1.An immunohistochemical study of plasma fibronectin expression in basal cell carcinoma.

Previous observations have demonstrated that fibronectia (FN) is deposited high abundance in basal cell carcinoma (BCC) stroma. Since FN is a glycoprotein which promotes interaction between cells and extracellular matrix, it may been important component of slowly expansile carcinoma such as BCC. Furthermore, BCCs aggressiveness varies with it's histologic appearance. The authors investigated distribution of plasma FN (pFN) in 8 BCC cases with various histologic types (3 nodulocystic type, 2 infiltrative type, 2 keracotic type, 1 adenoid type) and 4 squamous cell carcinomas (SCC), which have a more aggrcisive biologic behavior than BCC using immunohistochemical staining. In normal skin, pFN was strongly positive at the clenzoepidermal junction and perivascular area with a linear pattern, and in the papillary dermis with a reticular pattern, and was weakly positive in the reticular dermis. In BCC, pFN was strongly positive in nodulocystic BCC, with the highest concentration along the periphery of the tumor island in a filamentous pattern, .hereas, it was moderately positive in adenoid, keratotic, infiltrative BCC stroma. In SCC, pFN was weakly positive around tumor the margins with a thin, coarse deposition These results show that with epidermal tumors, the degree of the pFN expression in stroma is propotional to their aggressiveness.

2.Report of A Case of Situs Inversus Totalis.

A case of situs inversus totalis was observed in the cadaver of a 35 year old Korean female and the anatomical structures were investigated. The fact that she had received intestinal resection due to tuberculosis of intestines was confirmed. The results are summarized as follows : 1. All thoracic and abdominal viscera were completely transposed and in the left lower quadrant of abdomen, it was observed that the distal portion of the small intestine, cecum, appendix and part of the ascending colon had been resected. 2. Classic mirror-image dextrocardia with patent foramen ovale (large diameter, 7mm), sinusitis in the left maxillary sinus, bilateral thoracic duct, variation of the opennig site of the common bile duct and the main pancreatic duct, and maldvelopment of the left adrenal gland were observed together with the sinus inversus totalis.

3.A Changes of Electrolytes and Biochemical Components during Hemodialysis.

To investigate the changes of electrolytes and bio-chemical components during hemodialysis in 15 patients(7 men and 8 women) with end-stage renal disease undergoing long-term hemodialysis were studied. The mean age of 15 patients undergoing long-term hemodialysis was 45 year(range ; 26-62). The mean duration of hemodialysis was 58 months(range: 19-143) at time of the study. Hemodialysis was performed with acetate dialy- sate having 37mEq/1 acetate and 8.57g/100ml glu-cose(group A), with bicarbonate dialysate having 8 mEq/1 acetate and 30mEq/1 bicarbonate without glu-cose(group B) for 4-hour. Each blood sample was drawn for blood gas analysis, blood glucose, blood urea nitrogen and electrolytes from the arterial line at 0, 1, 2, 3, and 4-four during hemodialysis. In group A, the body temperature measured at 1 hour increased significantly compared with that of predialysis. The blood glucose level increased pro-gressively during dialysis. In group B, the body temperature measured at 1 hour also increased sig- nificantly compared with that of predialysis. However, the blood glucose level increased progressively during dialysis as removing urea in blood during dialysis. These results suggested that blood urea nitrogen may be a cause of hypothermia. Mean corpuscular volume(MCV) increased significantly at 2-hour in group A and returned to baseline values at 4-hour. In group B, the mean level of glucose value in dialysate of outlet line of dialyzer increased to 38.3mg/dl from zero in inlet line, and PaC&z decreased significantly because of diffusion of COz across the dialyzer. In Group A, PaOz decreased significantly at 1-hour and returned to baseline values at 4-hour, whereas HCC4 was increased significantly at 4-hour. The intracellular potassium content may decreased in long-term hemodialysis patients cornpared with normal control. In Group A, hernoglobin diphosphoglycerate(2, 3-DPG) concentraion increased from 10.9 pmoVg before dialysis to 19.9pmol/g after dialysis. Despite substantial anemia, hemoglobin 2, 3-DPG prior to dialysis was significantly lower than the value obtained in the normal control(17.5+4.3 p moVg). In group A, the blood lactate level decreased significantly at 1-hour and returned to baseline values at 4-hour. Our data suggested that body temperature might be increased due to removal of urea nitrogen, and blood glucose may be decreased because of the dif- fusion across dialyzer. In addition, this study showed that intracellular potassium content was lowered, the MCV of RBC was increased during hemodialysis. Transient decrement of PaCOz during early phase of dialysis and decreased hemoglobin 2, 3-DPG of despite anemia before dialysis improved with the increment of blood pH and HCC4 at the end of dialysis.

7.Radial Neck Fracture in Children

This paper was prepared to determine the effects of degree of initial angulation and the method of treatment toward the end results, and also to report cases of twenty-one children with a radial neck lowed up more than 12 months. The results are as follows: 1. An average follow up period was 34 months, and were ranged from 12 months to 70 months. 2. Displacement patterns of fracture were angulation in 5 cases, partial translocation in 11cases, and total displacement in 5 cases, and translocated fracture was most frequent. According to the degree of angulation, in 10 cases angulation was minor(below 30 degrees), in 8 cases moderate(30–60 degrees), and in 3 cases severe (above 60 degrees). 3. One case was treated by simple cast immobilization, 9 cases were treated by gentle closed reduction, 3 cases were by percutaneus pin reduction with K-wire, 7 cases were by open reduction and K-wires fixation, and 1 case was by closed reduction and incorporating cast immobilization. 4. The results of 20 cases were excellent in range of motion without specific complication. Without considering about the method of treatment, when angulation was severe, more time was required for recovery(Wilcoxon 2-simple test P < 0.02). When the degree of angulation was less than 60 degree, the functional recovery was faster with closed reduction than with open reduction, but with the angulation more 60 degree, there was no significant difference(Spearman correlation coefficient=0.599, P value < 0.05). In conclusion, the severity of initial displacement and the method of treatment were considered to be factors that could affect the recovery time.

Actinomycosis is an oppoturnistic infection of actinomyces, which are relatively avirulent endogenous oral commensals. After trauma or infection, they breach the normally protective mucosal barriers to invade adjacent soft tissue structures. Lesions routinely contain other bacteria, the normal resident flora at the site of primary infection, which act synergistically with actinomyces species to provoke this unique infection, which range from an acute suppurative process to a chronic fibrotic process. According to epidemic studies about pelvic actinomycosis, it should be significantly related to IUD(intrauterine device). It is accounted that IUD cause chronic intrauterine infection, tissue injury and act as nucleus for parasitic infestation. Here we present a case of pelvic actinomycosis related to IUD with brief review of the concerned literature.
