1.A study of anxiety scale applied to patients with anxiety disorder.

The authors studied anxiety, using Zung's self-rating anxiety scale (SAS), in the subjects of 127 male and 38 female patients with anxiety disorder. The authors investigated 165 patients in Yeungnam university hospital from January, 1987 to June, 1991. In order to analyze the data on anxiety scores Pearson's product moment coefficient correlation method and factor analysis were carried out by SPSS/PC+ program. The results were as follows: There was significant difference in the mean averages of total anxiety scores among patients with anxiety disorder and male and female college freshmen: patients with anxiety disorder scored 42.40±7.74, male students scored 32.91±5.70, female students scored 34.48±6.00. The anxiety scores relating to the items of body aches & pains, fatigue, anxiousness, panic and urinary frequency were relatively high in patients with anxiety disorder. The anxiety scores on the items of restlessness, apprehension, dyspnea, sweating, and insomnia were relatively low in patients with anxiety disorder. Twenty-nine anxiety disorder group (17.5%) showed significantly high anxiety scores of 50 or over. The inter-rater reliability of Zung's self-rating anxiety scale was 0.71.

2.A Clinical Analysis of Bone and Joint Tuberculosis Complicated with Mixed Infection

200 cases of bone and joint tuberculosis complicated with mixed infection treated at the Department of Orthopedic Surgery, National Medical Center through the year of 1958 to 1971 were analyzed and reported in this paper. 1. All the mixed infections complicated in bone and joint tuberculosis occurred secondarily as a complication of the formation of fistula in this series. 2. The incidence of the fistula formation as a complication of bone and joint tuberculosis was considerably higher as 117 out of 200 cases in the age group of 10–30 years compared with other age groups. Accordingly the incidence of mixed infection was higher in this age group than other groups in same proportion in this seiies. 3. Mixed infection occurred relatively earlier after the fistuIar was formed. 46 out of 200cases (23%) had mixed infection one month after the fistula was formed. 4. Majority of the fistula developed spontaneously as 129 out of 200 cases (64.5%) 5. 135 cases of fistula developed during the treatment with PAS. and INH, and 11 cases developed fistula during triple antituberculosis therapy with PAS. INH. and Streptomycin. 6. Majority of the causative microorganism was Staphylococcus aureus as 210 out 360 cultures. (58.3%). 7. The result of the treatment was evaluated, and the result of radical operation gave remarkably higher incidence of healing, 110 out of 130 cases, than that of medical treatment, 33 healing out of 64 cases.

3.A Case of Antithyroid Drug-Induced Agranulocytosis Treated with Granulocyte Colony-Stimulating Factor (G-CSF) and Methylprednisolone.

Although rare, agranulocytosis is the most serious, potentially fatal side effect of antithyroid drug. We experienced a 13-year-old girl who developed methimazole-induced agranulocytosis at 1 month after the initiation of treatment. Her granulocyte count recovered after discontinuation of methimazole and treatment with broad spectrum-antibiotics, G-CSF, and methylprednisolone. After recovery from agranulocytosis she was treated with radioiodine ablation therapy. Early detection and proper management of antithyroid drug-induced agranulocytosis is very important.

5.Reference Values of Cadmium in Kidney and Liver in Korean.

OBJECTIVES: Cadmium (Cd), a toxic and non-essential metal, is recognized as a human carcinogen, which has a tendency to accumulate in the human body. The levels of Cd in renal cortex and liver are good indicators as an index of Cd exposure in the general population. In this study, we present an estimation of reference Cd levels in tissue (renal cortex and liver) and total body burden in the general population of Korea. MEDTHODS: Cd and zinc (Zn) were analyzed in renal cortex and liver from 254 autopsies (male : 188 cases, female : 66 cases) aged 0 to 87 years. RESULTS: Geometric mean concentration of Cd was 27.4 and 3.1 ua/g wet weight in renal cortex and liver, respectively. The level of Zn in renal cortex and liver was 35.4 and 42. 6 v/g wet weight, respectively. The result suggests that kidney is the target organ for Cd accumulation. The accumulation of Cd in renal cortex was age-dependent with a biphasic pattern. The level of Cd in renal cortex increased with age up to the fifties, and then leveled off thereafter. Based on the data, the regression model for Cd accumulation in renal cortex by age is predicted by : Log KCd = 0. 2325 + 0. 0553 Age 0. 0005 Age. The highest Cd accumulation in renal cortex of Koreans was estimated at 43. 3 ua/g wet weight at 50. 8 years old. In addition, the total Cd body burden by age was estimated by the following equation: Total Cd Body Burden = -4. 5948 + l. 2278 Age - 0. 0121 Age. The highest body burden of Cd was estimated at 26. 5 mg at age 50. 7 years in the Korean general population. The positive correlation between Zn and Cd was observed in renal cortex and liver. CONCLUSIONS: The level of Cd exposure in Korean was found to be lower than in Japanese, but same as or higher than in American and Europeans.

6.Evaluation of the Mental Health Status of the Aged by Zung's Self-rating Anxiety Scale.

The authors studied on anxiety, using Zung's self-rating anxiety scale (SAS), on the subjects of 329 men and 522 women of 60 years old and older. This study was carried out for 8 months from November 1983 to June 1984 in the area of Seoul, Taegu and Kyungsan Gun, Kungpook province. The results showed that there was no significant difference in the means of total anxiety scores between the two groups of men and women. The anxiety scores in items of sweating, apprehension, dyspnea, restlessness and insomnia were relatively high in both groups. The scores of faintness, panic, fear, tremor and facial flushing were low in both groups. Among 329 men, 48 (14.6%) showed rather serious anxiety level of score 50 or higher, while 153 of 522 women (29.4%) showed the same score. Among these psychosocial factors, supporter, living, place, religion and security system are significantly related with anxiety scores in both groups (p<0.01).

7.A Study on MMPI in College Freshmen.

The authors studied the mental health status of the 1989 freshmen of Yeungnam University using the psychotic scale of Derogatis SCL0-90, Zung anxiety and depression scale and MMPI from January 1988 to February 1988. The subjects of this study were contained 3792 students (1206 female, 2586 male). 392 students (10.3%), out of all respondents were screened to fall on high score ranges. The authors performed a second test to 156(39.8% 47 female (30.1%), 109 male (69.9%) of the high-scored in order to clarify their mental health status and analyze various psychosocial factors; using MMPI. The MMPI results were categorized with Lachar's classification. The results were as follows: 1. By the MMPI scores, 24 respondents (15.4%) had neurotic trends, 20 (12.8%) had psychotic trends and 7 (4.5%) had trends of behavior disorder. It was about 1.4% of all 3792 freshmen. 2. On the close examination of the MMPI, 8 respondents were diagnosed for affect disorder, 6 for anxiety disorder, 9 for schizophrenia and 3 for personality disorder. It was 0.7% of all freshmen. 3. One the analysis of the psychosocial factors, the schizophrenic scores were high in the case of unsatisfy in family atmosphere, unsatisfy in marital intimacy of parents, serious in conflicts between father and I, and serious in conflicts between mother and I. 4. The hypochondriacal scores were high in the case of weak in physical condition, having hospitalization experience and physical illness.

Neuroleptic malignant syndrome is a rare, but potentially life-threatening adverse event associated with the use of neuroleptic agents. We describe the case of a 47-year-old schizophrenic woman who was treated with clozapine for years. The patient developed acute renal failure with pulmonary edema, and underwent mechanical ventilation and hemodialysis.

10.Chronic Recurrent Folliculitis with Atypical Hyperpigmented Scarring in an AIDS Patient.

Bacterial, fungal, and viral infections of the skin with extended skin involvement can occur during the early phase of human immunodeficiency virus infection. A significant reduction in circulating CD4+ lymphocytes in the late stage of the disease may cause tumors of the skin such as Kaposi's sarcoma. A 40-year male patient, a former sailor who had multiple sexual contact with native African women, presented with multiple tender follicular pustules and fibrotic brown patches on both his legs. these had been present for 6 months. The skin lesions were healed leaving brown pigmentation. Laboratory examinations revealed the presence of leukopenia, thromocytopenia and a reversed T4/T8 ratio. The ELISA and Western blot analysis to human immunodeficiency virus were positive. A skin biopsy from a brown patch showed early stages of scar tissue and perivascular hemosiderin deposition. We herein report a case of acquired immunodeficiency syndrome patient with atypical dark brown scarring atrophic patches on the lower legs following purulent bacterial folliculitis. This may have been an early manifestation of Kaposi's sarcoma from a preceding skin lesion.
