1.Pneumocephalus after Inadvertent Dural Puncture during Epidural Block: A case report.

Pneumocephalus is a rare complication of anesthetic procedures involving the epidural space. With the loss of resistance technique using air, air may be injected in through the dural puncture site. Epidural blood patch was performed for the treatment of post-lumbar puncture headache, and 10 minutes after the treatment, 8 ml of 0.5% bupivacaine hydrochloride was injected through epidural catheter. Loss of consciousness and apnea were developed about 8 minutes after injection of local anesthetics. Computerized tomographic brain scan revealed the presence of air within intracranial cavity. Pneumocephalus may be symptomatic or asymptomatic. It generally responds well to conservative management. The patient did not complain of a headache. The patient was discharged from the hospital on the seventh day postpartum with no neurologic sequelae.

3.A case of virilizing adrenocortical adenoma.

This is a case report of virilizing adrenal cortical adenoma in 22 month old male patient who was diagnosed by clinical features, endocrinological studies, radiologic studies and pathologic findings. During surgery, a 40gm well-encapsulated tumor was removed from the right adrenal gland. Histologic examination revealed no capsulr invasion and a diagnosis of adrenocortical adenoma was made. He is in well condition without the clinical or laboratory evidence of recurrence at 2 years of follow up.

4.Effects of Converting Enzyme Inhibitor on the Left Ventricular Remodeling after Coronary Artery Reperfusion in Rats.

BACKGROUND: Angiotensin convertiong enzyme inhibitors have been shown to exert favorable effects on the left ventricular remodeling process associated with ventricular dilation after coronary occlusion. However, the effects of such therapy on global and regional left ventricular remodeling after coronart artery reperfusion have not been characterized, nor have such effects been assessed after exercise training. METHODS AND RESULTS: Female Sprague-Dawley rats(n=80) were randodmized into 4 groups at 5 days after 45 minutes of left coronary artery occlusion followed by reperfusion. Animals completion the experiment included : Untreated Sedentary group(n=20), Untreated with Swimming Exercise group(n=21), Captopril Treated Sedentary group(n=18) and Captoril Treated with Exercise group(n=21). At 3 weeks after randomization, global and regional morphologic changes of the left ventricle(LV) were examined from mid-ventricular transverse slices which were perfusion-fixed at a constant aortic pressure of 60mmHg and a left ventricular cavity pressure of 10mmHG. At rest and during exercise, compared to untreated rats, the captopril treated animals showed significantly decreased LV weight/tibial length ratio(LV/TL)(p<0.01),increased LV cavity area and dimension(both p<0.01), decreased total myocardial area and noninfarcted area(both p<30.001) and reduced wall thicknesses in the noninfarcted and infarcted regions(both p<0.001). Compared to treated and untreated dsedentary rats, exercise significantly increased LV/TL(p<0.05) and epicardial and endocardial areas in the infarcted zone(both p<0.05) and decreased transmurality(p<0.01). Exercise decreased LV cavity area in the captopril treated groups(42.3+/-10.4 vs. 40.4+/-6.0mm2),whereas exercise increased LV cavity area in the untreated groups(33.5+/-8.9 vs. 39.1+/-6.2mm2)(p<0.05). CONCLUSION: These findings provide evidence in rats for evidence in rats for exaggerated left ventricular dilation and supperssion of compensatory myocardial hypertrophy globally and in the infarct zone with 3 weeks of captopril treatment following coronary artery reperfusion with acute nontransmural myocardial infarction. In addition, the effects of captopril on LV dilation and suppression of global and regional hypertrophic response were partially reversible by swimming exercise.

6.FITNESS TEST USING THE PHASE-SHIFTING PROFILOMETRY ACCORDING TO THE DENTURE CURING METHODS.

According to repeated measurements and correction procedures, the accuracy of the phase-shifting profilometry was developed. At first, after 20 final models for maxillary complete denture were duplicated, the mucosa surfaces of models were measured with the phase-shifting profilometry and each mirror view of these was calibrated. Maxillary cats were divided into 4 groups of 5 casts per each, and wax dentures with 2 sheets of baseplate wax and artificial teeth were made and then cured according to the curing method of each group. Group I ; quick curing with QC-20 acrylic resin Group II ; 9 hour curing with QC-20 acrylic resin Group III ; SR-Ivocap system Group IV ; metal base and quick curing with QC-20 acrylic resin After curing, polishing, and storing at 37degrees C n saline for 30 days, the forms of the impression surface of the dentures were measured with the phase-shifting profilometry. Then, the impression surface form of each denture was placed in the optimal position of comparison with the mirror view of the same final cast. The amount and direction of distortion of each denture was analyzed and the effects of polishing and storage in each denture were compared. The obtained results were as follows : 1. In Group I, the denture was observed as the appearance distorted in the opposite direction of the mucosa and the postero-lateral part of palatal portion of the denture was observed as the appearance separated from the mucosa. Also, the buccal flanges of the denture were observed as the appearance distorted in the direction of the mucosa. 2. In Group II, the postero-lateral part of palatal portion of the denture was observed as the appearance separated slightly from the mucosa. The bilateral buccal flanges of denture were observed as the appearance distorted severely in the direction of the mucosa. 3. In Group III, the bilateral part of the residual ridge crest portions and the buccal flanges of the denture were observed as the appearance distorted in the direction of the mucosa, and specially, the buccal flanges of the maxillary tuberosities were distorted severely. 4. In Group IV, the acrylic resin base of the buccal portion of the denture was observed as the appearance distorted in the opposite direction of the mucosa. 5. The phase-shifting profilometry, done with repeated measurements and correction procedures, was effective in comparing the amount and direction of distortion at every position after the laboratory work and the delivery of maxillary complete denture.

7.A Study of Types and Distribution of Orthodontic Patients in the Department of Dentistry.

174 patients who visited in the Department of Dentistry, College of Medicine, Yeungnam University from 1983. 7. 15 to 1986. 11. 30 were surveyed on the tendency of patient distribution and the state of Angle's classification. The results were as follows 1. There was increased visiting rate of patient per year. Female outnumbered male by a ratio of 1.3:1. 2. 8-15 age group was 74% in total visiting in the most frequency. The average visiting numbers of each month were higher among the vacation and was about 74% in that period. 3. As showed the living distribution, Nam Go and Su Sung Gu's patients were 53% of the total. 4. As the motive of visiting in Dental Department, patient of 30% in total patients visited for themself to receive orthodontic treatment and other was introduced patients. 5. By classification, Class I malocclusion was 63% in total visiting patient, Class II malocclusion 17%, Class III malocclusion 20%. Crowding among Class I malocclusion was 60%.

9.Soft Tissue Change After Single Jaw(mandible) Surgery in Skeletal Class III Malocclusion.

The purpose of this study was to evaluate the amount and interrelationship of the soft and hard tissue change after mandibular setback surgery in skeletal Class III malocclusion. The sample consisted of 25 adult patient (12 male and 13 female) who had severe anteropostrior skeletal discepancy. These patient had received presurgical orthodontic treatment and surgical treatment which is bilateral sagittal split ramus osteotomy. The presurgical and postsurgical lateral cephalograms were evaluated. The computerized statistical analysis was carried out with SPSS/PC program. The result were as follows: 1. After mandibular bilateral sagittal split ramus osteotomy, lower facial soft tissue horizontal posterior changes were high significance value. but vertical soft tissue changes were low significance value. 2. After mandibular bilateral sagittal split ramus osteotomy, relative upper lip protrusion increased(p<0.01) and relative lower lip protrusion decreased(p<0.01) and lower facial soft tissue thickness increased(p<0.01).
