1.Diverticular Diseases of the colon in Korea.

The purpose of this study is to review epidemiological and clinical characteristics of diverticular disease of the colan in Korea and to discuss the difference of the findings from those in other countries. Reviewing all thebarium enema films taken at the Seoul National University Hospital retrospectively for the last 8 years. that is, from January 1, 1980 to December 31, 1987, we observed the annual and overall incidence of the disease, the number and location of the diverticula, and the presenting symptoms of the cases. The results are summarized as follows: 1) Out of the patients examined, 237 cases had one or more diverticula in the colon, the overall incidence being 1.32%, 2) The annual incidence increased progressively for the last 8 years, from 0.25% in 1980 to 2.53% in 1987. 3) The mean age of the patients at the time of diagnosis was 51 years. The male to female ratio was 2.5. 4) The diverticula were located on the right side in 81.0% of the cases, on the left side in 10.6%, and on both sides in 8.4%. 5) In 82 cases (34.6%) single diverticulum was found, whereas 34 cases (14.3%) had more than 10 diverticula. 6) The most frequent symptoms for taking barium enema were abdominal discomfort and pain (34. 2%) and changes in bowel habits (33.8%}, whereas 45 casea (19.0%) had the examination just for a routine health check. In conclusiion, the diverticular disease of the colon is still uncommon in Korea as compared with in western countries, and the right colon type is far more freqtaent than the left colon type. However in recent years the incidence increases quite rapidly and the left colon type is getting more common.

2.Arthroscopic Surgery of the Mediopatellar Plica Syndrome

The role played by synovial plica in internal derangement of the knee has been increased recently. A clinical application of arthroscopy of the one hundred eighty patients were performed, and eighteen patients with a symptomatic plica were successfully managed and followed by arthroscopic surgery at the department of orthopaedic surgery, Soon Chun Hyang University Hospital from May 1983 to May 1984. The results were summerized as follows; 1. Arthroscopy is the most accurate diagnostic and operative methods for the investigation of the incidence, pathology, clinical significance and treatment of the synovial plica. 2. The incidence of the mediopatellar plica in our view was 41% that is, 74 of 180 symptomatic knee. 3. Arthroscopy is a practical and valuable procedure with minimal associated morbidity or complication. 4. The increased accuracy of diagnosis help to avoid unnecessary operations and to be basic methods of treatment on a better knowledge of the pathology.

Extravasation of toxic chemotherapeutic 'agents cause severe skin ulceration and necrosis which often need secondary surgical intervention. Still, there were not established antidote agent in case of extravasation with mitomycin-c. Dimethyl sulfoxide is known as an effective chemical scavenger of toxic hydroxyl free radical and sodium thiosulfate also was demonstrated significant protector from mitomycin-c induced ulceration by a few experimental studies. Author investigated necrotic area of mitomycin-c injected site and compare to the effectiveness of topical treatment with dimethyl sulfoxide and intradermal injection of sodium thiosulfate according to starting times, forty five mice were divided into 3 groups. Control group(n=5) had no treatment after subcutaneous injection of mitomycin-c. Experimental group I and 11 were 20 mice treated dimethyl sulfoxide and sodium.

7.Airway Management by Bougienation in Tracheal Stenosis: A case report.

The incidence of tracheal stenosis is increased because of the longterm respiratory care with endotracheal intubation and tracheostomy. Present therapeutic modalities for the relief of an tracheal or bronchial stenosis include laser resection, radiotherapy, cryotherapy, bougienation, stent insertion, dilatation with balloon catheter and finally reconstruction surgery. However, reconstruction surgery have some problems in ventilation during anesthetic management. Small sized tube insertion through lesion, high frequency jet ventilation, cardiopulmonary bypass are served to resolve ventilatory problem. We experienced a case of severe tracheal stenosis due to tracheostomy. The stenotic lesion was 2.5 cm above the carina, 3 mm in diameter and length of the stenotic segment was 1 cm. We used bougienation with endotracheal tube replace obturator for the ventilation before the reconstruction surgery and the patient was successfully managed without complications.

8.Open Reduction and Internal Fixation of Intraarticular Calcaneal Fractures by the Extended Lateral Approach.

The operative treatment of intraarticular calcaneal fractures has three principal aims; restoration of the height and width of the calcaneus, reconstruction of the subtalar and calcaneocuboid joint surfaces, and stable osteosynthesis. The purpose of this study is to demonstrate the effectiveness of the open reduction and internal fixation by the extended lateral approach for the treatment of the intraarticular calcaneal fractures. Twenty-one calcaneal fractures of 20 patients were treated by open reduction and internal fixation using an extended lateral approach from April, 1993 to March, 1994. The autogenous iliac bone graft was done in all patients. These patients were evaluated from 12 to 24 months (average 15.2 months) after surgery. The following results were obtained; 1. Seventecn cases (80%) out of 21 were estimated as good or excellent result. 2. Preoperative Bohler's angles, Gissane's angles and inclination angles were observed as average -0.5degrees, 101degrees and 38degrees respectively, and these were improved as average 23.3degrees, 117degrees and 54degrees, postoperatively. 3. There were two cases of skin necrosis as complication. In conclusion, the extended lateral approach is one of the good methods for the treatment of intraarticular calcaneal fractures. We think that accurate reduction of the posterior facet, acceptable recovery of Bohler's angle and early postoperative mobilization are the most important things for the best result of the intraarticular calcaneal fracture surgery.

9.Sacral Perineural cyst: report of 3 cases.

Sacral perineural cyst is one group of extradural meningeal cyst at the sacral region. These lesions are distinguished from other spinal cysts because perineural cyst does not communicate with subarachnoidal space. Fluid filled cysts may compress adjacent nerve roots causing low back pain and sciatica which are dramatically improved by surgical excision. Differential diagnosis from other cysts can be accomplished by failure of collection of dye into cyst by initial myelography. C-T myelography rules out other mass lesions and often reveals communication of perineural cyst that filled with contrast medium. Magnetic resonance imaging well demonstrates three dimensinnal configures of an intraspinal cystic mass in initial study. However it is important to recognize that these cysts are one of causes of radiculopathy. We report three cases of sacral perineural cysts with radiculopathy.

10.Arthroscopic Study of the Separated Suprapatellar Bursa

A clinical application of the arthroscopy to the three hundred symptomatic knees were performed, and seven knees with a symptomatic seprated suprapatellar bursa were successfully managed and followed by arthroscopic surgery at the Department of Orthopaedic Surgery, Soon Chun Hyang University Hospital from May 1983 to May 1985. The results obtained are as follows; 1. Separated suprapatellar bursa may cause some internal derangement of the knee. 2. The incidence of the separated suprapatella bursa in our view was 5% that is, of 300 symptomatic knees. 3. Separated suprapatellar bursa may have to be excised under arthroscopy.
