Flavimonas oryzihabitans is a gram-negative, glucose non-fermentative bacillus, and is rarely reported as a pathogen in human. In recent years, reports of infection due to F. oryzihabitans, especially in immunocompromised patients with indwelling catheter or peritoneal Tenckhoff catheter have increased. We report two cases of F. oryzihabitans peritonitis in patients undergoing continuous ambulatory peritoneal dialysis (CAPD).

3.A Clinical Study of Androgenetic Alopecia (III).

BACKGROUND: Androgenetic alopecia is considered to be a genetically determined disorder influenced by age and androgen. The proportion of patients with androgenetic alopecia among the total number of patients with alopecia seems to be gradually increasing. OBJECTIVES: The purpose of this study is to evaluate the family history,clinical and endocrine status of the patients with androgenetic alopecia. METHODS: 1113 patients with androgenetic alopecia who had visited the Department of Dermatology, Yongsan Hospital, College of Medicine,Chung-Ang University during the 3 years (1995.1-1998. 12) have been examined. RESULTS: The results are summarized as follows 1) The incidence of androgenetic alopecia among the total number of alopecia patients was 64.5%, showing recent increment. 2) There were 855 male and 258 female patients being most prevalent in the third decade in both sexes and the patients younger than 30 years old with premature androgenetic alopecia,made up 70.3% of the male patients and 48.8% of the female patients with androgenetic alopecia. 3) While Norwood's type Iia was the most common and following type II, III vertex,and IV in the male AGA, Ludwig's type II was the most common in female AGA 4) There was a family history of baldness in 53.5% of first degree relatives in male patients and 51.6%in female patients. 5)Associated diseases were observed in 565(66.8%) of the male patients and 219 (84.8%) of the female patients:diseases associated with androgen such as seborrheic dermatitis and acne vulgaris occupied 39.1%. CONCLUSION: Based on our findings, those who want to treat androgenetic alopecia at the earlier ages are gradually increasing and it seems to be reasonable to believe that the age, genetic factors, localized effects of androgens on the scalp and the density and/or functional activity of androgen receptors may influence the pathogenesis of androgenetic alopecia.

4.Expression of bcl-2 Protein in Gastric Adenoma and Adenocarcinoma.

The bcl-2 oncoprotein confers a survival advantage to cells by blocking programmed cell death. Overexpression of bcl-2 probably plays a role in tumorigenesis, and the expression of the bcl-2 protein has been investigated in many kinds of tumors. However, there have been only a few reports on expression of bcl-2 in human gastric adenocarcinoma. The aim of this study was to investigate the relationship between the expression of bcl-2 protein and several clinical and pathological parameters such as age, tumor site, size, histological type, depth of invasion, Lauren's classification, and grade. Immunohistochemical staining using monoclonal bcl-2 protein antibody, clone 124, was performed on paraffin embedded specimens from 23 gastric adenomas and from 45 gastric adenocarcinomas. The results are as follows. 1. Variable intensity of epithelial staining was noted from case to case, although the lymphocytic component showed similar intensity in all examples. The staining was located at the gland and mucous neck region of non-neoplastic epithelium. 2. The more differentiated type of gastric adenocarcinoma showed the higher expression rate and intensity. 3. The relationship between the expression rates of bcl-2 protein and tumor grade (adenoma early gastric adenocarcinoma advanced gastric adenocarcinoma) was statistically significant. The reactivity in adenoma was somewhat stronger with a uniform pattern, while in adenocarcinoma it was much weaker with a heterogenous pattern. 4. Intestinal type carcinomas by Lauren's criteria showed a higher expression rate and intensity than diffuse type. These results suggest that the bcl-2 expression would be found in the early phase of gastric tumorigenesis, and the expression rate and intensity would decrease according to the tumor progression.

6.TCVO ) in the Extremities: Report of 4 Cases

Angiographic procedures and their applications maybe divided into following roups: I) procedures used to increased blood folw, as the angioplasty, II) procedures used to decreased blood flow, as the embolization and local infusion of vasoconstrictors and III) miscellaneous procedures. Transcatheter vessel occlusion was first performed in 1930 by Brook to occlude the carotid cavernous fistula by embolization of the surgically exposed carotid artery with muscle marked with a sliver clip injected directly into the internal carotid artery. Autologous clot, first used by Doppman(1968) to occlude a spinal arteriovenous malformation was quickly adopted use in traumatic hemorrhage with pelvic fracture, nonoperable aneurysm, gastrointerstinal hemorrhage and diagnosis and treatment of the tumors. In Kores, there have been several reports of transcatheter vessel occlusion after Han(1978) made a first report in the radiology part. In orthopedics part of Korea, Kim(1981) and Yoo(1982) reported the cases of surgical treatments of arteriovenous malformation without embolization. Also, in Korea, there has been no report of transcather embolizations of vascular diseases of the extremities, such as huge arterial aneurysm and dilated varicose vein. We reported the 4 cases who were treated at the Chonbuk National University Hospital between January 1987 and April 1988. Transcatheter vessel occlusion have such advantages : 1. The vascular status of the mass can be recognized prior to operation, such as biopsy. 2. Since the bleeding can be reduce, the operation can be performed easily. 3. Nonoperable huge arterial aneurysm could sometimes be treated only with transcatheter vessel occlusion adequately. 4. It can make a differential diagnosis with other vascular anomaly easy.

8.A Case of Endovascular Stent-Graft Implantation in Aortic Dissection mimicking Intramural Hematoma in Descending Thoracic Aorta.

Intramural hematoma(IMH) and penetrating aortic ulcer have been increasingly recognized as causes of acute aortic pathology in addition to aortic dissection. The presence of the intimal tear and a flap traversing the aortic lumen is considered to be a most reliable differential point of aortic dissection and IMH. Transesophageal echocardiography(TEE) has become a valuable modality for the diagnosis, prognosis and management of acute aortic syndrome with the unique advantages of portability and the ability to obtain high-resolution real time images. Endovascular Stent-graft placement over the primary entry tear may be an alternative to open surgery because it can close the intimal tear, which leads to thrombosis of the false lumen, excluding flow through the intimal tear and redirecting aortic flow exclusively into the true lumen. We report 88 year-old male with aortic dissection in descending thoracic aorta, successfully treated with endovascular Stent-graft implantation, which was mimicking intramural hematoma by its appearance and subclinical intimal tear diagnosed exclusively by TEE against other imaging studies.

9.A Case of Complete Resolution of Aortic Dissection in the Descending Thoracic Aorta Treated with Endovascular Stent-Graft Implantation.

The aortic dissection is an acute aortic syndrome, caused by an intimal tear and subsequent splitting of the media by the pulsatile blood flow. Though there would be differences in the origin of aortic dissection and therapeutic modalities, the intermediate and long-term prognoses are poor. Endovascular Stent-graft implantation is a revolutionary technique in the treatment of aortic dissection. The endovascular stent grafting in aortic dissection is less invasive and feasible method and is an effective tool for closing the entry site and promoting clot formation, reducing the size of the false lumen. Therefore, endovascular Stent-graft implantation makes possible the desirable remodelling of aorta. We report 33 year-old male with aortic dissection in the thoracic aorta, which was treated with endovascular Stent-graft implantation. Over the favorable remodelling, his dissection was healed completely by the endovascular treatment using Stent-graft.

10.Paraganglioma of Cauda Equina.

This case report describes a paraganglioma of the cauda equina in a 37-year-old man, as documented by light microscopy and immunohistochemistry. The patient experienced low back pain of 3 years duration, with the recent onset of sciatic pain and altered sensation in the right leg. Magnetic resonance imaging of L4 vertebral level revealed an ovoid, solid mass in the cauda equina. The mass was measured 1.5 cm in the greatest diameter. The histologic appearance was characterized by organoid pattern with clusters of chief cells (zellballen). Immunohistochemically, tumor cells are positive for keratin, epithelial membrane antigen, vimentin, neuron specific enolase and chromogranin.
