1.A Case of Clear Cell Sarcoma of the Anterior Chest.

Clear cell sarcoma of tendon and aponeurosis is a rare malignant tumor. It occurs chiefly in young adults, predominates in women and is most common in the regions of the foot and ankle. We report a case of clear cell sarcoma of tendon and aponeurosis in s 22-year-old man. he pstient had had a asymptomatic, normal skin colored, relativerly hard, dome shsped nodule on the anterior chest for 6 months. Histopsthologic findings revealed uniform pattern composed of compact nests of round or fusiform cells which had clear cytoplasm and were surrounded by delicate framework of fibrocollagenous tissue, and the individual tumor cell had a fairly regular appearance of possessing round to avoid vesicular nucleus with prominent basophilic nucleolus. One year after surgical excision and post operative radiotherapy, there was no recurrence.

3.Crush-Cleavage Fracture in Thoracolumbar and Lumbar Spine: Comparative Study with Type B Burst Fracture

We noticed a group of thoracolumbar and lumbar spine fractures showing a unique fracture pattern that consisted of 1) superior disc injury, 2) crush fracture of the upper half of the vertebral body, 3) sagittal fracture of the lower half of the vertebral body, 4) bone fragments in the spinal canal, and 5) lamina fracture. Some of these fracture patterns were present in type B burst fracture of Denis classification. The purpose of this study was to compare the difference between crush-cleavage fracture and type B burst fracture in the viewpoint of neural canal involvement and neurologic status, and functional outcome with surgical treatment. Ten cases were identified as crush-cleavage fractures in 22 type B burst fractures during a 5 year period from Mar. 1989 to Jun. 1993 at the Department of Orthopedic Surgery of Chonbuk National University Hospital. Four out of 10 crush-cleavage fractures and 4 out of 12 type B burst fractures were paraparetic. Crush-cleavage fracture must be an unstable thoracolumbar comminuted fracture, but there were no significant differences in the neural canal involvement, incidence of neurologic deficit, and in the functional outcome, compared with type B burst fracture (P>0.05). It could be classified as a progressed form of type B burst fracture in Denis classification, or as a burst-split fracture in Magerl classification in the anatomical viewpoint. In conclusion, crush-cleavage fracture must be a variant of burst fracture.

4.Efficacy of Percutaneous Transhepatic Choledochoplasty of the Biliary Stricture, Using a 16F Silastic Catheter, in Patients with Recurrent Pyogenic Cholangitis.

PURPOSE: To evaluate the efficacy of percutaneous transhepatic choledochoplasty of the main biliary stricture, using a 16F silastic foley catheter, in patients suffering from recurrent pyogenic cholangitis MATERIALS AND METHODS: In nine patients who had undergone stenting procedure at the site of the main stricture after the total removal of biliary stones, a 16F silastic catheter was held in position for 1-4 (mean, 2.7) months. Using ultrasonogrophy, we assessed the diameter of the bile duct 5 cm above the main stricture. RESULTS: During the follow-up period of 1-30 (mean, 18.1) months, the diameter of the bile duct above the main stricture was, in all cases, under 3 mm. CONCLUSION: In patients with recurrent pyogenic cholangitis, choledochoplasty of the biliary stricture, using a 16F silastic catheter, was therapeutically effective.

Neuroblastoma is a malignant neoplasm which arises from primitive sympathetic neuroblasts. It is the second most common solid tumor in young children, exceeded in incidence only by brain tumors. This tumor shows immunological features and spontaneous regression. But, in spite of recent advances in multimodality therapy survival rate has not been improved much. 6.Intraocular Cysticercosis.

We investigated seven cases of intraocular cysticercosis: 4 subretinal in the macular region, 1 subretinal just above the optic disc and 2 in the vitreous cavity. Preoperative diagnosis was confirmed in all cases by observing the characteristic movement of the cysticercus larva. Trans-scleral extraction of the cysticercus was performed and resulted in improvement of vision in two cases and preservation of the preoperative vision in the other cases. We reviewed the medical histories, preoperative evaluation of the status of the vitreous and the retina, and the procedures of the operation.

7.Treatment of taeniasis with bithionol.

Ten cases infected with Taenia saginata were treated with Bithionol, 2, 2' thiobis (4,6-dichlorophenol) which is well known as an excellent anthelminthhic against paragonimiasis and their follow-up studies were carried on within 3 to 12 months after treatment. Bithionol, 50 mg dose per kg of body weight was divided into two equal dose and administered orally in 30 minutes intervals. Three hours later, this was followed with a sodium sulfate or magnesium sulfate purge. The strobilas without scolices were found in the stools after the medication and the follow-up studies have shown the complete deworming in all cases. It is well tolerated, toxic effects being uncommon and when present, limited to mild gastro-intestinal disturbances.

8.A Case of Coronary Artery Aneurysm.

A coronary artery aneurysm was first reported by Bougon in 1812, and was able to be diagnosed during life with advance of selective coronary arteriography. We have recently seen a 62-year-old woman who had intermittent anterior chest pain and dyspnea on exertion(NYHA functional class II) and diagnosed a coronary artery aneurysm by coronary arteriography. A saccular formed coronary artery aneurysm was located at left main coronary artery at the juction of the bifurcation of LAD and left circumflex artery and it's was 2cmx2cm and it's thickness of wall was 0.5cm. After the patency and consistency of coronary arteries were confirmed,resection of aneurysm with aneurysmorrhaphy was performed.

9.Chemoembolization through Intercostal Arteries in Hepatocellular Carcinoma' Report of A Case of Transient Spinal Cord Injury.

Liver has a dual blood supply from portal vein and hepatic artery. Hepatocellular carcinoma receive their blood supply almost exclusively from hepatic artery. Thus, the concept of treating hepatocellular carcinoma by chemoembolization through these arteries is very effective. However, there may be several collateral or parasitic vessels feeding them in case of huge tumor or previous chemoembolization. We experierced a case of huge tumor involving right upper posterior portion of liver fed by 9th, 10th, 11th right posterior intercostal arteries and an anomalous hepatic artery. We tried chemoembolization with Adriamycin-Lipiodol suspension and Gelfoam material through the right posterior intercostal arteries to treat the lesion. After the procedure, the patient(55 years old female) became paraplegic with voiding and defecation difficulty which could be due to spinal cord infarction .by anterior spinal arteri. al occlusion caused by embolic material through the artery of Adamkiewicz from a posterior intercostal artery. She recovered completely after 20 days of treatment.

10.A Case of Pure Red Cell Aplasia Due to Parvovirus B19 in Renal Transplantation.

Parvovirus B19 is a minute, single stranded DNA virus which has been identified as the etiological agent of pure red cell aplasia, erythremia infectiosum (5th disease), hydrops fetalis, or arthralgia. We report a case of pure red cell aplasia caused by parvoviurs B19 in renal transplantation. The patient was 39-year-old male who had been diagnosed as chronic renal failure 22 months ago and taken hemodialysis twice a week. He had been taken renal transplantation in October 30th, 1997. But anemia was not improved in first postoperative period. Bone marrow revealed hypocellularity (about 30%) with maturation arrest of erythroid series at pronormoblasts. The pronormoblasts showed very large size, deep blue cytoplasm, cytoplasmic projection, cytoplasmic vacuole and distinct intranuclear eosinophilic inclusions. The parvovirus B19 PCR and anti-parvovirus B19 IgM were positive, but anti-parvovirus B19 IgG was negative. The patient was treated with intravenous immunoglobulin and then reticulocyte count was increased three weeks later. The follow-up bone marrow revealed normal erythroid precursors and was not found infected giant pronormoblasts seven weeks later.
