4.Effects of Patency of the Infarct-Related Artery on the Signal-Averaged ECG in Acute Myocardial Infarction.

BACKGROUND: In patients after acute myocardial infarction, signal-averaged electrocardiography is used as the one of the non-invasive methods for the prediction for ventricular arrhythmia, one of the causes ofn death in acute myocardial infarction. Signal-averaged electrocardiography has allowed the identification of low-amplitude, high-frequency signals(late potentials)in the terminal portion of the QRS complex. They are thougt to be occured in the portion of electrophysiologically unstable myocardium. The presence of late potentials identifies regions of delayed conducton in the elctrophysiologically unstable border zone of an acute infarction. These electrophysiologic change of myocardium is influenced by the patency of infarct-related artery. A patent artery is associated with electrical stability of myocardium, decreased in cidence of late potentials and improved survival. METHODS: 58 patients of acute myocardial infarction underwent signal-averaged electrocardiography, coronary angiography within 10 days after AMI, 20 of healthy persons underwent signal-averaged electrocardiography. RESULTS: In patent group, late potentials were recorded in 7 of 38 patients(18%) as compared with 13 of 20 patients(65%) of non-patent group. The statistically significant parameters of signal-averaged electrocardiography beteen patent and non-patent group were filtered total QRS duration(TQRS, 106.7+/-20.9msec), high frequency low amplitude signal (HLAS, 30.8+/-7.5 vs 41.3+/-16.5 msec)with HLAS being the most powerful varialble in the model. CONCLUSION: These results suggest that the patency of infarct-related artery is associated with electrophysiologic stability of myocardium and signal-averaged electrocardiography is one of the useful non-invasive method in risk stratification of acute myocrdial infarction.

Bisphosphonates (BPs) are a class of agent used to treat patient with osteoporosis or malignant bone metastases. BPs can be categorized into 2 groups: nitrogen-containing and non-nitrogen containing. Nitrogen-containing BPs are considered to have more toxicity. Despite their clinical benefits, bisphosphonate-related osteonecrosis of jaw(BRONJ) is a significant complication to patients receveing these drugs. Since the first description of BRONJ in 2003 by Marx, the number of reports on BRONJ has been rapidly increasing. BRONJ is considered as an emerging problem in oral & maxillofacial surgery. Generally, osteonecrosis in the maxilla is rare, however BRONJ is found both in the maxilla and the mandible. This is an important feature of BRONJ compared to common infectious osteomyelitis of the jaw. Growing number of case reports, suggest that bisphosphonate therapy may cause exposed, necrotic bone. BRONJ has simillar features compared to IORN (infected osteoradionecrosis). BRONJ has meaningful features established through the interestigation on histopathologic and radiographic findings. These features have an impact on treatment plan and prognosis. This presentation contemplates on features of histopathologic and radiographic findings in bisphosphonate-related osteonecrosis of the jaw.

We present a case report of brain death caused by Salmonella enterica serovar Enteritidis bacteremia. A 53-year-old diabetic male was admitted to our hospital with stupor mentality and fever. No evidence of meningitis was found upon cerebrospinal fluid analysis, but diffuse brain edema was shown by magnetic resonance imaging. S. Enteritidis phage type 7 was isolated from blood cultures. We diagnosed the patient with encephalopathy associated with S. Enteritidis bacteremia and treated him by intravenous administration of ceftriaxone and dexamethasone. Fifteen days after admission, the patient was diagnosed with brain death and became a cadaveric donor. Although encephalopathy associated with S. Enteritidis bacteremia is rare, it should be considered as a serious and potentially lethal complication.

8.Tuberculous Pseudoaneurysm of the Infrarenal Abdominal Aorta: 1 case report.

Tuberculous pseudoaneurysm of the abdominal aorta (TBAA) is an exceedingly rare, but fatal complication of the tuberculosis, especially in the condition of the disseminated tuberculosis. We reported a case of TBAA treated successfully by surgical and medical therapy. A 65-year-old woman admitted to our hospital complaining of sudden abdominal pain, nausea and vomiting for 3 days. She underwent 4 times operation of orthopedic surgery for tuberculous spondylitis at the level of the L4-5, T5-7, tuberculous epidural abscess, both psoas abscess, and had been receiving antituberculous medications for disseminated miliary tuberculosis at the both lung fields. Soon after this, she developed aggravated abdominal pain and we examined contrast enhanced abdominal CT scan and revealed saccular, thrombosed aneurysmal dilatation on the level of the aortic bifurcation. We performed elective operation for direct closure of the small rent at the anterior wall of the abdominal aorta after confirming no active inflammatory reaction on the frozen biopsy. Currently, the patient is an antituberculous medications without any surgical complication for 12 months after operation.

9.Clinical Studies of Measles Prevalence in Western Kyungnam District in 2000.

PURPOSE: To investigate the feature of the range of measles in 2000, we studied epidemics of the measles in western Kyungnam District. METHODS: Among 489 patients under 15 years of age who visited or were admitted to in our hospital from January to December in 2000, we selected 344 patients whose measles IgM antibody was positive. We investigated age, monthly incidence, clinical features, vaccination history, and measles IgG antibody. RESULTS: Patients under 12 months of age occupied the largest proportion(36.9%). Patients firstly occurred in May and geometrically increased from October. Fever, cough and rashes were observed in all patients but conjunctivitis in 54.9%, Koplik spot in 23.8%. The first vaccination ratio was 41.8%. Compared with the number of patients and the first vaccination ratio, the first vaccination ratio was the lowest under 12 months of age. After that age group, the number of patients was decreased as the first vaccination ratio was increased. Among 152 patients sampled for measles IgG antibody, 35 patients had received the first vaccination and 6 patients had received the second vaccination. Among 35 patients who received the first vaccination, 22(62.9%) patients were negative of measles IgG antibody. Among 6 patients who received the second vaccination, 3(50%) patients were negative. CONCLUSIONS: Measles patients under 12 months still dominated. Therefore, routine vaccination of single measles vaccine, is currently done at 6 month when measles are prevalent, should be considered. High first vaccination failure suggests problems of vaccines itself, transport and storage rather than vaccination methods. Therefore a thorough investigation should be made.

The occurrences of thyrotoxicosis in patients with myasthenia gravis have been reported before the knowledge of the pathogenesis of the two disease. Thytotoxicosis is known to occur in 3 to 6 percent of patients with myasthenia gravis and myasthenia gravis occurs in only a fraction of 1 percent of the thyrotoxic populatian. Myasthenia gravis is currently considered as a systemic autoimmune disorder of acetylcholine receptor and often presented with other autoimmune diseases such as SLE, Rheumatoid arthritis. We experienced a 18-year-old woman who presented with graves disease and isolated ocular myasthenia gravis. Chest CT didnot reveal enlarged thymus. The usual treatement of myasthenia gravis associated with thymtoxicosis consists of medical control of the thyrotoxicosis, then thymectomy and later subtotal thyroidectomy. Her ptosis and thyrotoxicosis have improved after the medicatian of anticholinesterase and propylthiourecil. A case of ocular myasthenia gravis with Gravesdisease was experienced, so we reported the case with a brief review of literature.
