Chronic fatigue of the respiratory muscles has contributed to the decreased ventilatory capacity and reduced excercise tolerance of individuals with COPD, especially in kyphoscoliosis. Nasal mask BiPAP has been shown to be useful for the patient with nocturnal muscle fatigue and COPD. A 35-year-old man with severe kyphoscoliosis was admitted to ICU due to acute respiratory failure. He had been diagnosed of COPD and had been intubated with mechanical ventilatory support for 7 times. This time he was intubated with ventilatory support, too, in ICU and readmitted to the ICU for severe hypoxemia and hypercarbia from general ward. Thereafter he refused the intubation. Nasal mask BiPAP ventilatory support system was applied and IPAP, EPAP level being adjusted to the 12, 4 cmH2O under monitoring vital signs and arterial blood gas analysis. His condition was improved and discharged home with support of nasal mask BiPAP system after 33 day-stay in the ICU.

5.A Case of Systematized Nevus Sebaceous.

A patient with nevus sebaceous showing linear or wide distribution can present with a triad of nevus sebaceous, seizures, and mental retardation. Most of those cases are classified as sebaceous nevus syndrome. Ophthalmological, skeletal, and other abnormalities may also be present. A male weighing 3,580 g was born at the gestational age of 38 weeks after a normal pregnancy. He was referred to the dermatologic department on the first day of life. He presented with yellow-orange and verrucous plaques on both sides of the temporal areas, face, neck, upper and lower extremities, and trunk. Skin biopsy specimens were taken from the scalp and right lower leg. Histologically, both specimens showed sebaceous gland hyperplasia, absence of mature hair follicles, infundibular cystic structures, and apocrine glands. Brain magnetic resonance imaging, transthoracic echocardiography, abdomen ultrasonography, and routine laboratory investigations were performed and showed nonspecific findings. The patient did not show seizure activity at 19 days of observation, and he is now on close observation. We herein report a very rare case of systematized nevus sebaceous presenting at birth.

6.Multiple Giant Keratoacanthoma Treated with Acitretin.

Giant keratoacanthoma is an uncommon variant of keratoacanthoma, which may increase to a diameter of several centimeters. Although keratoacanthomas usually resolve spontaneously, giant keratoacanthoma can be invasive and destructive. A 49-year-old man presented with a 5-year history of multiple large hyperkeratotic and crusted plaques and nodules on sun-exposed areas such as the face, ear, hand, and forearm. Some lesions involuted spontaneously, whereas others became rather enlarged. The biopsy specimen revealed horn-filled crater formation, epidermal extending resembling a buttress, and an eosinophilic glassy appearance in the keratinocytic cytoplasm. We treated the lesions with acitretin, and they almost completely resolved after 13 weeks. Here we describe a case of multiple giant keratoacanthoma treated with acitretin.

7.Prevalences of Actinic Skin Lesions and Comparison of Single and Multiple Lesions in Patients with Actinic Keratosis: A Retrospective Study of 61 Patients.

BACKGROUND: Photoaged skin is characterized by actinic skin lesions such as wrinkling, elastosis, and premalignant and malignant lesions. Actinic keratosis (AK) is one of the most common precancerous lesions, and multiple actinic keratosis lesions are one of the risk factors for skin cancer. Few studies have investigated the prevalences of actinic skin lesions and compared the characteristics of the lesions and the patients with sinle and multiple AK lesions. OBJECTIVE: The aim of this study was to evaluate the prevalences of actinic skin lesions and to compare characteristics between single and multiple lesions in Korean patients with AK. METHODS: We retrospectively reviewed the medical records and biopsy specimens of 61 patients. RESULTS: Frequencies of deep wrinkle, solar elastosis, cutis rhomboidalis nuchae, and senile purpura were higher in the multiple lesions group. The ratio of males to females was 1:3.43 and the mean age was 79 years in the multiple lesions group, both of which were higher than those in the single lesion group (1:1.14, 72 years). The mean duration was longer in the multiple lesions group. Sun protective behavior was more frequent in the single lesion group, and the occupation of farmer was more common in the multiple lesions group. There was no statistically significant clinicopathological difference between the two groups. CONCLUSION: This study may improve our understanding of the characteristics of actinic keratosis with both single and multiple lesions.

8.Assessing Olfactory Function in Healthy Korean Children Using the Cross-Cultural Smell Identification Test and Butanol Threshold Test.

BACKGROUND AND OBJECTIVES: There are no reported studies of olfactory function of Korean children, and the existing tests of olfactory function for Korean adults may not be suitable for very young children. This study assessed the applicability of the Butanol Threshold Test (BTT) and Cross-Cultural Smell Identification Test (CC-SIT) to children. SUBJECTS AND METHOD: A total of 79 children were included in the study: they were between 6-12 years of age, and had visited University Hospital Health Care Center between January 2012 and December 2013. All children were administered the BTT and CC-SIT. RESULTS: Using BTT, 69.62% of the sample was classified as moderate hyposmia. On the other hand, when CC-SIT was used, 45.57% of the sample was classified as moderate and 43.04% as mild hyposmia. CC-SIT and BTT scores were not correlated. Although gender and age were not taken into account in the test results, the CC-SIT could measure age-specific olfactory development. CONCLUSION: Our study provides fundamental data on the clinical use of the CC-SIT and BTT in healthy Korean children.

9.Predictors of drug-resistance in patients with pulmonary tuberculosis.

BACKGROUND: The drug-resistant tuberculosis has recently decreased in Korea, but it is still one of the major obstacles in the treatment of pulmonary tuberculosis. Unfortunately there are no reliable ways to figure out the drug sensitivity pattern of the M. tuberculosis in the starting point of treatment. At least several months which is critical for the success of treatment have to be passed away before getting the report of drug-sensitivity test. The aim of this study was to find out the clinical and radiological parameters that make it possible to predict the drug-resistant pulmonary tuberculosis and to make a correct decision on the antituberculosis drug regimens. METHOD: We studied 253 pulmonary TB patients with sputum and/or bronchial washing fluid culture-positive diagnosed at the Chung-Ang University Young-San Hospital in the period of 1989-1994. The differences in the clinical and raiological variables between the drug-sensitive and the drug-resistant tuberculosis patients were evaluated. RESULTS: In 66 out of 253 patients(26.1%), drug resistant tuberculosis to at least one antituberculosis drug were found. Patients with retreatment showed higher resistance rate than those with initial treatment (30/69,43.5% vs 36/184, 19.5%, p<0.01). Patients with cavitary TB showed higher resistance rate than those with non-cavitary TB( (24/54, 44.4% vs 42/199, 21.1%, p<0.05). Among patients with initial treatment, those with far-advanced TB showed a higher drug resistance rate than those with minimal lesion(9/23, 36.9% vs 10/82, 12.5%, p<0.05). Patients with culture positive only in the bronchial washing fluid showed lower resistance rate than those with sputum culture positive(7/63, 11.1% vs 59/190, 31.1%, p<0.05) CONCLUSION: Prior treatment history for pulmonary tuberculosis, the presence of cavity & far advanced tuberculosis in the radiologic exam, sputum rather than solely bronchial washing culture positivity would be the related factors to the drug resistance. So in the patients with such characteristics, it is needed to try to find out the drug sensitivity pattern of the infecting tuberculosis organism as soon as possible.

10.The Effect of Steroid on the Treatment of Endobronchial Tuberculosis.

BACKGROUND: Endobronchial tuberculosis is one of the serious complications of pulmonary tuberculosis. Without early diagnosis and proper treatment of endobronchial tuberculosis, bronchostenosis can leave and lead to the collapse of distal lung parenchyme, bronchiectasis, and secondary pneumonia accompanied with moderate to severe dyspnea, cough, hemoptysis, and localized wheezing. Therefore steroid therapy has been tried to prevent bronchostenosis. But the effect of steroid therapy on the endobronchial tuberculosis is not definite at present. We tried to elucidate the effect of steroid on the treatment of endobronchial tuberculosis for prevention of bronchoste- nosis. METHODS: We observed the initial and sequential bronchoscopic findings, pulmonary function tests and simple chest roentgenograms in 58 patients diagnosed as endobronchial tuberculosis and admitted to Chung-Ang university hospital from 1988 to 1992. The patients in nonsteroid group (n=39) were treated with anti-tuberculosis chemotherapy only and steroid group(n=17) with combined steroid therapy. Sequential bronchoscopic findings, pulmonary function tests, and chest roentgenograms were comparatively analyzed between the two groups. RESULTS: 1) The endobronchial tuberculosis was highly prevalent in young females especially in third decade. 2) Both actively caseating type and the stenotic type without fibrosis was the most common in the bronchoscopic classification. 3) The sequential bronchoscopic findings in steroid group 2 months after treatment showed no significant improvements compared with nonsteroid group. 4) There was no significant difference between the two groups in the sequential bronchoscopic improvements according to bronchoscopic types. 5) We did not find any significant difference in improvements on follow-up pulmonary function tests and simple chest roentgenograms between the two groups 2 month after treatment. 6) There was no significant adverse effect of steroid during the treatment. CONCLUSION: Combined steroid therapy provably would not influence outcome of the treatment of endobronchial tuberculosis.
