1.Male Infertility: XII. Pharmacological Treatment for Improving Semen Motility.

Some investigators suggest that the pancreatic proteinase kallikrein plays an important role in the regulation of spermatozoal motility. Particularly, oral kallikrein therapy exerted a favorable effect on Sperm motility in oligozoospermia and asthenozoospermia. We have carried out a similar clinical investigation of the efficacy of kallikrein, taken orally 60 kU per day for 3-9 months, on the quantitative and qualitative motility of spermatozoa in normogonadotropic infertile men. with 15 idiopathic oligozoospermia and 18 idiopathic asthenozoospermia. Number of spermatozoa increased more than double number of basic levels (over 40 x 10(6)/ml) in the 5 patients (33%) and pregnancy occurred in the 3 patients (20%) out of the 15 patients with idiopathic oligozoospermia (less than 20 x 10(6)/ml) after the kallikrein therapy. In these responded 5 patients, the sperm concentration changed from 13.6 x 10(6)/ml to 54.0 x 10(6)/ml, Motility and viability of spermatozoa improved more than 30% in the 5 patients (28%) and pregnancy occurred in the 2patients (11%) out of the 18 patients with idiopathic asthenozoospermia (less than 20% of sperm motility) after the therapy. In these. improved 5 patients, the sperm motility changed from 9.0% to 45.0%. No remarkable side effect was detected.

4.Self Role Perception of Health Teachers in Elementary Schools.

PURPOSE: The purpose of this study was to describe and understand elementary school health teachers' role perception in their own perspectives. METHODS: Data were collected from three focus groups composed of five participants for each group. Each focus group had an interview for two and a half hours on the average. The main question was "What is your perception on the roles of health teachers in elementary schools?" Qualitative data from transcribed notes and field notes were analyzed using qualitative content analysis. RESULTS: Five main roles were identified from the participants. The participants perceived themselves as 'an expert of health education in school and community', 'a health service provider for school members', 'a health guardian for school members', 'the coordinator of healthcare related works in school', and 'the leader of school health'. CONCLUSION: The participants have multiple role identities, which are very important for health management of school members. And the roles such as an expert of health education and a leader for school health are recently emphasized. The findings of this study can provide useful information to design orientation programs for newly appointed health teachers and continuing education programs for enhancing role performance of health teachers in elementary schools.

5.Follow up Study of Outcome in Severe Hyperbilirubinemic Newborns Treated with Exchange Transfusion and Phototherapy.

PURPOSE: Cerebral palsy or hearing disability of hyperbilirubinemic complication was reduced by blood exchange transfusion(BET) and phototherapy(PT). But in spite of these treatment, abnormal Auditory Brainstem evoked Response(ABR) finding after BET or PT and neurodevelopmental defect due to chronic bilirubin encephalopathy were observed. So we have studied risk factors and outcome of chronic bilirubin encephalopathy after BET, and treatment of hyperbilirubinemia. METHODS: We have analyzed clinical characteristics, the finding and change of ABR after BET in 17 hyperbilirubinemic neonates, and in 8 hyperbilirubinemic neonates who were treated by phototherapy and 15 normal control neonates. RESULTS: 1) Mean bilirubin concentraion were 27.5+/-4.1mg/dL in BET group and 22.1+/-2.3 mg/dL in PT group. There were no difference of clinical findings between BET and PT group. 2) Change of ABR (1) Wave I loss resulted in 4 neonates, wave III loss in 3 neonates, and wave V loss in 2 neonates in BET group(P<0.05). (2) Wave I peak latency and hearing threshold in BET group were significantly increased more than normal control group(P<0.01). 3) In 10 neonates(58.8%) among 17 BET group, 4 neonates(50%) in 8 PT group were observed abnormal initial ABR finding after jaundice treatment. Age at treatment and duration of jaundice(interval between onset of jaundice and treatment) in abnormal ABR group were significant prolongation compared with normal ABR group(P<0.05). 4) Chronic bilirubin encephalopathy(CBE) was observed in 3 neonates(17.6%) among 17 BET group and showed higher of bilirubin level than normalized group after BET (31.1mg/dL vs 26.6mg/dL), other clinical findings showed no significant differences. CONCLUSION: Bilirubin level was significantly elevated in CBE more than in BET group and duration of jaundice, age at treatment were longer in abnormal ABR group than in normal ABR group. So not only bilirubin level but also duration of jaundice shoud be considered at jaundice treatment, and ABR has a potential utility in detection of acute brain toxicity of bilirubin and follow up evaluation of bilirubin encephalopathy.

6.A Case of Meibomian Gland Adenocarcinoma.

The clinical resemblance of carcinoma of the Meibomian gland to the relatively frequent chaiazion should be kept in mind by anyone performing eye lid surgery. A 46-year-oId Korean house wife was first visited here in May 2nd '69 with complaints of localized swelling, ulceration and dull pain on the left upper lid. She had been taken the operation for chaiazion on the same side of the left upper eye lid a year ago. Examination revealed a reddened hard, tender lesion elevating the skin on the medial one third of the left upper lid with ulceration at its center. The tarsal conjunctiva was noted yellowish patches with ulceraion extended to its margin. Local resection with full thickness containing normal tissue around he lesion of upper lid was performed and graft was made in lower lid and rotatory transplantation was given, and pathologic examination revealed adeno-carcinoma of Meibimian gland. There was no evidence of recurrence and metastasis to remote site a year after surgery.

8.Primary Duodenal Cancer Combined with Gastric Cancer after Treatment of Cervical Cancer -triple primary cancer.

Multiple primary tumors are defined as cases involving primary malignant tumors of different histologic origins in one person. The absolute number of reported cases of double primary malignant tumors has increased in recent years diagnostic procedures. Primary adenocarcinoma of duodenum is a rare disease and represents less than 0.5% of all gastrointestinal malignancies. Therefore, synchronous multiple primary cancers of the stomach and duodenum are very rare. Recently, we experienced a case of triple primary malignant tumors of different site originating from the stomach, duodenum and cervix in 71- year-old woman. She had had radiation therapy and chemotherapy for squamous cell carcinoma of the uterine cervix 8 years previously. We performed subtotal gastrectomy and pancreaticoduodenectomy with radical lymph node dissection. She remained healthy without any evidence of recurrence 12 months after the operation.

9.Urinary Transforming Growth Factor-beta-inducible Gene-h3 in Patients with Glomerular Diseases.

BACKGOUND: Transforming growth factor-beta (TGF-alpha) has been implicated in the pathogenesis of a number of kidney diseases. However, TGF-alpha is secreted in a latent form requiring extracellular modification to become biologically active. Recently, the activity of TGF-alpha has been assessed by the measurement betaig-h3, a novel TGF-alpha induced gene product. Thus we evaluated the pattern of urinary betaig-h3 expression in various glomerular diseases. METHODS: 64 patients with biopsy-proven primary glomerulonephritis (FSGS 6, HSPN 16, IgAN 20, MPGN I 7, and MesPGN 15), 10 patients with nephrotic syndrome and 12 healthy controls were enrolled in the study. A total 86 subjects (51 males, 59.3% and 35 females, 40.7%, mean age 13.9+/-4.28 years) constituted study population. First morning urine were collected and betaig-h3 in the urine was determined by indirect competitive ELISA (Regen Biotech Inc, Seoul, Korea). RESULTS: betaig-h3 excretion was significantly higher in the urine from patients with HSPN (27.5+/-6.46, p=0.002), with IgAN (40.83+/-12.27, p=0.026), with MPGN I (21.64+/-7.29, p=0.042), MesPGN (26.42+/-6.68, p=0.007). In patients with FSGS (21.65+/-17.12) and minimal change nephrotic syndrome (6.26+/-2.18), mean urinary betaig-h3 excretion was not significant higher than that in control group (3.56+/-0.78). CONCLUSION: Urinary betaig-h3 excretion was high in proliferative renal diseases. However, betaig-h3 excretion was not high in non-proliferative renal diseases.
