1.A Case of Neurilemmoma of the Lateral Wall of the Hypopharynx Treated with Endoscopic Removal.

Neurilemmoma is a relatively rare benign tumor originating from the Schwann cells of all nerve fibers outside the central nervous system, with the sole exception being the optic and olfactory nerves. About 25-40% of neurilemmoma occurs in the head and neck region and the acoustic nerve is most frequently involved. But the tumor rarely affect the hypopharynx. In this paper, authors report a case of neurilemmoma of the lateral wall of the hypopharynx in a 61-year old female patient. The tumor was successfully removed via endoscopic approach. So, we report the case with literature.

2.Two Case of Acute Pulmonary Edema due to Local Epinephrine Injection during Tonsillectomy.

Epinephrine is used to control bleeding and clear operative field during tonsillectomy procedure by local and general anesthesia. But epinephrine also has many adverse effects, such as palpitation, headache, hypertension, arrhythmia, tachycardia, anxiety and some significantly more dangerous conditions including intracranial hemorrhage, pulmonary edema, cardiac arrest, etc. We have experienced two acute pulmonary edema owing to infiltration of epinephrine during tonsillectomy, where the condition of patients rapidly became more serious. We performed intensive care and ventilation therapy. In all cases, patients were recovered after intensive care and additional postoperative complication has not been detected until now. We report these cases with literature.

3.Gallbladder Perforation after Transarterial Chemoembolization in a Patient with a Huge Hepatocellular Carcinoma

Transarterial chemoembolization (TACE) is a common treatment for unresectable hepatocellular carcinoma (HCC). The most common complications after TACE are non-specific symptoms called post-embolization syndrome, such as abdominal pain or fever. Rare complications, such as liver failure, liver abscess, sepsis, pulmonary embolism, cholecystitis, can also occur. On the other hand, gallbladder perforation is quite rare. This paper reports a case of gallbladder perforation following TACE. A 76-year-old male with a single 9-cm-sized HCC underwent TACE. Five days after TACE, he developed persistent right upper quadrant pain and ileus. An abdomen CT scan confirmed gallbladder perforation with bile in the right paracolic gutter and pelvic cavity. Percutaneous transhepatic gallbladder drainage was performed with the intravenous administration of antibiotics. After 1 month, the patient underwent right hemihepatectomy and cholecystectomy. Physicians should consider the possibility of gallbladder perforation, which is a rare complication after TACE, when unexplained abdominal pain persists.
4.Herpes zoster meningoencephalitis with decreased CSF glucose level-A case report and review of literatures.

We report a case of decreased cerebrospinal fluid glucose with lymphocytic pleocytoisis in a patient with herpes zoster meningoencephalitis. The finding was likely to be confused with that of tuberculous meningoencephalitis. The concentration of CSF glucose is a critical point in the differential diagosis of various causes of nervous system infection. Although the herpes zoster meningoencephalitis isa well recognized, cases with markedly low, level of CSF glucose has been rare. We reviewed such unusual cases in the literature that were accompanied by hypoglycorrhachia. The duration of hypoglycorrhachia was transient. This suggests a differential point from that of tuberculous meningitis.

5.Intravascular Ultrasound Assessment of the Coronary Intervention.

BACKGROUND: Coronary arterigraphy has been used as a tool to assess the degree of coronary artery narrowing and the result of balloon angioplasty, which frequently underestimates the degree of atherosclerosis. Intravascular ultrasound(IVUS) can give more delicate information about plaque morphology and the result of coronary intervention. We compared qualitaive and quantitative measurement between IVUS and coronary angiography after coronary intervention. METHODS: We used 30 or 20 MHz intravascular ultrasound catheter which was connected to Hewlett Packard Sonos 1500 Intravasscular equiment in 5 coronary balloon angiopasty and 3 Palmaz-Schatz stent implantation cases. Sites of intervention were at the left anterior descending coronary artery in 7 patients and at the left circumflex artery in one patient. Quantitative coronary angiography(QCA) was done by CAAS II system. We measured referenc diameter(RD), minimal lumen diamter(MLD), Lumen and vessel cross sectional areas(LCSA,VCSA) obstraction area(OA) and plaque area(PA) and also analysed plaque morphology. RESULTS: 1) IVUS is more sensitive in the detection of eccentricity, Calcification and dissection. 2) Before intervention, the mean reference diameter was 2.87+/-0.42mm,3.07+/-0.39mm,% diameter stenosis was 52.4+/-11.6%,65.3+/-9.22% and MLD was 1.32+/-0.24mm, 1.07+/-0.23mm in IVUS and QCA, respectively, which were no statistical significance between these parameters(p>0.05). After intervention, MLD and OA increased significantly(p<0.01) com pared with basal values but there were no significant difference between MLD and OA between 2 measurements(p>0.05). Plaque area measured by IVUS decreased from 9.84 to 7.26mm2 without statistical significance(p>0.05). 3) There was a good correlation in the measurement of the reference segments before intervention but this correlation was much lower after intervention in the reference and stenosis segments between 2 methods(r=0.8723 vs 0.6538, p<0.01). CONCLUSION: IVUS is considered as a sensitive tool in the detection of calcification, eccentricity and dissection and in evaluationg the results of the coronary intervention.

6.Therapeutic Effects of Growth Factor Cocktail Treatment in Patients with Androgenetic Alopecia According to the Depth of Microneedle.

BACKGROUND: Growth factor treatment in combination with microneedling in androgenetic alopecia (AGA) patients is safe and effective. However, there is a lack of studies examining the effect of treatment according to the depth of the microneedle. OBJECTIVE: The aim was to evaluate differences in efficacy according to the depth of microneedle. METHODS: This study was performed on 6 male and 5 female AGA patients who were treated with topical growth factor cocktail (GFC) with microneedling every 2 weeks for 3 months. The scalp was divided into right and left sides and treated with microneedle depths of 0.5 and 0.3 mm, respectively. GFC was topically applied using a microneedle medical device. Treatment efficacy was evaluated by phototrichogram and digital photograph analysis on 6 repeated treatments for 3 months. RESULTS: Phototrichogram of the 0.5- and 0.3-mm-treated sides of the scalp showed 19 and 12 cm2 increase in hair density and 2.6- and 1.8-µm increase in hair diameter, respectively. These results were statistically significant (p<0.05). In terms of density, the 0.5-mm depth was significantly more effective than the 0.3-mm depth (p<0.05). With regard to diameter, the effect according to microneedle depth was not significantly different (p>0.05). CONCLUSION: GFC treatment with microneedling is effective and safe and a microneedle depth of 0.5 mm appears to be more effective than that of 0.3 mm. More research is necessary to confirm these results and determine the most effective depth for microneedling by investigating various depths.

7.A Case of Chondroid Syringoma of the External Auditory Canal.

Chondroid syringoma is a benign sweat gland tumor composed of a mixture of epithelial cells and mesenchymal tissue. It is most frequently located on the head and neck. Chondroid syringoma is usually present as a firm intradermal or subcutaneous nodule. Histologically, the epithelial component consists of tubulocystic structures and aggregates of epithelial cells as well as single scattered epithelial cells throughout the stroma. The treatment of choice is complete local excision. We report a case of chondroid syringoma presenting as a external auditory canal mass in 54 year old man with a brief review of literature.

8.A Case of Pleomorphic Adenoma in Cheek.

Pleomorphic adenoma are benign neoplasms that typically occur in the major salivary glands. Uncommonly, they arise in minor salivary glands, which account for only 10% of all reported salivary gland pleomorphic adenomas. Minor salivary gland tumors are clinico-histopathological curiosities and arise from glands scattered throughout the oral and pharyngeal mucosa. The cheeks, lips and gingiva are rare sites of occurrence. We experienced a case of a 68-year-old woman who presented a 45-years history of a left cheek mass. The pathologic diagnosis was pleomorphic adenoma. We removed the mass intraorally and obtained good postoperative results. We report this case with a review of literature.

9.The association between long working hours and marital status change: middle-aged and educated Korean in 2014–2015

BACKGROUND: We investigated the association between long workhours and marital status change from married to divorced or separated status that might have bad health effects. METHODS: A total of 40,654 participants with married status in 2014 were followed up in 2015. Weekly workhours were categorized into four groups: ≤ 40, 41–52, 53–60, and > 60 hours per week. Univariate and multivariate logistic regression analyses were performed to determine the relationship between groups of workhours and marital status change after adjusting for age, total monthly household income, working type, and depression with sex stratification. RESULTS: The study populations consisted of 8,346 (20.5%) females and 32,308 (79.5%) males. Odd ratios (ORs) of marital status change for females working for more than 60 hours per week was 4.26 (95% confidence interval [CI]: 1.25–14.5), when working less than or equal to 40 hours per week was used as reference in the crude model. ORs of working more than 60 hours per week was 4.57 (95% CI: 1.02–20.5) in female workers when considering age, total household earning per month, working type of daytime, and depression in a dose-response manner. However, for male workers, long workhours were not significantly related to change of marriage status. CONCLUSIONS: Long workhours for more than 60 hours per week had significantly higher risk of divorce or separation in females, but not in males. Further follow-up studies are needed to evaluate long term effects of long workhours on divorce risk.

10.Psychiatric Analysis about Transsexuals.

PURPOSE: Transsexualism is considered to be the extreme end of the spectrum of gender identity disorders characterized by, among other things, a pursuit of sex reassignment surgery(SRS). We evaluated psychologic status, health-related quality of life in female-to-male (FTM), male-to-female(MTF) transgender individuals. METHODS: We used the Minnesota Multiphasic Personality Inventory, Beck Anxiety Inventory, Beck Depression Inventory, Moudsley obsessive-compulsive Inventory, SCL-90-R, Short-Form 36-Question Health Survey version 2(SF-36v2). We enrolled 40 transsexual participants. RESULTS: Analysis of quality of life health concepts demonstrated statistically significant(p<0.01) diminished quality of life among the transsexual participants as compared to the Korea male and female population. FTM transgender participants reported more higher hostile, phobic than MTF transgenders. Overall, in all psychologic status examination, Transgender individuals are within normal population boundary. On all category, result is improved post-operatively. CONCLUSION: Transgender participants reported mental status within normal boundary. SRS improved their quality of life and mental stability.
