The common bile duct and the duct of Wirsung cojoin at the level of the duodenum, forming the major. papilla of Vater. Existence of a double major papilla, i.e., two neighboring independent papillary structure:, is infrequent. In our endoscopy unit we have experienced one case of double papilla of Vater wherein canulation of the common bile duct and pancreatic duct could be accom plished through either orifice independently.

The athors have presented recently treated 4 cases of malignant tumors of the eye and adnexa. 1. All of them were rapidly aggrevated after previous incompleted surgical management (3 cases) or postponement 01 operation (1 case). 2. Three cases of them except adenoid cystic Ca. were recurred after orbital exenteration. 3. The authors were concluded that the poor prognosis is due to its nature of malignancy and delayed radical surgical management.

5.A Case of Cutaneous Hemorrhagic Bullous Eruptions in Lymphoma of the Small Intestine.

Authors experienced a 59-year-old male patient with the lymphoma of the small iritestine which involved the adjoining mesenteric lymph nodes, the parietaI pleura, and the skin. The skin lesions, simulating necrotizing angiitis, were deep purple colored, asymptomatic, tense, hemorrhagic bullae with soft consistency on the both thighs and left infraorbital area. He had extranodal type, and diffuse, poorly differentiated lymphocytic lymphoma by the Rappaport classification, and stage IV by the Ann Arbor staging classification. After about 50 days of clinical manifestations, he was died mainly due to massive intestinal bleeding.

6.Intraocular Pressure After Cataract Extraction in Silicone tube Implanted Glaucomatous eye.

The functioning filtering bleb constructed after trabeculectomy in glaucomatous patient might be fail after cataract operation probably due to transient collapse during cataract extraction and inflammatiry reaction of the filtering bleb which result in unsuccessful intraocular pressure control. To compare if the results were identical in cases with the use of encircling band to enlarge the aqueous absorbing scar tissue to trabeculectomy, intraocular pressure was followed for more than 6 months after cataract extraction and posterior chamber lens implantation in 7 eyes that previously underwent silicone tube insertion with the use of encircling band. There were no cases with increased intraocular pressure needed to control postoperatively. Therefore it could be possible to concluded that inflammatory reaction of the filtering bleb or transient collapse during cataract extraction has no negative influences on intraocular pressure in silicone tube implanted eye using encircling band.

8.Distally-Based Sural Artery Flap.

Reconstruction of soft tissue defect with exposure of the tendons and bone in the lower third of the leg and the heel represents a challenge to plastic surgeons. The sural artery flap is a fasciocutaneous flap supplied by the sural artery that accompanies the sural nerve and connects with a septocutaneous perforator of the peroneal artery via a suprafascial network of vessels. For the coverage of soft tissue defects, we operated on 10 patients using a distally-based sural artery flap. The sites of the soft tissue defect were the lower third of the leg in 7 cases and the heel in 3 cases. The size of flap varied from 3.5x4cm to 12x18cm. Nine of 10 flaps survived completely. One flap in which the sural nerve was preserved showed partial necrosis but healed spontaneously. Two flaps showed slightly venous congestion which disappeared after a few days. The advantages of the sural flap are a reliable blood supply, easy and quick elevation of the flap, preservation of the major artery and minimal donor site morbidity. The disadvantage of the flap is hypoesthesia at the lateral part of the foot. In conclusion, the distally-based sural artery flap can be used safely for soft tissues coverage in the lower third of the leg and the heel.

9.Immunocytochemical Study on the Monoaminergic Innervation of Diencephalic Visual Structures in the Squirrel Monkey.

This study was for investigating relations between distributions of monoamines-norepinephrine, serotonin, and dopamine-on the visual system and their functions. Distributions of these monoamines in the lateral geniculate body, pulvinar, lateral posterior nucleus, and suprachiasmatic nucleus were investigated. Brain of a squirrel monkey was removed and frozen sectioned. Immunocytochemical study was performed for the tissue of the brain. Results showed that the anterior part of the lateral geniculate body contained more monoamines than the posterior part. More serotonins were distrbuted at the magnocellular part, and more dopamines were found at the parvocellular part. In pulvinar, more norepinephrines were distributed at the medial part, while serotonins were evenly distributed at all parts. In lateral posterior nucleus and suprachiasmatic nucleus, three kinds of monoamines were distributed with high density. Among the three, density of the serotonin showed the highest value. The lateral geniculate body relates with visual perception such as visual acuity, form and color perception, and stereopsis, while the pulvinar relates with visual functions, such as visual attention, sensory integration, and differentiation. Since norepinephrine and serotonine are distributed with high density in the pulvinar than in the lateral geniculate body those two monoamines are expected to playa major role for visual functions. Inferior part of the pulvinar relates with visual imagination, and the lateral posterior nucleus relates with integration of visual sensory. Relatively high distribution of dopamine in these two parts means that dopamine may playa major role for visual imagination and integration. As suprachiasmatic nucleus relates with controlling biorhythm, dense distribution of monoamines in suprachiasmatic nucleus implies that the monoamines may work for controlling biorhythm.

10.An Experimental Study on the Survival of Membranous Inlay Bone Graft on the Mandible.

Bone graft is an important procedure in craniomaxillofacial reconstruction and the success of reconstruction depends on the survival of the grafted bone. In this study, the survival of the membranous inlay bone graft on the mandibles of dogs was investigated with bone scan and histologic examination. The inlay bone graft, 1x2cm critical-sized bone, was completely separated from the lower border of the mandible of dogs and then refixed to the original site. Bone scan and histologic examination were done at 1,2,3 and 4 weeks postoperatively. The bone scan after 1 week showed radioisotope uptake on the margin of the grafted bone and the isotope count was 21% compared to the uptake of the normal bone. After 2 weeks, the radioisotope uptake in the grafted bone increased to 52% of normal bone uptake. After 3 and 4 weeks, the degree of isotope uptake was 111% and 124% respectively. Histological findings after 1 week showed the absence of osteoblastic activity and 6 viable blood vessels in one 200X magnified field, which was 25% compared to the vessels of the normal bone. After 2 weeks, osteoblastic activities were noted and the number of viable blood vessels totalled 15, which was 63% of the vessels of the normal bone. After 3 weeks, osteoblastic activities increased and the number of viable blood vessels totalled 21, which was 88% of the vessels of the normal bone. After 4 weeks, there were markedly increased osteoblastic activities with a total number of 23 vessels, which was 96% of the normal bone. In summary, the revascularization of the membranous inlay bone graft began from the first week after bone graft, and then it gradually increased. After 3 weeks, the revascularization had returned to a nearly normal value compared with the value of the near-by normal mandibular bone.
