3.Arthrolysis and Quadriceps Release in Stiff Knee: Report of Five Cases

Many procedures of quadricepsplasty were reported by Thompson (1944), Judet (1959), van Nes (1962), Nicoll (1963) and Hesketh (1963) in confining the indication of them for the strictly extra-articular origin of their causes. During the period of January 1979 to August 1979, 5 cases of stiff knee joints were surgically treated by means of arthrolysis and quadriceps release. The result were gratifying. The average gain of flexion was 119 degrees, and extension lag was absent except a case which had compression fracture of femoral condyle.

4.A Clinical Study on the Supracondylar Fractures of the Femur

The management of supracondylar fractures of the femur remains controversial. Most studies over the past twenty years have attempted to compare the results of non-surgical with those of surgical methods. Until a few years ago, conservative treatment was considered superior to internal fixation of supracondylar fractures of the femur. However, the development of new fixation devices and techniques have, according to several investigations, improved the results of the treatment of these fractures. The following clinical results were shown by analysis of 96 cases of supracondylar fractures of the femur treated in the Department of Orthopedic Surgery, Yonsei University, College of Medicine during the past 10 years from Jan. 1971 to Dec. 1980. 1. The prevalent age distribution was between 21 and 50 years of age (79.1%), and the ratio between males and females was 3.4:1. The most common cause of injury was car accidents (51.0%). 2. A classification of supracondylar fractures was unicondylar, simple supracondylar and intercondylar. Intercondylar which was subdivided into Type I, II-A, II-B, III according to Neer's classification. 3. Fifty nine patients (61.4%) were associated with injuries of other parts and the most frequent associated fracture was tibial fracture and the most common associated soft tissue injury was cerebral concussion or contusion. 4. The methods of treatment: conservative management by using skeletal traction for 34 cases, surgical treatment by open reduction and internal fixation for 58 cases and A-K amputation for 4 cases due to populiteral artery injury. 5. Fifty nine cases were suitable for result analysis. The satisfactory result of surgical treatment was 63.4% and conservative treatment was 50.0% respectively. The factors affecting the final results were Type of fracture, severity of injury, and rigid internal fixation. 6. The main complications of the supracondylar fractures of the femur were delayed union (10.2%), infection (10.2%), and traumatic arthritis (13.3%). 7. Finally, the results of treatment depend largely on anatomical reduction, rigid fixation, early joint motion.

8.Pelvic Fracture Classification, Associated Injury and Hemodynamic Change.

In general, pelvic fracture carries higher mortality especially from motor veicle accident, we have retrospectively reviewed charts and radiographs of 185 patients in order to identify clinical fading, kinds of fracture, hemodynamic changes and associated injuries in patients with pelvic fractures. We attempted to classify pelvic fractures according Young classification. The following results were obtained; 1. On age and sex distribution, the pelvic fractures showed peak incidence between 2nd decade and 5th decade. Male was affected more frequently than female, the ratio being male to female, 2.6 to 1. 2. The mechamisn of injury was motor veicle accident in 155(83.8%), fall in 25(13.5%), others in 5(2.7%). 3. The associated injuries of pelvic fractures: extremity, thoracic, urinary system, intraperitoneal and head injury. 4. Of the 185 cases of the pelvic fractures, simple pelvic fractures are 101 cases(54.6%), lateral compression in 39(21.1%), anteroposterior compression in 18(9.7%), vertical shearing in 17(9.2%), mixed pattern in 10(5.4%). 5. Retroperitoneal hematoma and urinary tracts injuries were the most common associated in mixed pattern. 6. The incidence of hemodynamically unstable fractures were 19.8% in simple pelvic fracture but, as expected was higher in major pelvic ring disruptions except to acetabular fracture(52.4%). 7. Mortality rates were 5.9% in all pelvic fractures but 10.7% in major pelvic ring disruptions except to acetabular fractures. Prognostic factor in pelvic fractures were the amount of hemorrhage and associated injuries, therefore early diagnosis and managements are essential to reduce the mortality in pelvic fracture.

9.A Case of Trichorhinophalangeal Syndrome.

Trichorhinophalangeal syndrome is a genetic disease and divided into three differen types. Trichorhinophalangeal syndrome type I is characterixed by alopecia, a bulbous pear-shaped nose and cone-shaped epiphyses in the hand. Diverse clinical manifestations can be observed such as short stature, mandibular abnormality, winged scapula, etc. It is inherited in an autosomal manner, and may cause grave joint abnormalities which should be corrected early in life. We describe a 23-year-old woman with diverse clinical manifestations of trichorhinophalangeal syndrome type I, including prognathism and a winged scapula, two features which have not been previously described in the Korean literature.

10.Diagnosis of Huntington's disease with Polymerase Chain Reaction.

Huntington's disease is an autosomal dominant neurodegenerative disorder that usually begins in mid-life and is characterized by a progression of involuntary choreiform movements, personality change, and dementia. 4 specific unstable trinucleotide (CAG) repeat expansion in a gene on the short arm of chromosome 4 was recently identified as the pathogenic mutation for this disease. We have analysed the CAG expansion in peripheral leukocyte from a woman suspected with Huntington's disease and her family. A 40-year-old woman visited for the 6 years history of progressing intractable involuntary hyperkinetic movement and antagonistic personality. She showed bilateral caudate nucleus atrophy with mild enlargement of both frontal horn at brain MRI and showed the decrement of glucose metabolism in both basal ganglia at 18F-FDG PET scan. We also studied about the clinical manifestations of her family. Her younger brother also showed mild cognitive impairment and dysarthria. She and her relatives (n = 6) were tested for the existence of high risk allele of Huntington's disease by polymerase chain reaction method. The high risk allele (above 40 CAG repeat) in the 1715 gene was confirmed in 6 persons including the patient. The CAG repeat variance was 46 to 54. Only one person showed the normal range of CAG repeat.
