Two cases of cutaneous sporotrichosis, especially one of which was verrucous type in baby aged 4 months, were described. Throush the literatures the verrucous manifestation in baby was not yet reported. It is known that verrucous sporotrichosis is to be developed in individual who has had local and general resistance obtained by the previous cutaneous infection or by the infection of internal organ. But no such evidences were noticed in our case.

2.A Case perifolliculitis Capitis Abscedens et Suffodiens.

A case of 24 years old male with perifolliculitis capitis abscedens et suffodiens was reported This is, I presume the first case to be reparted in Korea. Clinically, the skin lesions were characterized by tbe multiple confluented indolent abscesses and hypertrophic scars which were developed on the vertex. On these lesions, there were also noticed complete alopecia and several dilated hair follicle orifices plugged by keratinous materials. The regional lymphnodes were not enlarged and the abscesses were proved bacteriologically sterile.

5.Antibacterial Activity of Ceftizoxime Against Gram Negative Enteric Bacteria in vitro and in vivo.

Ceftizoxime sodium is a new synthetic β-lactam antibiotic combining potent antibacterial activity with high stability to a wide range of bacterial β-lactamase. This experiment was achieved to evaluate the antibacterial activities of ceftizoxime sodium against. Gram negative enteric bacteria isolated from in outpatient visiting Yeungnam university hospital and to study the emergence of drug induced bacterial variants which resist to ceftizoxime in vitro. The antibacterial activity of the ceftizoxime was compared with that of antibiotics and its effect on population of normal intestinal flora in mice was observed. The results are summarized as follows: 1. Highly effective antibacterial activity of ceftizoxime against Gram negative enteric bacilli was demonstrated and this antibacterial activity was superior to that of ampicillin. 2. Several test strains shows multiple antibiotic resistance. Among 15 strains of Escherichia coli, 1 strain was resistant to ampicillin, cefadroxil, gentamicin, tetracycline, and 2 strains were resistant to ampicillin, cefadroxil, tetracycline, five strains of Escherichia coli and Enterobacter cloacae was resistant to ampicillin, tetracycline and Shigella dysenteriae was resistant to ampicillin, gentamicin, tetracycline. 3. The frequency of in vitro emergence of resistant variants among ceftizoxime sensitive bacteria in the presence of increasing concentrations of the compound was found to be low. 4. Plasmid was isolated in 6 of 9 strains (6 strains of Escherichia coli, Shigella dysenteriae, Enterobacter cloacae and Salmonella typhi). That showed different antibiotic resistance. They were 5 strains of Escherichia coli and 1 strain of Shigella dysenteriae. However, plasmid could not be considered as a hallmark for antibiotic resistance by this Further studies with curing experiment are to be accomplished for this purpose. 5. Changes in the bacterial count of normal intestinal flora following 25 mg/kg/day administration of ceftizoxime over 5 consecutive days were not significant. In conclusion, ceftizoxime appeared to be a drug of choice in the treatment of Gram negative enteric bacilli infection.

6.Interhospital Comparison of Outcome from Intensive Care Unit with APACHE III Scoring System.

The objective of this study was to evaluate outcome for the patients of the intensive care unit, using APACHE III prognostic system. We prospectively collected the information of 429 patients in intensive care units at 2 tertiary care hospitals and 4 secondary care hospitals in PUSAN who had been admitted from December 1, 1993 to February 28, 1994. The results were as follows. 1. APACHE 111 scores were various from 0 to 173. But the distribution of the scores were similar between tertiary care hospitals and secondary care hospitals. 2. The mortality rate significantly increased as APACHE 111 score rised (P<0.001). Within the interval of same score, generally, the mortality of operative patients was higher in secondary care hospitals but in the case of nonoperative patients higher in tertiary care hospitals. 3. when the tertiary care hospitals compared with secondary for ratio of the predicted mortality rate to the actual mortality rate, there was little difference. 4. when we compared the 6 hospitals, one hospital had significantly better results and another hospital was significantly inferior (P<0.05).

Catecholamine-induced cardiomyopathy associated with neuroblastoma is rarely reported. We report a case of catecholamine-induced cardiomyopathy associated with neuroblastoma in a 33-month-old female that was treated with extracorporeal membrane oxygenation (ECMO). She was tentatively diagnosed with acute myocarditis and presented with hypertension. Because of rapid patient deterioration despite pharmacological treatments, ECMO was applied. ECMO can be helpful in cases of catecholamine-induced cardiomyopathy associated with neuroblastoma.

9.Clinical Investigation of Neck Masses in Children.

PURPOSE: Neck masses are frequently encountered in pediatric practice, and have various underlying diseases. If the underlying diseases of the neck mass are not confirmed by its clinical characteristics, it can be done by neck mass biopsy. To define the necessity and the appropriate time of neck mass biopsy is very difficult and also important. Therefore the authers have conducted this study to have aids in differential diagnosis of neck mass in children by analyzing the clinical manifestations of underlying diseases of neck mass. METHODS: The authors have reviewed 390 children under 15 years of age who visited and admitted with neck masses to Shinchon Severance Hospital between Jan. 1987 and Dec. 1994. In order to elucidate the nature and etiologic diseases of neck masses, we classify and analyze the neck masses by age and sex distribution, duration of symptoms and signs, size and consistencies and by site and number of neck masses, and finally by underlying diseases. RESULTS: 1)Of the overall sex distribution, male patients were more than female, the male to female ratio was 1.32 : 1(222:168). The most common age group was 2-5 years of age. The most common acompanying symptoms were upper respiratory tract infection symptoms, comprising 37.7 % of all, and the most common duration of symptoms and signs was within 3 months, comprising 65.6 % of all. 2)Of the underlying diseases of the neck masses, the largest proportions were inflammatory masses, which comprising 49.2 % of all cases, among these, nonspecific cervical lymphadenitis was most common(65.6 %). 3)We could also confirm the nonspecific cervical lymphadenitis by clinical manifestations, blood exams and neck ultrasonography except neck mass biopsy. 4)For the diagnosis of tuberculous lymphadenitis, the family history of tuberculosis, PPD skin test, and chest X- ray findings are almost helpful, but the neck mass biopsy was essential. 5)Malignant tumors were 24 cases, which comprise 6.2 % of all. CONCLUSIONS: For the appropriate differential diagnosis of neck masses in children, we should observe and describe accurately the clinical characteristics of the neck masses. On physical examination, if we can not rule out the possibility of tuberculous lymphadenitis or malignant tumors, the neck mass biopsy should be done immediately, but if it is likely the nonspecific cervical lymphadenitis, we must wait until the size of neck masses decrease or other symptoms disappear after empirical antibiotics therapy.
