2.Whole Lumbar Spinal Subdural Hematoma with Progressive Paraplegia after Lumbar Spinal Epidural Injection

Spinal hematomas are a rare but serious complication that is typically observed in the epidural space. Spinal subdural hematomas are a dangerous clinical situation because of their potential to cause significant compression of the neural elements and can be mistaken easily for spinal epidural hematomas. This paper reports a case of a severe whole lumbar subdural hematoma after a simple epidural injection that was treated with surgical decompression with excellent clinical results.
3.A Clinical Study of Nasolabial Cyst.

BACKGROUND AND OBJECTIVES: Being probably of embryonal in origin, the nasolabial cyst is relatively rare and is thought to arise either from the epithelial remnants entrapped along the lines of fusion during the development of face or from the remnants of the developing nasolacrimal duct. The purpose of this study was to examine the clinical and radiological features of nasolabial cyst in order to provide basis for its correct diagnosis and treatment. MATERIAL AND METHODS: The nine cases of nasolabial cyst which were treated in Masan Samsung Hospital from January in 1991 to April in 1997 were the used as subjects. These cases were examined according to age and sex distribution and their clinical features, radiologic and histologic findings, treatment and prognosis, were studied. RESULTS: In our study of nine cases of nasolabial cyst, more females than males were observed, and with respect to age, individuals in their Fortise were more frequently noted. Also, swelling of nasolabial fold were the most frequently observed among symptom and signs. Computerized tomography shows well-circumscribed cystic mass lateral to pyriform aperture. Seven cases underwent operation. Six cases were excised by intraoral sublabial approach, one case which forms fistula to skin was removed by transcutaneous approach. CONCLUSION: Nasolabial cyst is benign harmatomatous cyst of face. This lesion is often unrecognized or confused with other intranasal mass, other fissural and odontogenic cysts, midface infection, or swelling in the nasolabial area. Therefore careful clinical and radiologic evaluation should be considered in the differential diagnosis.

4.Relationships and Usefulness of Cervical Lateral Radiographs Compared with Whole-Spine Lateral Radiographs for Evaluating Cervical Sagittal Alignment

Objectives:
This study was conducted to identify the correlations and usefulness of lateral cervical radiographs compared with wholespine sagittal lateral radiographs for evaluating cervical sagittal alignment.Summary of Literature Review: Few reports have compared cervical lateral radiographs with whole-spine sagittal radiographs.
Materials and Methods:
We retrospectively analyzed 181 patients with both cervical standing lateral radiographs and whole-spine standing lateral radiographs. The radiographs were evaluated using the following sagittal alignment parameters: C2-7 lordosis, C2-7 sagittal vertical axis (SVA), T1 slope, spino-cranial angle (SCA), and whole-spine SVA. We evaluated the relationships between the two radiographs. The patients were divided into four groups according to age and the measured sagittal parameters were compared across groups. An analysis according to clinical symptoms was also done.
Results:
C2-7 lordosis was 16.86° and 15.76°, C2-7 SVA was 15.76° and 16.86°, T1 slope was 29.03° and 22.49°, and SCA was 74.74° and 74.5°, respectively, on the cervical and whole spine radiographs. The whole-spine SVA was -2.64 mm. Correlation analysis between the two types of radiographs showed significant relationships for each sagittal parameter. The whole-spine SVA was related with wholespine C2-7 lordosis, T1 slope, and SCA. Cervical C2-7 lordosis and the whole-spine SVA increased with age. A decrease of C2-7 lordosis on whole-spine radiographs was shown in patients with neck pain.
Conclusions
Cervical SVA and T1 slope were lower on whole-spine standing lateral radiographs than on simple cervical lateral radiographs. The cervical sagittal parameters measured on cervical radiographs were correlated with those measured on whole-spine radiographs. Cervical radiographs can be used to replace whole-spine radiographs with due consideration of these findings.

8.A comparison study of the effects of handpiece speed on teeth in debonding procedure.

This study was performed to examine treatment efficiency and patient discomfort rate according to used handpiece speed in clean-up technique. Brackets were bonded to extracted human premolar(50 teeth). After debonding, 50 extracted human premolar were divided into each two groups( low speed handpiece group with tungsten carbide bur and high speed handpiece group with ultra-fine diamond finishing bur ) of 25 according to used handpiece speed in clean-up technique. In clean-up procedure, teeth vibration and pulp thermal changes were measured. After clean-up procedure, the enamel surfaces of randomly selected 10 teeth from each two groups were taken by SEM and evaluated. The findings of this study were as follows ; 1. During resin removal, tooth vibrations of various amplitude in low speed handpiece group were more than those of high speed handpiece. 2. The pulpal thermal changes of high speed handpiece group were significantly higher than those of low speed handpiece group, also the resin removal time in high speed handpiece group was almost as twice as in low speed handpiece group. 3. The figures of SEM to enamel surfaces after resin removal showed that notches and resin remnants in high speed handpiece group were more than those in low speed handpiece group.

9.The Influence of Thyroid Function on Bone Mineral Density in Korean Postmenopausal Women.

OBJECTIVES: Osteoporosis, defined as decreased bone mass and structural deterioration of bone, increases the incidence of fractures. Recently, there have been reports suggesting that thyroid hormones are related to bone mineral density (BMD). It has been reported that low normal circulating thyrotropin (TSH) levels correlate with lower BMD and that thyroxine (T4) and bone density are negatively related. This research aims to examine the relationship between BMD and thyroid diseases and other functional changes in postmenopausal women. METHODS: The medical records of 2,279 postmenopausal women who attended the health care clinic in eight university hospitals between March 2001 and December 2007 were reviewed retrospectively. We determined the baseline characteristics of the women, including age, height, weight, and body mass index (BMI). The BMD was measured by dual-energy X-ray absorptiometry (DEXA). The correlation between the thyroid status and BMD was analyzed using the SPSS 12.0 program. RESULTS: This research used data from a relatively large number of postmenopausal women gathered in a multicenter approach. Of the thyroid functional tests, thyroid stimulating hormone (triiodothyronine or T3) and T4 correlated with BMD, while free T4 and TSH did not show a statistically significant correlation. After adjusted age, thyroid function test did not correlate with BMD. Osteopenia was significantly higher in the group with TSH below 0.5 mU/L compared with groups that had normal or high TSH. There was no statistically significant difference in lumbar BMD and total hip BMD among patients with thyroid diseases and healthy patients. CONCLUSION: The level of T3 and T4 correlated well with BMD in Korean post-menopausal women.

10.Bone Response to Hormone Therapy according to Basal Bone Mineral Density and Previous Response to Hormone Therapy.

OBJECTIVES: To investigate bone responses to hormone therapy (HT) according to basal bone mineral density (BMD) and previous responses to HT, as well as the frequency and clinical characteristics of HT non-responders in Korean postmenopausal women. METHODS: We retrospectively reviewed a total of 1,836 postmenopausal women who received HT from seven university hospitals. BMD data at the lumbar spine (LS), femur neck (FN), femur trochanter (FT) and total hip (TH) before HT, and at one, two, and three years after HT were collected. All patients were divided into three groups according to basal BMD: normal, osteopenia, and osteoporosis. RESULTS: Women with a greater loss of BMD during the first year of HT were more likely to gain BMD in the second year at any of the four skeletal sites. Bone responses to HT during the third year were not related to the responses during the first year. Mean BMD changes during the first year were significantly higher in the osteoporosis group, but mean BMD changes during the second year were not different between three groups except in LS. The frequency of non-responder (annual BMD losses more than 3%) during the first year was significantly higher in the normal basal BMD group. Mean basal BMDs were higher in the two-year consecutive non-responder group at LS, FN and FT, but those of the three-year consecutive non-responder group were not significantly higher except in FN. CONCLUSION: Most women who lose BMD after HT are likely to gain BMD during the next year. The frequency of non-responders is higher in the higher basal BMD group, and patients with lower basal BMD will be likely to respond better to HT.
