2.A Case on Risperidone-Induced Tardive Dyskinesia.

OBJECTIVE: Base on clinical practice, the authors report a case of tardive dyskinesia arising during the course of treatment with resperidal. METHODS: This article was review and analysis of a case on risperidone-induced tardive dyskinea. RESULTS: Mrs K, a 51-year-old women with a 1-year history of schizophrenic disorder, gradually developed tardive dyskinetic movement of the mouth, lip, and tongue over a 4 month period(From July 1996 to June 1997) while taking risperidone. Initially she was treated with haloperidol and alprazolam. However, the haloperidol was subsequently discontinued because of EPS developed. From 11th March 1997, she was observed to have a severe form of tardive dyskinesia involving her tongue, lip, and mouth. After risperidone was withdrawn at 9th May 1997, her tardive dyskinetic movement was disappeared. CONCLUSIONS: This is, to our knowledge, the first report of the onset of tardive dyskinesia in a patient taking risperidone. However, additional controlled studies of specific questions are needed ; e.g., the dose-response curves for produce tardive dyskinesia and the mechanism of producing risperidone-induced tardive dyskinea and so on.

3.The Lumbar Discogram Incidental to the Operation: Analysis of 35 Cases

Since the time when the lumbar discogram was used as the clinical diagnostic method for the first time in 1952, many problems have been pointed out to this method, and moreover, in presentday tendencies for the diagnostic method, the lumbar discogram is carried on even useless by majority. One of the important reasons is that so many studies for the lumbar discograms have reported so many false positive lumbar discograms at the high ratio. Hower we found a certain usefulness of the lumbar discogram incidental to the operation. The purposes of our operative lumbar discogram are those; 1) to observe the radiological configuration of the lumbar discogram. 2) to measure the amount of the the dye injected into normal and abnormal lumbar discs, especially in our Koreans. 3) for confirmation of the diagnosis when the myelographic finding is suspicious. 4) for confirmation of the Ievel especially when the number of the possible lesion is more than one. We investigated and analyzed the 35 lumbar discograms of 26 patients and compiled the statistics.

5.Revisit the Original Whitehead Hemorrhoidectomy The postoperative results of W-shaped circular incision & preservation of perianal skin.

Since Whitehead had described a circular hemorrhoidectomy in 1882, many surgeons adopted it for decades for patients with protruding anal deformity. After a few decades of performing Whitehead operation, devastating complications such as anal stricture, fecal incontinence, and wet anus with mucosal eversion had been reported on the literatures and then it was buried as an abandoned procedure by surgeons for a long period. Recently, a few prominent anal surgeons reported that they could avoid such devastating complications by introducing diverse modifications of the original Whitehead's operation. The authors analyzed 22 patients who had undergone original Whitehead circular hemorrhoidectomy with the technique of preserving most of the perianal skin and W-shaped circular incision during the period from 1991 to 1996, with special regard to the com plications such as anal stricture and anal mucosal eversion which have been debated on so far and reviewed the articles about these issues. In immediate postoperative period, suture failure and resultant non-surgery requiring, mild anal stricture were documented in 3 of the 22 cases(13.6%). On long-term follow-up with the mean period of 44 months (18~79 months) in 14 cases, except those 8 cases that were lost, with phone-call questionaire, 13 patients(93%) had quite normal anal functions. The authors would like to suggest that the original Whitehead's circular hemorrhoidec tomy is a valuable surgical technique to manage the protruding anal deformity if surgeons can avoid well known complications such as anal stricture and anal mucosal eversion by choosing a correct location of initial W-shaped incision to preserve as much perianal skin as possible.

7.Taxonomic Studies on Alternaria in Korea (1).

The taxonomy of the Alternaria spp. has been a subject, of controversy because of their high variability in conidial morphology and polymorphism displayed even in pure cultures. The published Korean literature on the genus Alternaria is scattered and fragmentary, and pertains to about 25 species with special emphasis on occurrence and pathogenicity, but mycological studies on this group of fungi are insufficient. This is the first series of detailed and consolidated account of Korean species of Alternaria, which includes 11 species; viz., Alternaria brassicae, A. brassicicola, A. cinerariae, A. dauci, A. dianthi, A. dianthicola, A. helianthi, A. helianthinffciens, A. iridicola, A. japonica and A. protenta. Detailed diagnostic descriptions, specific characterizations, taxonomic discussions and illustrations for each species are presented.

8.Relation between Ischemia on Exercise Testing and on Holter Monitoring.

BACKGROUND: The relation between treadmill exercise testing and ambulatory ST segment monitoring in detection of ischemia in patients with coronary artery disease has not been well established, with pathophysiologic mechanisms underlying the development of ischemia in the ambulatory setting not being well elucidated. This study was performed to determine the relation between myocardial ischemic indexes on exercise testing and on Holter monitoring. METHODS: We analized 25 patients with stable angina and angiographically documented coronary artery disease, who exhibited ischemia both on a Bruce protocol exercise test and on 24-hr Holter monitoring while receiving routine antianginal medications. RESULTS: Mean heart rate at the onset of 1mm ST segment depression during exercise testing(74.4 VS 114.1 beats/min, p<0.01). Overall the correlations between ischemic indexes on both test were weak. But heart rate at the onset of ischemic episodes during Holter monitoring had a good correlation with heart rate at 1mm ST depression during exercise test. And the number of ischemic episodes on Holter monitoring revealed a weak negative correlation with duration of exercise during treadmill test. CONCLUSION: Ischemic indexes on exercise testing cannot accurately predict ischemic indexes on Holter monitoring in patients with coronary artery disease. In that regard, Holter monitoring may provide other clinical information in addition to that obtained by exercise testing in selected patients.
