1.A Clinical Observation on the Nephrectomized Patients.

A clinical observation was on the nephrectomized patients in the Department of Urology, Korea General Hospital, during the period from January, 1978 to December, 1981 and the following were obtained. 1. The total number of in-patients during the period was 723 patients and nephrectomies were performed in63 patients of the total Urologic operation (13.2%). 2. There were 40 males and 23 females with a ratio 1.7:1, patient ranged in age from 9 months to 69 years old and 71.4% of the patients were from 20 years old to 49 years old. 3. The common symptoms were flank pain (34.9%), hematuria (31.7%) and frequency (23.8%). 4. The causative diseases of the operated kidney were tuberculosis (31.7%), chronic pyelonephritis (15.9%),hydronephrosis (12.7%), renal stone (11.1%) and renal stone (11.1%) in order. 5. Flank approach was most frequently used (76.2%). 6. The postoperative complications were 12 cases (19.0%), which included 4 cases of wound infection, 2 cases of transient paralytic ileus, 2 cases of pneumothorax and 1 case of bleeding, pulmonary edema, duodenal fistula and death respectively. 7. 58 cases (92.0%) were discharged within 2 weeks after operation and 2 cases were hospitalized more than 1 month.

2.Clinical 0bservation on 48 Cases of Transrectal Prostatic Biopsy.

Diagnosis of adenocarcinoma of the prostate based purely on digital rectal examination will be correct in only 70% of cases. Transitional cell carcinoma also arises in the prostate, as may more rarely sarcoma. The fibrous prostate, granulomatous prostatitis and stone may also result in a prostate that feels firmer than normal. Estrogen or orchiectomy will be of no benefit in these condition and may cause harm. Histological or cytological proof of the diagnosis is essential. Examination of prostatic tissue obtained by biopsy has been used to confirm malignancy or any type, judge the response of a tumor to either hormonal or cytotoxic manipulation and bacteriologic confirmation. The transrectal prostatic biopsy was performed on 48 cases of oriental, from May 1979 to February 1982. The results are as follows. 1. The patients ranged in age from 25 to 82 years old and the percentage of patients more than 50 years old is 85.4% (38 cases). 2. The highest reason of referral for prostatic biopsy is obstructive symptoms (62.5% - 30 cases). 3. As shown in table 3, only in 2 cases the specimens obtained with Silverman needle proved inadequate for a reliable diagnosis even though in 46 cases the specimens were thought to be adequate. In 46 cases, the highest disease is B.P.H. (73.9% - 34 cases), 2nd prostatic adenocarcinoma (17.5% - 8 cases), 3fd chronic prostatitis (4.3% - 2 cases) and normal prostate (4.3% - 2 cases). 4. There were 7 complications of transrectal prostatic biopsy and the hematuria (4 cases) was the most common.

6.Renal scarring in relation to visicoureternal reflux in urinary tract infection.

The vesicoureteral reflux (VUR) is frequently found among the patients with urinary tract infection (UIT) and some patients with VUR progress to reflux nephropathy. The presence of scarring at UIT is an important determinant in the selection of those at risk of progressive damage. 99m Technetium dimercaptosuccinic acid (99m Tc-DMSA) is a renal scanning agent provides a good quality of renal image as a result of preferential cortical accumulation. 99m Tc-DMSA scan and VCUG were performed in 133 patients diagnosed UTI at pediatric department of Kyung Hee University, College of Medicine from May, 1990 to May, 1992 The results were as follows: 1) VUR was seen in 27 children among 133 patients with UTI. The incidence was 20.3%. Under 1 year of age, male to female ratio was higher incidence of VUR. 2) Incidence of renal scarring was higher in patients with VUR than those of without VUR. 3) Grading of VUR in relation to age, the older the age, the lower the grade. 4) There is no correlation between renal scarring and grades of VUR. 5) In distribution of renal scarring, it was found to have tendency to develop at upper polar area, there was diffuse and multiple in patients with VUR. In conclusion the incidence of renal scarring was related to the age of onset, duration of UTI and the severity of the VUR and 99mTc-DMSA scan is mandatory in patient with UTI even without VUR in order to detect early phase of renal scarring.

7.Pulmonary Artery Sling with Situs Solitus Dextroposition of Heart and Left Superior Vena Cava.

Pulmonary artery sling is a rare congenital cardiovascular anomaly which presents clinical obstructive symptoms and signs of trachea, right main bronchus and esophagus due to left pulmonary artery abnormaly arises from proximal part of right pulmonary artery. Aberrant left pulmonary artery courses to the right above right main bronchus and then turns to the left, and courses to between the anterior of the esophagus and the posterior aspect of trachea. One of important clinical significances of the pulmonary artery sling is associated with intracardiac anomalies, and with tracheal stenosis and bronchomalacia due to press tracheobronchial tree by the aberrant pulmonary artery. It has been thought that embryological malconnection between the pulmonary artery bud from left sixth arch and the pulmonary posterior branchial plexus in the left lung bud. We had experienced 21 months old male infant with left pulmonary artery sling associated with persistent left superior vena cava and dextropostioned heart. The heart seems to move to right hemithorax due to hypoplasia or segmental atelectasis of right lung.

8.Identification of Caenorhabditis elegans MicroRNA Targets Using a Kernel Method.

BACKGROUND: MicroRNAs (miRNAs)are a class of noncoding RNAs found in various organisms such as plants and mammals. However, most of the mRNAs regulated by miRNAs are unknown. Furthermore, miRNA targets in genomes cannot be identified by standard sequence comparison since their complementarity to the target sequence is imperfect in general. In thi s paper, we propose a kernel-based method for the efficient prediction of miRNA targets. To help in distinguishing the false positives from potentially valid targets, we elucidate the features common in experimentally confirmed targets. RESULTS: The performance of our prediction method was evaluated by five-fold cross-validation. Our method showed 0.64 and 0.98 in sensitivity and in specificity, respectively. Also, the proposed method reduced the number of false positives by half compared with TargetScan. We investigated the effect of feature sets on the classification of miRNA targets. Finally, we predicted miRNA targets for several miRNAs in the Caenorhabditis elegans (C.elegans )3'untranslated region (3'UTR) database. CONCLUSIONS: The targets predicted by the suggested method will help in validating more miRNA targets and ultimately in revealing the role of small RNAs in the regulation of genomes. Our algorithm for miRNA target site detection will be able to be improved by additional experimental-knowledge. Also, the increase of the number of confirmed targets is expected to reveal general structural features that can be used to improve their detection.
