2.A Case of Cutaneous Polyarteritis Nodosa.

Cutaneous polyarteritis nodosa(CPAN) is a benign form of rare vasculitis of small and medium-size arteries with a recurrent but benign course without systemic involvement. We experienced a 61-year-old male who had two months history of multiple deep-purpurish livedo reticularis on both lower legs. Noncutaneous manifestations including malaise, fever, myalgia, and arthritis were absent. A skin biopsy specimen from the livedo reticularis on the leg showed perivascular and trans-mural neutrophilic and lymphocytic infiltration of medium-sized arteries in the dermal-subcutaneous junction and fibrinoid necrosis of the vessel walls. The patient was treated with colchicine for 2months and showed markded improvement.

7.The Treatment of Old Unreduced Dislocation of Shoulders: A Report of two cases

Two cases of old unreduced dislocation of shoulder are reported. One was the posterior dislocation for six months with malunited fracture of humeral shaft in thirty degree lateral angulation. Her shoulder and arm aches with limitation of motion remaining only ten degrees in every direction but abduction in thirty degrees, The malunion of humeral shaft and shoulder were reduced surgically in result of eighty degrees abduction, seventy degrees flexion, sixty degrees extension and twenty degrees of external rotation with free of pain. Another case was an anterior dislocation for one year. Her range of shoulder motions are almost normal degree but brachial plaxus compression symptoms. Open reduction in practically Putti-Platt procedure was performed. She became completely free of brachial plexus symptom and ranges of shoulder motion are normal except about fifteen degrees less external rotation than preoperative condition. The surgical indication and the result of treatment were discussed for the the neglected cases of old unreduced dislocation of the shoulder.

8.Chronic Hydroxyurea-induced Dermatomyositis-like Eruption Showing Epidermal Dysmaturation.

Hydroxyurea is an effective treatment for a variety of myeloproliferative disodrers. A distinct cutaneous reaction to long-term administration of hydroxyurea has been characterized and designated hydroxyurea dermopathy. Epidermal dysmaturation refers to histologic changes that may be observed in the epidermis after any significant cytoreductive therapy. We report a patient with hydroxyurea-induced dermatomyositis-like eruption showing epidermal dysmaturation who developed an erythematous scaly patches on the dorsal aspects of the hands while on long-term administration of hydroxyurea for chronic myelogenous leukemia.

9.The Etiology and Management of Female Urinary Retention.

The incidence of acute urinary retention in females is very low but the causes are diverse. So, it is important to verify the causative diseases trough history taking, physical examination including neurologic tests and it is essential to find adequate therapeutic modalities. MATERIALS AND METHODS: We experienced 45 female patients who visited under the chief complaint of acute urinary retention and reviewed, evaluated retrospectively or prospectively during 4 years. The patients are divided 1) transient causes: immobilization after surgery, constipation, medications, herpes zoster, psychogenic causes 2) neurologic causes: cerebral concussion, peripheral neuropathy, brain tumor, herniated nucleus pulposus, spinal cord injury, previously undiagnosed diabetes 3) pelvic causes: difficult labor, uterine prolapse,, pelvic bone tumor, vaginal invasion of rectal cancer, imperforate hymen. RESULTS: Screening test were done through physical examination, history taking, routine urinalysis. Most patients recovered voiding after correction of underlying diseases. CONCLUSIONS: The patients who showed persistent retention were further evaluated through neurologic tests, urodynamics, pelvic ultrasonography and serious diseases as blain tumor, polyneuritis, metastaic pelvic bone tumor were diagnosed.
