With the improvement of economic conditions and medical care, the average life span has increased, thus making geriatrics an important consideration in the medical field. Degenerative changes of the cervical spine in aged are not uncommon. Also it is well known tha subjects with radiologic features of cervical spondylosis may be symptom free. On the other hand, cervical myelopathy may occur in the presence of minimal spondylosis as shown radiologically. This discrepancy between symptoms and radiologic features seems to be attributed to the differences in the initial structure of the cervical spine. The authors surveyed at random the various radiographic measurements of 300 Koreans having normal cervical spines and 120 Koreans with cervical spondylosis. The results of the survey are as follows: 1. Osteophyte was noticed more frequently at the 3 rd, 4 th and 5 th cervical vertebrae. 2. Lordosis angle was greater in spondylosis group than in normal group, which might be attributed to paravertebral muscle spasm. 3. Normal sagittal diameter of the canal decreased between C3-C6 and the narrowest was at C4 level. The sagittal diameter, on the average, was 1.5 mm shorter than in European population and 1 mm longer than Japanese. This might be attributed to the difference in the body height. 4. In the normal spine group the average interpedicular distance was greatest at the 5 th, and 6 th cervical vertebra. In the cervical spondylosis group the average interpedicular distance was greater than in normal population it was greatest at the 5 th and 6 th cervical vertebra.

We observed allergic cutaneous reaction to vitsmin K, injection in 3 patients who developed pruritic erythematous indurated plaques at the sites of intramuscular injection and leakage sites of vitamin K1 preparation into dermis during IV injection 12 to 23 days after injection. Intradermal tests with vitamin K1,K3 and their placebos performed on 145 healthy medical students revealed positive skin reaction in 13 students 7 to 22 days after injection similar to the original 3 cases. Five students also showed the same reaction to vitarnin K1 placebo, however, a more severe reaction to vitamin K1 preparation than its placebo in 3 of them, indicating that the index of cutaneous sensitivity of vitamin K1 lies somewhere between 5. 5,% and 8. 9%. There was no cross sensitivity between vitamin K1 and K3 We consider these reactions to be of allergic nature.

De Lange Syndrome*

The application of fine needle aspiration (FNA) cytology to the soft tissue tumors had been neglected. In recent years, however, FNA has been used increasingly in the preoperative diagnosis of these tumors due to its usefulness and accuracy. We present 3 cases of liposarcoma, myxoid, myxoid with round cell, and pleomorphic, diagnosed by FNA cytology with histologic confirmation. Good correlation between his- tologic and FNA cytologic findings was found. Although the cytologic appearances of liposarcomas varied with histologic type, the main criterion was the presence of atypical multivacuolated lipoblast with characteristically scalloped nuclei.

Hypereosinophilic Syndrome*

Post-traumatic encephalopathy is characterized by the combination of upper motor neurons, basal ganglia, cerebellar, and psychiatric disturbances. A "striatal" variant, with predominant parkinsonism, is uncommon and may be difficult to distinguish from idiopathic Parkinson's disease. A 38-year-old man, who had history of head injuries associated with brief losses of consciousness, developed a resting tremor and rigidity of the left lower extremity. Upon admission, he showed masked face and bradykinesia without dementia or other neurologic abnormalities. Brain MRI and SPECT studies showed no abnormalities. A BAEP study revealed an abnormal prolongation of inter-peak latencies between wave III and V.

Isolated dislocation of all five carpometacarpal joints is extremely rare, only eleven cases had been reported since 1873. In Korea, we have not seen it probably. We experienced one case of isolated dislocation of all five carpometacarpal joints with a good result in I year after closed reduction and internal fixation.

Isometric positioning of the ACL graft is an important consideration in successful reconstruction of the ACL-deficient knee. The purpose of this study is to get a certain guideline in the endoscopic one-tunnel technique of anterior cruciate ligament reconstruction by measuring the skin angle and determine the degree of minimum flexion of the knee joint during femoral tunneling and interference screw fixations. To get the guide lines, first we get the tibial tunnel angle parallel to the Blumensaat's line from fully extended lateral knee joint radiography. Secondly measure the differences between angles of the femur-tibia shaft and anterior thigh-leg skin. Then measure the minimum femur-tibia flexion angle does not perforated the posterior cortex of the distal femur during femoral tunneling. Intraoperative measuring the angle between interference screw guide pin and tibial tunnel to get the parallelism of the femoral tunnel and interference screw. The results were as follows; The average femur-tibia shaft angle with 30degrees anterior thigh-leg skin angle was 30.2+/-1.75degrees, with 45degrees was 45.2+/-1.23degrees, with 60degrees was 61.9+/-4.23degrees, with 75degrees was 78.6+/-2.62degrees, with 90degrees was 97.8+/-3.96degrees. Predetermined sagittal tibial tunnel vector on the 0degrees extension knee joint lateral radiographs were applied to the several knee joint dynamograms. The mean minimum flexion angle of the femur-tibia shaft that doesn't perforate the posterior cortex of the femur was 45+/-1.58degrees (male), 44.5+/-4.97degrees (female). The average angle between interference screw guide pin and tibial tunnel was 23.0+/-2.23degrees. The findings of the present study suggest that anterior thigh-leg skin angle can be used instead of the true femur-tibia shaft angle. Less knee flexion angle makes good arthroscopic view during the tibio-femoral tunneling and interference screw fixation.
