1.Cases with Endometrial Polyp and Endocervical Polyp Associated With Tamoxifen Use.

Tamoxifen is a nonsteroidal triphenylethylene delivative that has been widely used in the treatment of breast cancer. tamoxifen is indicated for the adjuvant treatment of breast cancer in women following total mastectomy or segmental mastectomy, breast irradiation and chemotherapy. tamoxifen has been shown to have significant benificial effect in the treatment of breast cancer patient as hormonal therapy. However ,there is mounting evidence that tamoxifen may affect other hormon sensitive organs, including the uterus and ovaries. An increased risk of endometrial polyps, endometrial hyperplasia and adenocarcinoma has been reported in tamoxifen treated women. We have met cases of endometrial polyp and endocervical polyp which were associated with tamoxifen use, after modified radical mastectomy for infilterating ductal carcinoma of breast . So we report these cases with the brief review of literatures.

2.Transarterial Guglielmi Detachable Coils Embolization with Stenting for the Treatment of a Traumatic Carotid Cavernous Fistula: Case Report .

Embolization of a carotid cavernous fistula(CCF) by means of a detachable balloon is a well-established method for treating CCFs while preserving a patent parent internal carotid artery(ICA). However, failure to embolize the CCF may occur on a few occasions. Herein we describe a stent-assisted Guglielmi detachable coil embolization that completely occludes the fistulous opening rather than fills the cavernous sinus. By applying this technique, we successfully treated a CCF, without compromise of the parent ICA in patients who has failed with balloon technique previously.

3.Histopathologic Comparison of Vascularization between Dacron and Donor Sclera as Wrapping Material in Hydroxyapatite Implantation.

Recently, a hydroxyapatite sphere has been increasingly used as a buried integrated orbital implant for the great improved motility of the prosthesis after enucleation. To allow more rapid ascess of vasculature into the hydroxyapatite implant, Dacron as a new synthetic wrapping material of the implant for anophthalmic sockets had been used and compared histologically with donor sclera in rabbits. At 4 weeks after implantation, fibrovascular ingrowth was found mainly near the scleral windows, where fibrosis extended 2 mm centrally in scleral-wrapped implants. In Contrast, in Dacron-wrapped implants, there was extensive fibrovascular infiltration through Dacron filaments and windows as well, making the hydroxyapatite sphere completely invested with fibrovascular tissue to the center within 4 weeks of implantation. Thus, Dacron as a new wrapping material appears to allow a more rapid vascularization into the hydroxyapatite implant than other collagen material without any serious complications, resulting in early rehabilitation with an artificial eye.

4.Effects of Estrogen on the Urinary Symptoms and Urodynamic Parameters in Postmenopausal Women.

Effects of oral estrogen (Tibolone) on the urinary symptoms and urodynamics parameters were assessed by a double-blind prospective trial in 24 postmenopausal women. Patients were assessed subjectively and objectively before and after treatment by symptom score change, urethral pressure profiles and uroflow. There was statistical difference in the subjective response (symptom score) to treatment between the two groups (p<0.001), there was no significant urodynamic changes (p> 0.05). Although there was no significant urodynamic changes, estrogen treatment in postmenopausal women was effective to improve urinary symptoms. Careful observation with periodic follow up will be needed because of possible harmful side effects of estrogen.

5.Effect of Gl Cyclins Expression on Clinical Prognostic Parameters in Cervical Carcinoma.

Alterations in the expression of genes that control the cell cycle may be of critical importance in tumorigenesis and malignant transformation. The major regulatory events leading to cell proliferation occur in Gl phase of cell cycle, and the deregulated expression of Gl cyclins is related to oncogenesis. Cyclins Dl and E play important roles in the progression of cell through Gl phase of the cell cycle. Amplification and/or overexpression of the cyclin Dl gene and aberrant expression of cyclin E has been described in various forms of human cancer. However, the role of cyclins Dl and E in cervical cancer has been poorly defined. In this study, we examined the expression of cyclins Dl and E by Northern blot technique and the status of human papil- lomavirus(HPV) type 16 and 18 by polymerase chain reaction in 25 cases of cervical carcinoma to explore the relationship between cyclins Dl and E and cervical cancer. We found cyclin Dl expression showed down-regulated expression in cervical cancer but cyclin E expression was increased in cancer group. Other clinicopathological prognostic factors were not correlated with cyclins D1 and E expression. Further study based on larger numbers of cases with correlation of cyclins D1 and E status and survival data will be needed to elucidate the use of cyclin expressions as prognostic factor.

6.Clinical Applications of Plasma B-type Natriuretic Peptide Assays in Preterm Infants with Patent Ductus Arteriosus.

Hemodynamically significant patent ductus arteriosus (hsPDA) in preterm infants is a common and important problem associated with significant morbidity and mortality including: intraventricular hemorrhage, necrotizing enterocolitis and bronchopulmonary dysplasia. The rapid and accurate determination of the indications for therapeutic closure of hsPDA in preterm infants is important but often difficult. Currently used tools, such as the clinical findings, including heart failure and the typical echocardiographic features of hsPDA, have unsatisfactory sensitivity and/or specificity. Over the last decade, measurement of plasma B-type natriuretic peptide (BNP) levels using a commercially available bedside testing kit has been proposed to aid in the diagnosis and treatment of hsPDA in preterm infants. The present review analyzes the clinical usefulness of a rapid plasma BNP assay for the management of hsPDA in preterm infants and speculates on potential future therapeutic directions, including predicting subsequent hsPDA for early targeted treatment and predicting of treatment response for decision-making regarding surgical intervention before and after medical treatments.

7.Retroperitoneal Liposarcoma (Pleomorphic Type): A Case Report.

A 66-year-old woman was admitted to our hospital because of a mass in the left abdomen. Ultrasonography, aortography and computed tomographic (CF) scanning revealed the mass occupying the entire left abdominal cavity and the left kidney displaced by huge and avascular mass. Also CT guided needle biopsy demonstrated liposarcoma. At operation, a huge yellow-brownish tumor found in the retroperitoneal space, was removed incompletely due to severe adhesion to the left external iliac artery The tumor was 12x11x10cm and weighed 560gm. Histologically, it was a pleomorphic type liposarcoma. She bas received adjuvant radiotherapy but recurred 10 months later.

8.Histopathologic study of Dacron Graft following Experimental Fun-thickness Scleral Resection in Rabbits.

To evaluate the possibility of Dacron patch for artificial sclera. Dacron(polyethylene terephthalate) which is readily available, biocompatable, safe, and inert synthetic material was implanted in the area of full-thickness scleral defect in rabbits. The histopathologic examinations showed that fibrovascular tissue invested into all interstices of Dacron filaments and fibroconnective tissue encapsulation around Dacron patch had strong connection with collagen fibers of recipient sclera at 4 weeks after graft There were no adverse reactions such as retinal detachment and fibrous proliferation into the vitreous cavity except mild inflammatory response of surrounding tissue at the early postoperative stage. As a result, Dacron graft showed successful firm adhesion to the recipient sclera without serious complications. Thus, We think that Dacron could possibly be used, instead of donor sclera, as artificial scleral graft material for the surgical treatment of perforating scleral diseases.

9.A Clinical Study of Herpes Zoster During the Last 10 Years.

BACKGROUND: Herpes zoster is characterized by vesicular skin lesions over the unilateral sensory dermatomes being caused by the reactivation of varicella-zoster virus and its incidence seems to be increasing recently. OBJECTIVE: The purpose of this study was to elucidate the epidemiology and the clinical characteristics of herpes zoster. METHODS: The authors observed 877 cases of herpes zoster clinically for annual incidence, age, sex, monthly distribution, predilection site, associated disease, and complications at the Department of Dermatology in Chung-Ang University Hospital during a 10 years period(1982.9-1992.8). RESULTS: 1) The annual incidence averaged over 10 years was 2.26%(877 cases of total 38.717 outpatients) being on the increase recently. 2) The age distribution was in the range of 2-86 years old being most prevalent in the 6th decade and there was equal sex ratio(444:433). 3) There was no statistically significant monthly or seasonal variation in the incidence of herpes zoster. 4) The most common anatomical distribution was thoracic dermatome(51.7%), followed by trigeminal(17.1%), cervical(16.1%), lumbar (10.6%), sacral(1.4%)and multiple deramatomic involvement(3.1%). Left or right side was affected in about the same ratio(439:437), and bilateral involvement was in 1 case. 5) Associated disease of herpes zoster were observed in 210 patients(23.9%) ; hypertension(7.0%), diabetes mellitus(4.1%), tuberculosis(1.8%), postoperative status(0.7%), malignancy(0.7%), gastric ulcer(0.7%), and so on, 6) The most common complication of herpes zoster was postherpetic neuralgia(7.4%), followed by eye complication(2.1%), bacterial infection(1.4%), scar formation(0.7%), neurogenic bladder(0.2%), Ramsay-Hunt syndrome(0.2%), motor paralysis(0.1%), herpes zoster generalisatus(0.1%) 7) The incidence of postherpetic neuralgia increased with age and was highest in trigeminal dermatome. 8) Recurrence of herpes zoster was observed in 4 patients(0.5%). CONCLUSION: According to this study, the annual incidence of herpes zoster showed a tendency to increase recently but there was no significant monthly or seasonal variation. The anatomical distribution was most commom on the thoracic dermatome. The incidenc of postherpetic neuralgia increased with age being most common in trigeminal nerve.

10.The Effects of Dimethly Sulfoxide and Sodium thiosulfate for the Prevention of Tissue Necrosis due to Extravasation of Mitomycin-C.

Extravasation of toxic chemotherapeutic 'agents cause severe skin ulceration and necrosis which often need secondary surgical intervention. Still, there were not established antidote agent in case of extravasation with mitomycin-c. Dimethyl sulfoxide is known as an effective chemical scavenger of toxic hydroxyl free radical and sodium thiosulfate also was demonstrated significant protector from mitomycin-c induced ulceration by a few experimental studies. Author investigated necrotic area of mitomycin-c injected site and compare to the effectiveness of topical treatment with dimethyl sulfoxide and intradermal injection of sodium thiosulfate according to starting times, forty five mice were divided into 3 groups. Control group(n=5) had no treatment after subcutaneous injection of mitomycin-c. Experimental group I and 11 were 20 mice treated dimethyl sulfoxide and sodium.
