1.Report of A Case of Situs Inversus Totalis.

A case of situs inversus totalis was observed in the cadaver of a 35 year old Korean female and the anatomical structures were investigated. The fact that she had received intestinal resection due to tuberculosis of intestines was confirmed. The results are summarized as follows : 1. All thoracic and abdominal viscera were completely transposed and in the left lower quadrant of abdomen, it was observed that the distal portion of the small intestine, cecum, appendix and part of the ascending colon had been resected. 2. Classic mirror-image dextrocardia with patent foramen ovale (large diameter, 7mm), sinusitis in the left maxillary sinus, bilateral thoracic duct, variation of the opennig site of the common bile duct and the main pancreatic duct, and maldvelopment of the left adrenal gland were observed together with the sinus inversus totalis.

3.Lipid Profiles after Discontinuation of Growth Hormone Treatment in Adults with Childhood-onset Hypopituitarism.

PURPOSE: The need for continuing Growth Hormone(GH) replacement after adolescence in patients with childhood-onset GH deficiency has been recognized. The purpose of this study was to evaluate the abnormalities of lipid profiles in young adults with childhood-onset hypopituitarism who discontinued GH therapy after the completion of height growth. METHODS: Nine male patients(mean age:22.4+/-3.3 years) with childhood-onset hypopituiatarism in whom GH treatment had been discontinued after final height was achieved were included. Their body mass index(BMI) and serum levels of total cholesterol, triglyceride(TG), high-density lipoprotein(HDL) cholesterol, and low-density lipoprotein(LDL) cholesterol were measured. The relationships of duration after GH discontinuation, age, and BMI to lipid profiles were anaylzed. RESULTS: BMI increased significantly from 21.8+/-1.9 kg/m2 before GH discontinuation to 23.0+/-3.0 kg/m2 after GH discontinuation(P<0.05). Serum levels of total cholesterol, TG, HDL cholesterol, and LDL cholesterol were 217.3+/-33.4, 237.8+/-128.2, 42.1+/-7.1, and 127.4+/-27.2 mg/dL, respectively, 3.1+/-2.0 years after GH discontinuation. Percentage of patients who had total cholesterol > or = 200 mg/dL, TG > or = 150 mg/ dL, LDL cholesterol > or = 140 mg/dL, and HDL cholesterol < or = 40 mg/dL were 77.8%, 88.9%, 44.4%, and 33.3%, respectively. All subjects had some abnormalities of lipid profiles. A significant positive correlation was found between duration after GH discontinuation and serum levels of total cholesterol and TG(r=0.84, P<0.01; r=0.83, P<0.01). A significant positive correlation was also found between age and serum levels of total cholesterol and TG(r=0.86, P<0.01; r=0.81, P<0.01). There were no correlations between BMI and serum lipid levels. CONCLUSION: Most of young adult patients with childhood-onset hypopituitarism had abnormal lipid profiles by 1-5 years after discontinuation of GH treatment. These data suggest that continuous GH treatment after completion of height growth is necessary.

4.Action Duration of Atracurium in the Elderly Patients.

BACKGROUND: The effects of a muscle relaxant may differ in elderly compared with young adult patients for a variety of the pharmacokinetic and pharmacodynamic reasons. Atracurium is eliminated by nonorgan dependent pathway, Hofmann elimination and ester hydrolysis. So there are some arguments for age-related differences. The purpose of this study was to compare the differences of the onset and duration of atracurium in young and elderly. METHODS: Eighteen young adults (21-54 yr) and 18 elderly (>65 yr) patients anesthetized with nitrous oxide and enflurane. Atracurium (0.5 mg/kg) was given and then intubation was performed after T1 response was blocked more than 80%. Neuromuscular relaxation was measured by the first twitch of train-of-four (T1) response at the adductor pollicis after supramaximal stimulation of ulnar nerve at 2Hz every 12 sec. The onset (disappearance of T1) and duration of 5, 25, 50, 75% recovery time of T1 and recovery index (time for 25-75% recovery of T1) were recorded. RESULTS: Onset of block was not significantly different between the young and elderly. Recovery time of 5, 25, 50, 75% and recovery index were not prolonged in elderly compared with young adults. CONCLUSIONS: There were no significant differences between young and elderly adults in onest time, recovery time of 5, 25, 50, 75% and recovery index when atracurium is used in a single bolus dose. The results suggest that atracurium in elderly patients has similar onset and action duration compared with younger patients.

5.Association of Cytotoxic T Lymphocyte Antigen-4 Gene Polymorphisms and HLA Class II Alleles with the Development of Type 1 Diabetes in Korean Children and Adolescents.

We studied the association of cytotoxic T lymphocyte antigen-4 gene (CTLA4) polymorphisms with the development of type 1 diabetes (T1D) in Korean children and adolescents. A total of 176 Korean subjects (92 females and 84 males) with childhood-onset T1D were studied. The A/G polymorphism at position 49 in CTLA4 exon 1 and the C/T polymorphism at position -318 in the CTLA4 promoter were analyzed by PCR-RFLP methods. The genotype and allele frequencies of the CTLA4 polymorphisms in the T1D patients were not different from those in the controls. These polymorphisms were not associated with the clinical characteristics or the development of autoimmune thyroid disease in the T1D patients. The frequency of the A allele was significantly higher in the patients that did not have two out of the three susceptible HLA-DRB1 alleles, which were DRB1*0301, *0405 and *09012, compared to the controls (P<0.05). These results suggest that CTLA4 polymorphisms do not directly confer any susceptibility to T1D. However, a CTLA4-mediated susceptibility effect on the development of T1D might be significant in children and adolescents that do not have susceptible HLA class II alleles.

6.Small Cell Osteosarcoma Similar to Ewing's Sarcoma in Histologic Findings and MIC2 Expression: A case report.

Small cell osteosarcoma is a rare form of osteosarcoma and the histological differential diagnosis from other small round cell tumors (SRCTs) is difficult. The immunohistochemical stain for MIC2 has been considered an useful diagnostic marker for Ewing's sarcoma and primitive neuroectodermal tumors but recently, other SRCTs such as malignant lymphoma and embryonal rhabdomyosarcoma also showed positive reaction. Therefore, the usefulness of MIC2 must still be proven. We experienced a case of small cell osteosarcoma of the mandible in a 25-year-old man. Histologically, the tumor consisted of small round cells that resembled those of Ewing's sarcoma. Immunohistochemically, the tumor cells expressed diffuse strong positive reaction for MIC2 gene products. However, the scanty foci of lacy osteoid material between the tumor cells seemed to be diagnostic of osteosarcoma. The histologic and immunohistochemical findings of this case suggest close relationship between small cell osteosarcoma and Ewing's sarcoma.

7.Recurrent Dislocation of the Shoulder: MR Imaging and MR Arthrographic Findings.

PURPOSE: To evaluate and compare the diagnostic findings and usefulness of MRI and MR arthrogram in recurrent anterior dislocation of the shoulder. MATERIALS AND METHODS: MRI and MR arthrogram of thirty-two cases of thirty patients with recurrent anterior dislocation of the shoulder were retrospectively analyzed to evaluate glenohumeral ligaments, pattern of labral injury, bony Bankart lesion, capsular stripping, Hill-Sachs lesion, and loose body. RESULTS: Labral abnormalities was detected on MRI in 23 cases(72%) and MR arthrogram in 22 cases(69%) as labral tear, detachment, and deficiency. All cases involved anteroinferior labrum. On MR arthrogram, labral abnormalities was confined to anteroinferior portion in 4 cases(18%), extended into midglenoid level in 13 cases(59%), and that into anterosuperior portion in 5 cases(23%). Capsular stripping was noted on MRI in 6 cases(19%) and MR arthrogram in 12 cases(38%). Loose body was detected only on MR arthrogram in 5 cases (16%). Bony Bankart lesion and Hill-Sachs lesion was detected equally on MRI and MR arthrogram in 2 cases (6%) and 21 cases(66%) respectively. We obtained identical findings on both MR arthrogram and operation field in 6 patients. CONCLUSION: MRI with its excellent soft tissue contrast resolution and multiplanar imaging capafility helps detection of labral injury in recurrent anterior dislocation of the shoulder. MR arthrogram is slightly invasive, but shows good artatomic details of labral-ligamentous-capsular relationship and thus provides accurate detec- tion of pattern of labral injury, capsular stripping, and loose body.

8.A Rare Case in the Pattern of the Origin of the Celiac Artery.

A rare case in the pattern of the origin of the celiac artery was observed in the cadaver of a 64 year old Korean male and the results are summarized as follows. The superior mesenteric artery originated from the abdominal aorta at the level of the intervertebral dusk between first and second lumbar vertebra. At a point 4cm from the origin of the superior mesenteric arterr, a common trunk appeared and ran 1cm and then divided into the common hepatic artery and the splenic artery. The common hepatic artery divided into the right gastric artery and the proper hepatic artery at a point 4.5cm after the bifurcation. A common trunk formed by the left inferior phrenic artery and the lefts gastric artery arose from the abdominal aorta about 0.8cm about the origin of the celiacomesenteric trunk.

9.Anatomical Study of Musculus Pyramidalis in Korean Adults.

The report of the M. pyramidalis was studied in 51 cadavers of Korean adults (male 33, female 18). Origin, insertion and number of this muscle were observed, and the length and breadth were also measured. The results were as follows : 1. The right pyramidal muscle was absent in one male cadaver, and the left pyramidal muscle was absent in two male cadavers. 2. In one male cadaver, pyramidal muscles of both sides had bifurcated origin. 3. In Korean adults, the average length of this muscle was 6.72cm in right, 7.05cm in left, and the breadth was about 2.0cm in both sides. The morphological index was 31.10 in right, and 29.55 in left. 4. In Korean male, the average length of this muscle was 1.93cm in right, 7.42cm in left, and the breadth was 1.93cm in right and 1.92cm in left. The morphological index was 28.70 in right and 26.58 in left. 5. In Korean female, the average length was 6.26cm in right, 6.38cm in left. and the breadth was 2.13cm in right and 2.12cm in left. The morphological index was 35.57 in right and 34.63 in left. 6. Compared with other human races, the pyramidal muscle was less frequently wanting in Asians (3.9%), including the Korean adults, than wanting in European (19.9%), American Whites (20.4%) and Negroes (15.1%). 7. The morphological index of this muscle was little differences existing in male of the human races, but this index was higher in Korean female (35.0) than in Japanese female (24.4) and in American female Whites (25.8). The morphological index was 32.3 in American female Negroes.
