1.Differentiation of amebic versus pyogenic liver abscess with US and CT.

To differentiate amebic and pyogenic liver abscesses, sonography(US) and CT findings of 21 amebic and 22 pyogenic liver abscesses were reviewed retrospectively. US was performed in all cases and CT in 15 cases of each group. Eleven cases of amebic abscesses had well defined margin which was observed in 10 cases of pyogenic abscessed. Amebic abscesses showed hyperechoic wall in 7 cases and calcification in 4 cases. Ten cases of pyogenic abscesses had mearby intrahepatic duct dilatation which was observed in 2 cases of amebic abscesses. Pneumobilia was seen in 6 cases of pyogenic abscesses. US findings of clear margin and hyperechoic wall, and CT findings of intrahepatic duct dilatation, pneumobilia, and calcification were helpful in differentation of amebic pyogenic liver abscesses.

2.Ultrasound Guided Biopsy of Malignant Focal Liver Lesions: Comparison of Automated Gun Biopsy with Fine Needle Aspiration Biopsy.

PURPOSE: To compare the efficacy and safety of ultrasound guided automated gun biopsy with fine needle aspiration biopsy of focal liver lesions. MATERIALS AND METHODS: We performed 32 automated gun(19.5G Autovac Biopsy Needle) biopsies and 27 fine needle aspiration(22G Westcott Style Biopsy Needle) biopsies in 48 patients with malignant focal liver lesion under the guidance of ultrasound. We compared the "positive for malignancy rate" and "pathologic diagnosis rate" of both methods with final diagnosis, and the rate of complications. RESULTS: The "positive for maiignancy rate" of automated gun biopsy and needle aspiration are 84.4%(27/32) and 66.7%(18/27) respectively(p=0.11). The "pathologic diagnosis rate" are 84.4%(27/32) and 55.6%(15/27) with significant statistical difference(p<0.05). And there is no difference of the rates of significant complications. CONCLUSION: When compared with the fine needle aspiration biopsy, ultrasound-guided automated gun biopsy is safe and more effective method for malignant focal liver lesion.

7.Clinical Study of Alopecia Areata in Children during the Five Years (2007~2012).

BACKGROUND: Alopecia areata (AA) is a non-scarring, autoimmune, inflammatory hair loss on the scalp and/or body. This disease affects both adults and children but there are limited data on AA in children than AA in adults. OBJECTIVE: The purpose of this retrospective study is to evaluate the clinical characteristics, disease associations and psychosomatic dynamics of pediatric AA for the past 5 years. METHODS: A retrospective study was performed on 120 out-patients under 14 years old who are diagnosed as alopecia areata in the department of dermatology from March, 2007 to February, 2012. RESULTS: The proportion of pediatric group in total alopecia areata was 20.2% (120/595). According to clinical records, the alopecia areata were of 111 patients (79.9%) followed by those with alopecia universalis (4 patients; 2.8%) and alopecia totalis (5 patients; 3.6%). The ratio of males (57 patients; 47.5%) to females (63 patients; 52.5%) was 1 : 1.11. Alopecia areata was seen most frequently in the school age group (69 patients; 57.5%) and the duration period was less than one year in most cases (79.2%). Family history of alopecia areata was observed in 10.8% (13/120). The only child or eldest child being overloaded with homework and has deficient parent-child relationships including familial discord was accounted as the most susceptible group. A multiple bald patch was 51.4% in alopecia areata and the most common associated disease was atopic dermatitis (26 patients; 21.7%). CONCLUSION: We observed several clinical features of the pediatric AA, including epidemiology, clinical characteristics and disease associations. This study provided useful data for future research regarding AA in children.

8.Factors affecting the price-reduction rates among the insurance medicines.

To provide the information necessary for the insurance medicine management plan, price discount rates among the insurance medicines were studied. A total of 2,107 items of insurance medicine of which prices were discounted via government inspections of real transactional process of insurance medicine were analysed. The conclusions are as follows; 1. Among the variables relevant to the characteristics of manufacturers, price discount rates of insurance medicines were statistically significant with production rankings of manufacturers, incorporation year, existence of investments by foreign corporation, existence of a research institute, and enrollment in the exchange. And among the variables relevant to the properties of medicines, the number of enrolled items which have the same components, classification, the date of new enrollment, the sales of items, and the number of raw materials in the items were statistically significant. 2. Stepwide multiple regression was done to identify the factors which affect the price discount rates of insurance medicines. The number of enrolled items which have the same components, production rankings of manufactures, classification number (medicines for function of tissue cells), incorporation year (1940-1949), existence of investments by foreign corporations, classification number (anti-germ medicines), number of raw materials in the items, the sales of items, and medicines whose major objective is not treatment were significant variables and the R2-value for these variables was 21.2%. Considering all of the above results, for management of insurance medicines, it seems important that the real transactional prices of insurance medicines should be identified systematically, focusing on the properties which affect the price discount rates of insurance medicines.

10.Transcatheter Double-Disc Occluder Closure of Patent Ductus Arteriosus.

We successfully closed the isolated patent ductusarteriosus(PDA) in 2 female patients using the Rashkind Double-Disc Occlusion system. nonsurgical closure of PDA usin plug was introduced by Porstmann in 1968 through transfemoral artery catheter and using umbrella by Rashkind in 1977 througth venous site. There has been continued improvement and simplification of the equipment as well as in the implantation technique. Nowaday, a multicenter study was conducted to test the safety and effectiveness of this interventioal method in States. Successful closure rate was high but some problems were reported such as embolization, abnormal deployment and residual shunt. Residual shunt was visualized by aortography in our both cases immediately after intervention but continuous murmur could not be heard in both cases. Oxygen step-up disappeared immediately after occluder implantation in both cases. Complete occlusion was expected with thrombus in near future.
