP)-a NO releasing substance- induced apoptosis in BMCMC. Cell cycle analysis showed that the number of the G,/G, and S phase decreased markedly, while the percentage of cell in G,/M phase was increased. Also, SNP alone induced cell death, whereas SNP in combination with SCF markedly decreased cell death of BMCMC. SNP-induced apoptosis was partially inhibited by the treatment of BMCMC with SCF. Our results suggest that NO might have sorne role in the regulation of the number of mast cells.

6.A Statistical Analysis of Fluorescein Angiographic Findings in Central Serous Retinopathy.

The author investigated on the ratio of central serous retinopathy (C.S.R.) cases among total out patients, the distribution of age and sex, and sites of affected eye in 214 cases of C.S.R. from March 1st 1977 to February 28th 1981. Fluorescein angiography was performed in 132 eyes of 108 cases among them. Each type according to the leaking pattern on fluorescein angiography were characteritically classified as followings: Type A; Leaking point is unremarkable (C.S.R. clinically). Type B; Leaking point appears at early arterial phase and increases in density gradually but the size of leaking area is not enlarged. Type C; Leaking point appears at early arterial phase and increases concentrically in its size and density. Type D; Leaking point appears at early arterial phase and increases vertically (mushroom shaped) in its size and density. Type E; Leaking point appears at early arterial phase and gradually fades out (scar formation). The results were obtained as follows: 1) The eyes of C.S.R. were observed in 0.83% among total out patients. 2) As to the distribution of age, 5th decade was most frequantly affected in 37.3%. 3) As to the distribution of sex. male was observed in 80.8%. 4) Among 214 cases, unilateral involvement was observed in 85.1% without difference of both eyes and binocular involvement was nbserved in 14.9%. 5) Type C in leaking pattern was most frequantly observed in 25.8%. 6) A single leakage was observed in 59. 1% of all eyes. 7) The site of leaking point of fluorescein was found most frequantly in the upper area. 8) The direction of diffusion in leaking pattern was most frequantIy upward in type C, and outward in type D. 9) The detached size of type C and D were more larger than other types. 10) Comparing the onsets of each type, type A, B, C and D could be seen within 3 months, and type E could be found over 3 months. 11) In general, the visual acuity was severely disturbed in type D rather than the other types. And the visual disturbance was remarkable in the case of large serous detachment.

7.Five-year Experience of Extracorporeal Life Support in Emergency Physicians.

BACKGROUND: This study aimed to present our 5-year experience of extracorporeal cardiopulmonary resuscitation (ECPR) performed by emergency physicians. METHODS: We retrospectively analyzed 58 patients who underwent ECPR between January 2010 and December 2014. The primary parameter analyzed was survival to hospital discharge. The secondary parameters analyzed were neurologic outcome at hospital discharge, cannulation time, and ECPR-related complications. RESULTS: Thirty-one patients (53.4%) were successfully weaned from extracorporeal membrane oxygenation, and 18 (31.0%) survived to hospital discharge. Twelve patients (20.7%) were discharged with good neurologic outcomes. The median cannulation time was 25.0 min (interquartile range 20.0-31.0 min). Nineteen patients (32.8%) had ECPR-related complications, the most frequent being distal limb ischemia. Regarding the initial presentation, 52 patients (83.9%) collapsed due to a cardiac etiology, and acute myocardial infarction (33/62, 53.2%) was the most common cause of cardiac arrest. CONCLUSIONS: The survival to hospital discharge rate for cardiac arrest patients who underwent ECPR conducted by an emergency physician was within the acceptable limits. The cannulation time and complications following ECPR were comparable to those found in previous studies.

8.The effect of Decorin, TGF-beta antagonist, on wound healing in rabbits.

There have been many trials to decrease the scar formation followed by wound treatment. TGF-beta plays a important role on wound healing in adult. Therefore the repression of TGF-beta expression will be helpful to decrease scar formation. Decorin is known to competitively inhibit TGF-beta expression. Decorin were subcutaneously administered in surgical wounds in rabbits to investgate the preventing effect of scar formation for clinical application. Histologic findings of wound healing progresses is similar with control and experimental group at 2week. 2.5 microgram decorin of administrated group was similar to those of control group at 4 and 8week. In wound healing process 10 microgram decorin of administrated groupsat showed that thickness of immature collagen fibers(scar) was decreased as compared with control group at 4, 8 weeks. 20 microgram decorin of administrated group showed similar histologic features to 10 microgram administrated group. The wounds of 8week experimental group(10, 20 microgram) were completely recovered to the normal surrounding skin tissue including sweat gland and hair follicle. These results suggested that decorin can be of help to the prevention of local scar formation.

9.A Case of Posttraumatic Ectopic Nail.

Ectopic nail is characterized by growth of nail-like tissue in a location other than the nail bed. It is an extremely rare disorder that can be either congenital or acquired from incidents such as trauma. An 8-year old female presented with a complaint of a nail-like lesion on the dorsal surface around the center of the right fifth toe. It had been present for 3 years, while the patient continuously clipped it upon growth. The fifth toe was initially injured by a chair that had fallen on that toe 4 years prior to her visit. There was no associated bony deformity or other physical changes in the nail. A clinical diagnosis of ectopic nail was made and it was surgically excised under local anesthesia. The histopathological examination revealed a normal nail unit and there was no recurrence during the 6 month follow-up period.

10.Two Cases of Giant Corneal Dermoids.
