We report one case of renal vein thrombosis that was mimicking symptoms for urinary caculi.

PURPOSE: Frey's syndrome is a common complication after parotid surgery and characterized by gustatory sweating and flushing in the periauricular area during meals. Although a variety of methods were proposed to prevent this postoperative problem, they were unsatisfactory. We used botulinum toxin A to treat 9 patients with Frey's syndrome and investigated the duration effect after the injection. METHODS: Nine patients became the object of study about the effect of botulinum toxins as treatment of Frey's syndrome. Age of patients ranged from 25 to 78 years (mean, 43.7 years). Six of nine patients had both symptoms of gustatory sweating and flushing. And the others had only gustatory sweating symptom. Using Minor starch iodine test, the affected skin area was detected, and it was marked by 1 cm2 sized grid appearance. After application of EMLA cream on the gustatory sweating area, botulinum toxin A was injected intracutaneously into the affected skin area (2.5 U/cm2). Patients were followed up from six to fifteen months (mean, about 12 months) and asked about improvement of their symptoms. RESULTS: The treatment with botulinum toxin A had an effect within 2 days after the injection. Six months after injection, gustatory sweating disappeared completely in all patients, and 5 of six patients who had gustatory flushing improved in their symptom. At the last follow-up, no patients complained of recurrent gustatory sweating and flushing except one. One patient, seven months after initial injection, was retreated with botulinum toxin A because of recurrence, and the result was successful. The duration of the effect after botulinum toxin A treatment was ranged from seven to thirteen months. One patient in our series experienced the upper eyelid weakness as adverse effect, but it improved spontaneously. CONCLUSION: Local injection of botulinum toxin A is an effective, safe and long-lasting method for treatment of Frey's syndrome. Hereafter, however, additional study is required to evaluate the duration effect of botulinum toxin A as to frequency in use and dosage

The present study compared the actigraphic indices between both wrist actigraphies (WATGs), and the sleep estimates between each WATG and nocturnal polysomnography (NPSG) to assess their differences and consistencies. We studied 22 right-handed subjects (mean age 43.9+/-13.3 years, M:F=14:8) with untreated primary sleep disorders (primary insomnia=8, simple snorer=2, obstructive sleep apnea=12) undergone by overnight both WATGs and NPSG, simultaneously. Comparison and correlation were analyzed between right and left wrist actigraphic data. In the sleep estimates of both WATGs and NPSG, each WATG was compared and correlated with NPSG in sleep period time (SPT), total sleep time (TST), sleep latency (SL), sleep efficiency (SE) and wake time (WT). Sleep indices between both WATGs showed significant positive correlations with no correlations in SL and fragmentation index (FI). There were no differences in sleep indices between both WATGs. SPTs of both WATGs, SL of left WATG, and TST of right WATG showed positively significant correlations, and SE of right WATG did negatively significant correlation in sleep indices between each WATG and NPSG. As each WATG was compared to PSG, SPTs of both WATGs and WT of right WATG were decreased, and TST and SE of right WATG and SL of left WATG were increased. Inconsistent SL and FI between both WATGs indicate that the activities between both WATGs can differentially happen during wake or arousal. Inconsistent sleep estimates between each WATG and NPSG may indicate the limited usefulness in measuring and analyzing one-night sleep by using WATG.

Whistle deformity is deficiency of vermilion of the upper lip. It is a common sequelae of primary lip repair. Many methods have been devised to correct this deformity, most using neighbouring normal tissue of the upper lip. Methods such as Z-plasty, Double rotation, V-Y advancement flap, etc., have been commonly used for the correction of unilateral whistle deformity. We found that the V-Y advancement flap with transposition of deepithelialized tissue was more effective for the correction of whistle deformity than the original V-Y advancement flap. We named this new method 'Modified V-Y advancement flap'. The technique has been performed in 5 patients, 9 to 23 years of age, each with a whistle deformity. One years and six months was the longest follow-up period. Satisfactory results have been obtained and here the authors reported along with a review of the current literature.

To find the severity and the type of the cervicobrachial syndrome, through physical examinations and laboratory tests were made toward 137 patients complaining the cervicobrachial syndrome (CBS) through the health interview among 313 workers occupied in a microwave-oven assembling factory from December 1993 to February 1994. Results were as follows. 1. The prevalence of spontaneous pain was 94.9%, while 78.1% suffered from muscle tenderness, 63.5% suffered from muscle induration and 62.0% showed kinesalgia. 54.7% of patients showed positive cervical X-ray findings and 28.5% showed positive Morley test. 2. The muscle induration was associated with the total duration of the job among female workers. 3. The severity of neck tenderness was negatively correlated with the strength of the grip at both hands, positively correlated with the duration of overtime in male and the severity of the wrist tenderness was negatively correlated with the height in female. 4. X-ray findings, muscle induration, Morley test results, muscle tenderness on the neck and muscle tenderness on the shoulder were associated with subjective symptoms of the workers.5. The muscle tenderness was associated with the number of the complaints at upper extremities among the sufferings in daily life significantly and the electromyogram (EMG) finding was associated with; the number of the complaints: about neuropsychological system among the sufferings in daily life significantly. In conclusion, the results of the physical examination were related with the subjective symptoms of the cervicobrachial syndrome. The muscle induration, the muscle tenderness, cervical X-ray findings and the Morley test results were more useful for the diagnosis of CBS. More efforts to objectify the subjective symptoms were asked.

Shunt malfunction is the most common cause of ventriculoperitoneal shunt failure. In literature, occlusion of the tube with brain parenchyma, choroid plexus, blood, and proteinaceous debris has been suggested as a mechanism of obstruction. We herein report a case of shunt malfunction without any identifiable occlusion. Our case findings suggest that unapparent abdominal pathology, including inflammation and fibrosis, should be considered when treating shunt failures.

There are five principal causes for excessive bleeding in the immediate postextraction phase ; (1) Vascular wall alteration (wound infection, scurvy, chemicals, allergy) (2) Disorders of platelet function (genetic defect, drug-aspirin, autoimmune disease) (3) Thrombocytopenic purpuras (radiation, leukemia), (4) Inherited disorders of coagulation (hemophilia, Christmas disease, vitamin deficiency, anticoagulation drug-heparin, coumarin). If the hemorrhage from postextraction wound is unusually aggressive, and then dehydration and airway problem are occurred, the socket must be packed with gelatine sponge(Gelfoam) that was moistened with thrombin and wound closure & pressure dressing are applied. The thrombin clots fibrinogen to produce rapid hemostasis. Gelatine sponges moistened with thrombin provide effective coagulation of hemorrhage from small veins and capillaries. But, in dental alveoli, gelatine sponges may absorb oral microorganisms and cause alveolar osteitis (infection). This is a case report of bleeding control by continuous rubber strip & iodoform gauze drainage (without gelfoam packing) of active bleeding infection sites of three teeth extraction wounds in a 46-years-old female patient with advanced liver cirrhosis.
