1.Three cases of nonadrenal (nonprogressive) female pseudohermaphroditism.

Female pseudohermaphrodities are 46XX genetic females with normal Mullerian derivatives, but have various degrees of ambiguous external genitalia. While most of them are commonly associated with adrenogenital syndrome, some of them have been occasionally associated with maternal ingestion of testosterone or synthetic progestational agent, maternal virilizing tumor or rarely idiopathic. Herein, we present three cases of female pseudohermaphroditism which is unrelated with adrenogenital syndrome. One case is resulted from maternal ingestion of progestational agent during the first trimester of pregnancy, and the other idiopathic.

2.A Case of Bowen's Disease Partially Responded to Photodynamic Therapy.

Photodynamic therapy(PDT) is a treatment modality by highly reactive oxygen intermediates generated through the interaction of light with a photosensiziter. It has been shown to be an effective treatment for various cutaneous and noncutaneous malignancies. It is efficient for the curative and palliative treatment of epithelial skin tumor in situ or early invasive lesions. In effect, it is a useful alternative treatment for the lesions located on anatomically difficult areas or the large-sized lesions. We treated a case of Bowen's disease arising on the plantar area and 3rd and 4th toewebs of left forefoot in a 61-year-old man with PDT using the hematoporphyrin derivative, porfirmer sodium(Photofrin, Russia) as a photosensitizer and gold vapor laser as a visible light source. The outcome showed partial clinical improvement after about 2 months' follow-up.

3.The Effects of Dimethly Sulfoxide and Sodium thiosulfate for the Prevention of Tissue Necrosis due to Extravasation of Mitomycin-C.

Extravasation of toxic chemotherapeutic 'agents cause severe skin ulceration and necrosis which often need secondary surgical intervention. Still, there were not established antidote agent in case of extravasation with mitomycin-c. Dimethyl sulfoxide is known as an effective chemical scavenger of toxic hydroxyl free radical and sodium thiosulfate also was demonstrated significant protector from mitomycin-c induced ulceration by a few experimental studies. Author investigated necrotic area of mitomycin-c injected site and compare to the effectiveness of topical treatment with dimethyl sulfoxide and intradermal injection of sodium thiosulfate according to starting times, forty five mice were divided into 3 groups. Control group(n=5) had no treatment after subcutaneous injection of mitomycin-c. Experimental group I and 11 were 20 mice treated dimethyl sulfoxide and sodium.

4.Postoperative long-term follow-up of benign prostatic hyperplasia on viewpoint of quality of life.

As a part of ongoing effort to evaluate alternative treatments for benign prostatic hyperplasia (BPH}. we compared the outcomes of transurethral resection of prostate(TURP) with those of open prostatectomy(OP). To identify effect of TURP on high mortality due to cardiac death. We compared long-term mortality and the causes of death after each surgical procedures. From Jan 1981 to Dec. 1990, surgical management was underwent on 338 patients with BPH in Pusan National University Hospital. Of 338 patients. survival was identified in 70 of OP group and 166 of TURP group. Direct interview was possible in 50 of OP group and 135 of TURP group. Mean age and follow-up were 69.8 years old and 89.5 months in OP group and 68.7 years old and 45.4 months in TURP group. Both OP and TURP were effective in improving subjective voiding symptoms including comprehensive symptom and Boyarsky symptom score, but, no significant difference was observed between them. Postoperative convalescence period was significantly shorter in TURP group compared to OP group. The incidence of sexual dysfunction including loss of libido and poor erection was similar between two surgical groups, but the incidence of retrograde ejaculation was higher in TURP group than OP group. Postoperative mortality and leading causes of death were no difference between two groups. Conclusively, we could not find the significant differences between OP group and TURP group as to symptom improvement, long-term mortality and morbidity, and effectiveness on patient`s consent. Although we confess that further evaluation is needed for precise and objective results, such results show postoperative improvement of voiding symptoms, quality of life and long-term outcomes that may not depend on surgical options.

6.Postoperative long follow-up assessment of benign prostatic hyperplasia with Boyarsky symptom score system.

Many parameters are available for study and evaluation for patients with benign prostatic hypertrophy (BPH). Of that, symptomatology has classically formed the urologist's initial data base on which to formulate evaluation of potential outlet obstruction, indication for surgery and evaluation of results of treatment. And, to identify surgical efficiency on reducing prostatism and clinical availability of symptom scoring system for quantitative, objective index of symptom status, we undertook long follow-up assessment of BPH with Boyarsky symptom score system on SO cases or open prostatectomy (OP group) and 135 cases of transurethral resection (TUR group) who treated surgically on Pusan National University from Jan. 1981 to Dec. 1990. Both surgical procedures are effective in reducing symptom status and higher preoperative symptom score was, the higher postoperative symptom store improvement resulted. Improvement of obstructive symptoms is significantly higher than that of irritative symptoms in both groups. The presence of acute urinary retention was significantly related with improvement of symptom score (p<0.05), but preoperative urinary tract infection, prostate weight and degree of residual urine were not (p>0.05). Although symptom score improvement did not completely reflex to comprehensive subjective symptom improvement, significant relationship was present. Therefore, it would be used as objective, quantitative index to access the voiding status of patients.

8.Ultrastructural Changes of Skeletal Muscle in Tourniquet Ischemic

Muscles from three patients were obtained during continuous ischemic periods and were investigated ultrastructural changes to know the reversible tourniquet time. Tourniquet was applied for periods lasting between 60 minutes and 90 minutes in the human and muscle biopsies were taken from 10 minutes to 90 minutes ischemic period and at the same periods after release of the tourniquet. All specimens were investigated with electron microscopy and the results were as follows: In 10 minutes tourniquet period, loss of cristae in few mitochondria were observed. In 30 minutes tourniquet period, partial swelling, loss of cristae and myelin figure in many mitochondrias and vacuolar dilatation in T-tubule and sarcoplasmic reticulumn were observed. In 60 minutes tourniquet period, above change were observed in nearly all mitochondrias and total number of mitochondrias were decreased. In 90 minutes tourniquet period, above changes were more distinct. 20 minutes after release of 90 minutes tourniquet period, small sized new mitochondria were appeared and abundant electron dense granule in their matrix and sarcoplasmic reticulumn were visible. 60 minutes after release of 90 minutes tourniquet period, the majority of damaged mitochondrias were replaced by small sized new mitochondrias and electron dense granule were abundant in sarcoplasmic reticulumn. In human, the changes on the ultrastructure were reversible for at least a succeeding 90 minutes ischemic periods.

9.A Case of Nevus Comedonicus.

We describe a 16-year-old female with Nevus comedonicus(NC) that has been developed since birth. The lesions had a unilateral, linear or zosteriform distribution on the left lower leg and posterior thigh, and were well closely arranged, slightly depressed papules that had dark firm plugs resembling comedones in their centers. In the meantime, recurrent infections with fistula, abscess and residual scars had been complicated. Histopathologic features showed deep invagination of epidermis and dilated follicular orifices filled with keratin plugs in the centers of papules.

10.Two Cases of Irritant Dermatitis due to Clematis apiifolia A.P.DC..

The clematis apiifolia A.P. DC. is a plant which belongs to Clematis Linne, Ranunculaceae. It contains anemonol, clematiscampher, clematin, labenzyme, saponin, of which anemonol is a probable irritant reacting component. We report two cases of irritant dermatitis due to Clematis apiifolia A.P.DC. plant. These patients developed painful, inflamed erythematous patches with flaccid bullae and erosions on the application sites of the crushed plant for self-treatment of arthralgia.
