1.Morphometric Study on the Coracoacromial Arch, the Acromial Articular Surface, and the Glenoid Cavit of the Scapula in Koreans.

The present study was performed to provide an anatomical basis of the coracoacromial (CA) arch and the articular surfaces of the scapula which can be applied to the diagnosis and treatment of some common shoulder problems. The standard dimensions and the range of variation of the CA arch, the acromial articular surface and the glenoid cavity were investigated in 114 dry scapulae obtained from 57 (male, 35 ; female, 22) Korean cadavers ranging in age from 18 to 97 years (average age of 67). The results were as follows : 1. The length (46.3mm), width(25.2mm), thickness (8.2mm) and height (4.5mm) of the acromion were measured. The length, width and thickness were significantly larger in the males. The slope of the acromion was 51.5°, and the slope of the scapular spine was 118.5°. 2. The height (13.5mm), slope of the root (138.4°) and the horizontal part (25.3°) of the coracoid process, and the angle between the root and the horizontal part (106.6°) were measured. There were no significant differences between sexes and sides in all morphometric values related to the coracoid process. 3. The length (67.6mm) and height (24.7mm) of the CA arch, the height of the CA ligament from the supraglenoid tubercle (13.1mm), and length of the CA ligament (27.6mm) were measured. Both the length and height of the CA arch and the length of the CA ligament were significantly larger in the males. The slope and anterior and posterior angles of the CA arch were 16.8°, 42.2°, and 34.7°, respectively. 4. The long (13.8mm) and short (8.0mm) diameters of the acromial articular surface were measured, and both diameters were significantly longer in the males. The acromial articular surface was 8.4mm away from the tip of the acromion and extended 1.4mm inferiorly below the inferior surface of the acromion. 5. The long (34.8mm) diameter, and superior (15.0mm), middle (19.5mm), and inferior (25.6mm) short diameters of the glenoid cavity were measured. The long and both superior and inferior short diameters were significantly longer in the males.

5.Medial Plantar Island or Free Flap for Heel Reconstruction.

Reconstruction of soft tissue defects in the heel, which is weight bearing area, represents a challenge because this area requires similar tissues to the original lost tissue, durable and sensible skin coverage, minimal donor morbidity and simple operative procedure. The instep region of the plantar surface of the foot presents an ideal tissue reserve for the reconstruction of soft tissue defects in the heel because of a sensate and unique form of skin with thick glabrous plantar skin, shock-absorbing fibrofatty subcutaneous tissue and plantar fascia. A total of 10 patients with soft tissue defects in the heel were operated from April 1995 to October 2001. The medial plantar fasciocutaneous island flap was used in 8 cases and the free flap in 2 cases. The size of flap varied from 3x3 cm to 10x10 cm. Follow-up period ranged from 2.3 years to 8.5 years. All flaps were viable and durable to chronic weight bearing and shearing force. Also, all flaps had enough sensation in normal activities. In conclusion, the medial plantar fasciocutaneous island or free flap can be used satisfactorily for soft tissue coverage in the heel which is weight bearing area.

6.Young Men with Acute Myocardial Infarction Review of their Clinical Characteristics and Coronary Angiographic Findings.

BACKGROUND: Not rarely we can find young people with acute myocardial infarction(AMI), many studies revealed they have fewer risk factors and less severe coronary angiographic abnormalities than middle and old aged group. METHODS: We studied clinical characteristics and coronary angiographic findings of 5 young men with AMI treated at Masan Koryo General Hospital from June 1986 June 1992. RESULTS: The age ranged between 19 and 32 years(mean 25.4). They had no other risk factors except cigrarette smoking(4 out of 5). Their coronary angiograms revealed no significant lesion in each infarct related artery. After discharge, all patient remained asymptomatic. CONCLUSION: AMI in young man might be related with cigarette smoking, and coronary artery spasm and/or thrombosis may play a significant role in its pathogenesis.

8.Preoperative Erythropoietin Administration in Patients With Prostate Cancer Undergoing Radical Prostatectomy Without Transfusion.

PURPOSE: In this study, we administered erythropoietin preoperatively to patients who underwent open radical prostatectomy without transfusion to increase their hemoglobin levels and investigated the efficacy of this procedure. MATERIALS AND METHODS: We evaluated 62 patients who underwent open radical prostatectomy performed by the same surgeon between June 2005 and January 2011. The 22 patients who refused transfusion were assigned to group 1; the patients who accepted transfusion were assigned to group 2. Before surgery, we administered erythropoietin beta to group 1 patients whose hemoglobin levels were <12 g/dL and retrospectively compared the clinical data of the two groups. We used the t-test and the chi-square test for statistical analysis. RESULTS: Mean preoperative hemoglobin levels in group 1 after erythropoietin administration (14.5 g/dL) were significantly higher than those in group 2 (13.59 g/dL, p=0.003). Moreover, the difference in the mean hemoglobin levels before and after surgery for group 1 patients (3.55 g/dL) significantly exceeded that for group 2 patients (2.08 g/dL, p=0.000). Additional analysis revealed no statistically significant differences in perioperative complications between the groups. CONCLUSIONS: Preoperative erythropoietin administration increased the safety margin of hemoglobin levels, and this strategy worked sufficiently well in our experience.

9.Primary Adenocarcinoma of the Rete Testis With Preceding Detection of Unilateral Hydronephrosis.

Primary adenocarcinoma of the rete testis is a rare malignant testicular tumor with a poor prognosis. Here we report the case of a 54-year-old man with right hydronephrosis that was detected before the diagnosis of the primary testicular lesion. During the evaluation of the right hydronephrosis by use of abdominopelvic computed tomography, a painless, hard, solid lesion was found on the right testis. The patient underwent radical orchiectomy, and the pathologic examination revealed an adenocarcinoma of the rete testis. Multiple metastases were present at the time of diagnosis. The patient received combined chemotherapy after the surgery but lived only 8 months after the initial diagnosis.
