3.Basophil histamine releasability in children with atopic asthma.

OBJECTIVE: To evaluate the basophil histamine releasability in response to IgE- and non- IgE-mediated stimuli in children with atopic asthma. Met: Basophil histamine releasability was measured in Dermatophagoides farinae (D. farinae)-sensitive atopic asthmatics, D.farinae-sensitive healthy atopics, non-atopic asthmatics, and healthy non-atopics. Basophils were stimulated with D.farinae, goat antihuman IgE antibody, formyl-Met-Leu-Phe(fMLP), and Calcium ionophore A23187. Histamine was measured by automated fluorometric technique. RESULTS: Sponianeous histamine release was higher in atopic asthmatics compared to healthy non-atopics. Histamine release by D.farinae and by anti-IgE antibody was higher in atopic asthmatics compared to the other groups. There was no difference in histamine release by fMLP among all groups. Histamine release by Calcium ionophore was higher in healthy atopics and non-atopic asthmatics compared to healthy non-atopics. The atopics showed correlation between histamine release by D.farinae, by anti-IgE antibody and total serum IgE levels. CONCLUSIONS: Spontaneous and IgE-mediated histamine release were related to the presence of both atopy and asthma, whereas non-IgE mediated histamine release was different depending on the stimuli.

4.A Case of Kimura's Disease Presenting as a Rhinophyma-like Configuration.

Kimura's disease is a benign, uncommon, chronic inflammatory condition that usually presents with painless subcutaneous nodules or plaques. Head and neck are the most frequently involved sites in Kimura's disease. Mandible is the most commonly involved, followed by neck, cheek, scalp and forehead. Other possible sites are oral cavity, inguinal area and extremities, but there have been no reports involving the nose, especially the one that looks like a rhinophyma. We describe a case of Kimura's disease presenting like a rhinophyma.

6.Emphysematous Cystitis: 3 Cases Report.

Emphysematous cystitis is a rare condition characterized by gas collection in the wall and lumen of the bladder. We experienced three cases of emphysematous cystiris. All patients were female; one was associated with a long term history of diabetes mellitus and another with urinary indwelling catheter. All of the cases were easily diagnosed on plain radiograph and CT scan, and were successfully treated with antibiotic therapy. In one of the cases, however, associated abscess due to perivesical extension of inflammation was treated by combined external drainage.

7.The Treatment of Acromioclavicular Separation

There are many procedures for treatment of injuries of acromioclavicular separation but there are still con- roversies concerning the best management of these injuries. With this in mind, we treated surgically 22 cases of acromioclavicular separation of which 3 cases were old type 2 and 19 cases were type 3 in the department of orthopaedic surgery, Eul Ji General Hospital from March 1977 to darch 1982. The results were as follows: l. Of 22 cases, 21 cases (95.5%) were male with peak incidence in the 3rd and 5th decades (90.9%). 2. The most common causes of the injuries were traffic accidents (63.6%) and followed by falling from height (22.7%). 3. The diagnosis in type 2 and type 3 injuries is based on the clavicular displacement and instability at the acromioclavicular joint. 4. Operative treatment consists of Weaver and Dunn method (54.5%), modified Phemister method (18.2%), Neviaser method (13.0%), Stewart method (9.1%) and modified Henry method (4.5%). 5. The operative procedure in old type 2 and typc 3 injuries is good treatment of acromioclavicular separation.

8.Differences of Glucose and Lipid Metabolism among the Type of Antipsychotic Drugs in Schizophrenia Patients with Long-Term Use of Antipsychotic Drugs.

OBJECTIVE: The development of metabolic disorders including diabetes mellitus and hyperlipidemia has been reported among schizophrenia patients treated with atypical antipsychotic drugs. The role of antipsychotic drugs in the development of this condition has not been proven yet. This study was conducted to investigate whether antipsychotic drugs that often induce weight gain influence glucose and lipid metabolism including insulin resistance and serum leptin level. METHODS: The study population consisted of 63 patients (all meeting DSM-IV criteria for schizophrenia), who were divided into 3 treatment groups: haloperidol (N=21), risperidone (N=21), and olanzapine (N=21) monotherapy, and 31 healthy control subjects. Fasting blood samples for glucose, insulin, leptin and lipids were analysed. In addition, insulin resistance (IR) was calculated through the homeostatic model assessment (HOMA) and body mass index (BMI) was also calculated. RESULTS: In patients receiving olanzapine, significant increases in BMI (p<0.01) and lipid profiles including LDL-cholesterol (p<0.05), triglyceride (p<0.01) and leptin levels (p<0.001) were found in comparison with the normal control group. A significantly higher degree of IR, as measured with the HOMA index, was found in patients receiving olanzapine than in patients receiving haloperidol (p<0.01) and risperidone (p<0.05), and in the normal control group (p<0.01). After removal of the impacts of BMI on the degree of HOMA-IR and serum leptin levels, the degree of HOMA-IR (p<0.05) and serum leptin levels (p<0.001) was also higher in patients receiving olanzapine than in the normal control group. CONCLUSION: The results of this study suggest that olanzapine has more significant influence on metabolic complications than haloperidol and risperidone and the characteristics of antipsychotic drug per se may be involved in the development of metabolic complication as well as weight change.

9.Tuberculous Brain Abscess of the Cerebellum.

The authors describe a case of tuberculous abscess of the cerebellum that originating probably from pulmonary lesion. The development of such a lesion indicates a persistence of infection and an immunological breakdown which may party have been due to unsatisfactory chemotherapy and protein malnutrition. Tuberculous brain abscess, an encapsulated collection of pus containing viable tubercle bacilli, is quite rare and it is different from tuberculoma in several points. Only five cases of tuberculous abscess have been fully documented and bacteriologically confirmed. The clinical, laboratory, and histopathological findings of those reported cases and the author's have been discussed.
