The athors have presented recently treated 4 cases of malignant tumors of the eye and adnexa. 1. All of them were rapidly aggrevated after previous incompleted surgical management (3 cases) or postponement 01 operation (1 case). 2. Three cases of them except adenoid cystic Ca. were recurred after orbital exenteration. 3. The authors were concluded that the poor prognosis is due to its nature of malignancy and delayed radical surgical management.

2.Sarcoma-like Mural Nodule in Ovarian Mucinous Cystadenocarcinoma: A case report.

We describe an ovarian mucinous cystadenocarcinoma with a sarcoma-like mural nodule. In the literature, rare cases of ovarian mucinous tumors have been described which contain foci of undifferentiated carcinoma, sarcoma, and sarcoma-like nodules. The distinction between these lesions is important because of poorer prognosis of true sarcoma and anaplastic carcinoma than sarcoma-like mural nodules. This case shows different results of immunohistochemical stain for anaplastic carcinoma.
3.A Case of Coloboma of the optic nerve disk.

The authors observed clinically typical case of unilateral coloboma of the optic disc. This patient, 5 years old Korean boy, visited to our clinic with the chief complaints of visual disturbance and intermittant exotropia of left defecting eye. The optic disc was enlarged about 2.5 times of normal optic disc diameter excavated about 7.0 Diopters in depth. The vision of colombomatous left eye was 20/50(n.c.) but the cause of defective vision was not evaluated certainely whether it might be due to coloboma itself or slight posterior subcapsular opacity of the lens.

4.Cosmetic Lateral Canthoplasty: Lateral Canthoplasty to Lengthen the Lateral Canthal Angle and Correct the Outer Tail of the Eye.

There are many women who want larger and brighter eyes that will give a favorable impression. Surgical methods that make the eye larger and brighter include double eyelidplasty, epicanthoplasty, as well as lateral canthoplasty. Double eyelidplasty produces changes in the vertical dimension of the eyes, whereas epicanthoplasty and lateral canthoplasty create changes in the horizontal dimension of the eyes. Epicanthoplasty, a surgical procedure which enlarges the eye horizontally, is performed at the inner corner of the eye, whereas lateral canthoplasty enlarges the outer edge of the eye. In particular, if the slant of the palpebral fissure is raised and the horizontal dimension of the palpebral fissure is short, adjusting the slant of the palpebral fissure through lateral canthoplasty can achieve an enlargement of eye width and smoother features. Depending on the patient's condition, even better results can be achieved if this procedure is performed in conjunction with other procedures, such as double eyelidplasty, epicanthoplasty, eye roll formation surgery, fat graft, and facial bone contouring surgery. In this paper, the authors will introduce in detail their surgical method for a cosmetic lateral canthoplasty that lengthens the lateral canthal angle and corrects the outer tail of the eyes, in order to ease the unfavorable impression.

5.Histologic Grading by Modified MDAH System and Prognosis of Prostatic Carcinoma.

Since the behavior of malignant cells can be predicted by their histologic appearance, various attempts have been made to develop a uniform grading system for prostatic cancer. The system employed at the University of Texas M.D. Anderson Hospital(MDAH system) is based simply on assessment of percentage of gland formation in the tumor. Herein, using the modified MDAH system, histologic grading and prognosis were retrospectively reviewed in 35 patients with prostatic adenocarcinoma managed at Chonnam University Hospital between l997 and 1984. The following results were obtained. 1. The average age of all the patients was 70.1 years, with a range of 25 to 85 years. 2. In 35 patients, the modified MDAH system identified 6 patients (l7%) as Grade 1, 15(43%) as Grade 2-3, and 14 (40%) as Grade 4. There were a 83% 3-year survival for Grade 1 patients, a 69% 3-year survival for Grade 2-3 patients, and a 17% 3-year survival for Grade 4 patients. 3. Of 15 patients with clinical stage D, 2 had Grade 1, 6 had Grade 2-3, and 7 had Grade 4.There were a 100 % 3-year survival for Grade 1 patients, a 50% 3-year survival for Grade 2-3patients, and a 14% 3-year survival for Grade 4 patients.

6.A study on diagnostic value of oblique lumbar spine radiographs

A retrospective study was designed to determine the diagnostic contribution of oblique view in 740 consecutive lumbar spine examinations done in adults. In 12 cases (1.62%) there was a change in the radiographic interpretation when the oblique views were used for diagnosis. Considering the significant gonadal radiation, additional cost, and limited diagnostic value, the authors feel that the oblique view should not be a routine part of the initial lumbar spine examination in adults.

7.Percutaneous transhepatic cholangiography in obstructive jaundice

Although many invasive studies in the biliary tract disase are replaced by noninvasive techniques such asultrasonography and CT, PTC does still play an important role indiagnosis of obstructive jaundice. Authors hadperformed 42 patients of PTC with Chiba needle at the department of radiology, Han Gang Sacred Heart Hospital for4 years from Jan. 1978 to April 1982. And we reviewed literatures. The results were as follows; 1. Forty twopatients of PTC were 22 males and 20 females. The success rate was 90.5%(38/42). The failed 4 cases of PTC werenonobstructive hepatobiliary diseases. 2. The PTC diagnosis of 38 cases were biliary stone (21 cases),cholangiocarcinoma(7 cases), pancreatic carcinoma (2 cases), hepatoma (2 cases), ascariasis(2 cases), andclonorchiasis(1 case), and the remainders were normal. 3. We made 2 cases of misdiagnosis on the basis of PTCfindings. They were one case of distal CBD stone with severe cholangitis diagnosed as ampullary carcioma andanother case of proximal CBD stone with cholangitis diagnosed as pancreas carcinoma. 4. We experienced 2 cases ofcomplication such as sepsis and severe hypotension, but they were recovered.

8.Complete Rupture of the Proximal Hamstring.

While a sprain of the hamstring muscle is relatively common in athletes or those who participate in physically active sports, a complete rupture of the proximal hamstring is relatively rare. A rupture of the long head of the biceps femoris without rupture of the semimembranosus and semitendinosus muscles has rarely been reported. In this study, we saw relatively favorable outcomes after reattachment with a suture anchor at the ischial tuberosity in two patients who had a rupture of the long head of the biceps femoris.

9.Solitary Duodenal Metastasis from Breast Cancer.

A 65-year-old female patient experienced melena for 10 days. Gastroduodenoscopy revealed a tumor in the duodenum, a portion of which was taken for biopsy, which showed a malignant tumor. She underwent pancreatoduodenectomy, and the final tumor pathology revealed invasive ductal carcinoma from the breast, which was confirmed using immunostaining of milk fat globule antigens. Nineteen years before, she had received a radical mastectomy due to invasive ductal carcinoma of the right breast. Hematogenous metastasis occurs in 33% of patients with breast cancer, mainly to the liver and lung, in invasive ductal carcinoma (IDC), and to the gastrointestinal tract, peritoneum, and retroperitoneum in invasive lobular carcinoma (ILC). Solitary metastatic duodenal tumors from breast cancer, especially IDC, is rare, particularly after a long time. This rare case is presented with a literature review.

10.Foveal Shape According to Age and Gender Using Spectral Domain Optical Coherence Tomography.

PURPOSE: To compare foveal shapes in Koreans according to age and gender using spectral domain optical coherence tomography (SD-OCT). METHODS: This study included 230 eyes of 115 healthy adults. The subjects were divided into three groups: group 1 (20-39 years of age), group 2 (40-59 years of age) and group 3 (60-79 years of age). Using spectralis OCT, we measured central foveal thickness (CFT), regional maximal retinal thickness (MRT), pit diameter and pit depth and compared the differences between the groups. RESULTS: The MRT of the superior, inferior and nasal sides in group 1 was higher than in groups 2 and 3 (p < 0.05). No significant difference was observed in the MRT of the temporal side. Regarding differences based on age, no significant differences in CFT, pit diameter and pit depth were observed. Regarding differences in gender, the temporal regional MRT of males in group 3 was significantly lower than in group 1 and the pit depth of males in group 1 was significantly higher than in groups 2 and 3. Therefore, differences were observed according to gender. CONCLUSIONS: In the present study, differences in foveal shape were found according to age and gender which should be considered when foveal diseases are evaluated.
