3.Metastatic Angiosarcoma of the Lung: HRCT Findings.

We describe a case of cavitary metastasis to the lungs from a small angiosarcoma of the scalp, in which themetastatic lesions were complicated by pneumothorax and pulmonary hemorrhage. On high-resolution CT, the lesionssimulated the findings of Langerhans cell histiocytosis. Thin-walled cavitary metastatic lesions were similar tothose of thin walled air cysts in Langerhans cell histiocytosis. Ground-glass opacity simulated the findings ofsmoke r's respiratory bronchiolitis in Langerhans cell histio-cytosis but histologically represented hemorrhageduring metastasis of the angiosarcoma.

5.Granulomatous Colitis: Findings on Double Contrast Barium Enema and Follow-up Studies.

PURPOSE: To evaluate the radiologic findings of granulomatous colitis on double contrast barium enema and changes on follow-up studies. MATERIALS AND METHODS: Serial double contrast barium enemas of six patients with granulomatous colitis confirmed by endoscopic biopsy were reviewed. We analyzed the radiologic findings and their follow-up changes, including aphthous ulcers, lymphoid hyperplasia, deep ulcers, cobble stone appearance, geographic ulcers, asymmetric involvement of ulcers, skip lesions, sinus tract, fistula formation, pseudosacculation, focal stricture, and small bowel involvement. RESULTS: Pretreatment double contrast baruim enema findings were aphthous ulcers in five patients, deep ulcer in six, cobble stone appearance in five, longitudinal geographic ulcers in two, fistulas in one, pseudo-sacculations in two, focal stricture in one, and pseudopolyps in six. Also, anal ulcers were observed in two patients, asymmetric involvement of ulcers in three, skip lesions in four, and small bowel involvement in five in five patients proved to have inactive disease after treatment, aphthous ulcers and deep ulcers disappeared. Geographic ulcers of two patients and anal ulcer of one patients decreased in size or depth. Pseudosacculation in one patient disappeared. Pseudopolyps decreased in two patients, increased in one, and decreased after increase in two. One patient whose disease remained active after treatment showed maintenance or increase of ulcers or fistula. And their pseudosacculation or focal stricture unchanged and pseudopolyps decreased. CONCLUSION: The major radiologic findings of chronic granulomatous colitis on double contrst barium enema are aphthous ulcer, deep ulcer, cobble stone appearance, discontinuity of the lesion, and coexistence of ulcers and pseudopolyps. And, double contrast barium enema is good follow-up modality because its findings correlate with clinical course of the granulomatous colitis after treatment.

7.Laryngeal Granuloma Following General Anesthesia and Anesthetic Experience for Resection of Granuloma - A case report .

The authors have been experienced a case of laryngeal granuloma following endotracheal anesthesia which is rare in children and have used two different method of general anesthesia for resection of granuloma. These two methods are; 1) using endotracheal tube insertion for general anesthesia. 2) applying general anesthesia via under mask. Comparing with two different methods of general anesthesia, we come to the conclusion that the former is more reliable for clinical anesthesia because of supplying continuous positive pressure with oxygen and anesthetics in spite of poor operation field.

8.Clinical Effects of Propafenone and Disopyramide on Ventricular Premature Complexes: Double-Blind, Placebo-Controlled, Randomized Cross-Over Study.

BACKGROUND: Propafenone is a new class Ic antiarrhythmic compound.This study was performed to evaluate the clinical efficacy and safety of propafenone by double-blind, placebo-controlled, randomized cross-over comparison of propafenone and disopyramide in patients with stable ventricular ectopy. METHODS: All patients were included in the study if they had an average of at least 30 PVC/hr on a 24-hour Holter recordings. During the 1st 7 days, two placebo tablets(identical in apperance to the propafenone and the disopyramide tablets) were administrated in a double-blind manner(run-in period). Then 1st treatment period lasting 1 week with one verum and the other placebo, wish-out period of 3 day,2nd treatment period lasting 1 week with cross-over drugs were followed. RESULTS: Twenty patients were enrolled. During the run-in period, VPCs were reduced to 18%, compared to the baseline data before the administration of placebo.During the treatment period,propafenone 600mg/day reduced VPCs by 43% and disopyramide 400mg/day reduced VPCs by -10% Propafenone was effective(80% or greater reduction of VPCs) in 7 of 20 patients. Disopyramide was not effective in all patients. Propafenone and disopyramide produced no significant change of paired VPCs and VT events. Propafenone had no effect on heart rate. It increased the PR interval(7.9%;p<0.01) and QRS interval(5.2%;p<0.01). The drug did not change QTc interval(-1.1%) significantly. There were no cardiovascular side effects. Propafenone produced nausea in one patient. Disopyramide produced dysuria in 2 patients. CONCLUSIONS: Propafenone was more effective in controlling VPC than disopyramide, and there was no major limiting side effects.

9.The comparative study of three-dimensional cephalograms to actual models and conventional lateral cephalograma in linear and angular measurements.

Conventional cephalometrics have inherent errors because their evaluation is performed in two-dimension for threedimensional object. To compensate these errors, three-dimensional cephalograms - derivation of three-dimensional data from conventional lateral and postero-anterior cephalograms - were developed. In this study, the accuracy and precision of three dimensional cephalograms were determined by means of 10 linear and 12 angular measurements on 36 acrylic skull models and by the comparison of conventional lateral cephalograms. The results were as follows 1. Mean difference between three-dimensional cephalograms and actual models in linear measurements was 0.94+/-0.62 mm and mean rate of magnification of three-dimensional cephalograms was 100.31+/-0.91%. There were no statistically significant differences between three-dimensional cephalograms and actual models in linear measurements(alpha=0.1). 2. Mean difference between conventional lateral cephalograms and actual models in linear measurements was 6.44+/-1.48 mm and mean rate of magnification of lateral cephalograms was 106.99+/-1.45%. There were statistically significant differences between lateral cephalograms and actual models in linear measurements(P<0.005). 3. Mean difference between three-dimensional cephalograms and actual models in angular measurements was 1.22+/-0.82degrees and mean rate of magnification of three-dimensional cephalograms was 105.71+/-12.07%. There were no statistically significant differences between three-dimensional cephalograms and actual models in angular measurements(alpha=0.1). 4. Mean difference between conventional lateral cephalograms and actual models in angular measurements was 1.70+/-0.94degrees and mean rate of magnification of lateral cephalograms was 106.35+/-15.70%. There were no statistically significant differences between lateral cephalograms and actual models in angular measurements(alpha=0.1). There were similarity between three-dimensional and lateral cephalograms in angular measurements.
