3.A Study on the Frequency of the Atopic Disease in the Parients with Allergic Rhinitis and their Families.

The purpose of this study was to elucidate the prevalence rate of tbe concomitant; atopic diseases in the patients with allergic rhinitis and, concurrently, the incidence of the atopic diseases in the families of the patients. A total of 40 patients who were referred to the department of dermatology, Kang Nam St. Marys Hospital, for the confirmative examinations of allergic srhiniti were taken for this study. The results were as follows, 1. The prevalence rate of concomitant atopic dermatitis in the sample patients was 22.5% and that of concomitant bronchial asthma was 12. 5% The prevalence rate of concomitancy with both diseases was 2.5% and that of concomitancy with atopic dermatitis and/or bronchial asthma was 37.5%. 2. The frequency of the associated manifestations, in descending order, was allergic conjunctivitis (30%), urticaria or angioedema (25%), insect sting (22.5%), oral symptoms (5%), migrane or headache (5%), immediate drug reactions (2.5%) and gastrointestinal symptoms (2.5%).

4.Prick Test Reactions in the Patients of Allergic Rhinitis, Asthma and Urticatia.

Since the recognition that certain conditions such as allergic rhinitis, asthma and urticaria are due to the substances which are innocuous to the individuals, it has been a common practice to establish a diagnosis by reexposure of the individual to the suspected subsutance. Their pathogenesis was classified into immunologic and nonimmunologic mechanism. The immunologic mechanism in such conditions is mainly due to interaction between antigen and IgE skin-sensitizing antibody bound to tissue mast cells. There are two approaching methods in evaluating allergic disease. One is in vitro test which includes R.A.S.T. and leukocukocyte histamine release test and the other method is in vivo test which includes provocation test and skin tests. Prick test, a kind of skin test, is most frequently used in clinical procedure because of its simplicity and safety of appliation and better correlation with clinical history as well as specificity and highly reproducibility. It is well accepted that the therapeutic principle in allergic disease is to find out the causative allergens and get rid of them from patient's environment. If the causative allergens are inavoidable, the patient should be treated by means of specific hyposensitization. In evaluating allergic disease, the patient's clinical history is most important. 5.Splenectomy in Hereditary Spherocytosis in Childhood.

Among the erythrocyte membrane defects, hereditary spherocytosis is the most common. The erythrocyte membrane defect results from a deficiency of spectrin, the most important structural protein in red cell. Hereditary spherocytosis often presents with hemolytic anemia, jaundice, moderate splenomegaly. Diagnosis is established by the presence of spherocytes in the peripheral blood, reticulocytosis, an increased osmotic fragility, and a negative Coombs test. In children, splenectomy is usually performed after age 6 years but can be done at a younger age if warranted by the severity of the anemia and the need for frequent transfusions. In the period December 1987 to Agust 1993, 9 patients with hereditrary spherocytosis underwent splenectomy and the following results were obtained. 1. Nine patients were comprised of five males and four females. 2. Five patients(55.6%) had been admitted to our hospital during age 6-10 years. 3. Four of the nine patients had autosomal dominant inheritance with variable expression. The other five patients had no known inheritance. 4. The diagnosis of the spherocytosis was based on the increased osmotic fragility and increased autohemolysis of the erythrocytes, as well as on the appearance of spherocytes in the peripheral blood smear. 5. In all cases splenectomy was performed. Two patients had concomitant gall stones and choledocholithiasis, respectively. One patient with concomitant gall stones underwent simultaneous cholecystectomy and splenectomy. The other patients associated with choledocholithiasis underwent splenectomy, cholecystectomy, choledocholithotomy, and T-tube drainage. 6. Complete hematologic recovery was obtained by the splenectomy in all cases. 7. Postoperative complication was not occurred.
