1.Wernicke Encephalopathy Associated with Acute Wet Beriberi.

Wernicke encephalopathy is usually accompanied with peripheral neuropathy, known as dry beriberi. In contrast, wet beriberi presenting as cardiovascular symptoms rarely occurs. The acute type of wet beriberi can be fatal, if untreated quickly. It is reported that the cerebellar vermis has a role of the coordination and control of cardiovascular and autonomic reflex activities. We report a 58-year-old man showing acute wet beriberi in Wernicke encephalopathy with cerebellar vermis lesion.

2.A Case of Extramedullary Plasmacytoma of the Larynx.

Extramedullary plasmacytoma is a rare soft tissue malignancy composed of plasma cells. This tumor may be primary or secondary to the generalized multiple myeloma. The great majority of extramedullary plasmacytoma occurs in the head and neck region, mainly in the upper respiratory tract and oral cavity. The most frequently involved sites are the nasal cavity, paranasal sinus, and nasopharynx, in the decreasing order. However extramedullary plasmacytoma of the larynx is rare. Laryngeal plasmacytomas accounts for between 6 to 18% of all extramedullary plasmacytomas. We report, with a review of literature, a case of extramedullary plasmacytoma of the larynx in a 41 year-old female patient.

Focal subarachnoid hemorrhage occasionally presents as transient focal neurologic episodes mimicking transient ischemic attack (TIA). Unless properly diagnosed, it may aggravate cerebral hemorrhage by administering antithrombotic agents. Therefore, clinicians need to be aware that such focal subarachnoid hemorrhage sometimes cannot be detected on noncontrast computed tomography and blood-sensitive magnetic resonance imaging can detect even a small amount of hemorrhage. We describe an 85-year-old woman with focal subarachnoid hemorrhage and possible cerebral amyloid angiopathy who presented transient left arm weakness recurrently, which mimicked TIA.

5.Surgical Guideline of Polypoid Lesions of Gallbladder.

BACKGRAOUND/AIMS: The biologic nature of polypoid lesions of gallbladder is difficult to define before surgical intervention and operative indication is still controversial. The aim of this study is to provide surgical guideline for polypoid lesions of gallbladder. METHODS: Clinical data were retrospectively correlated with the histopathologic characteristics of polypoid lesions of gallbladder in 48 patients who had cholecystectomy from January 1992 to August 2002 in the Department of Surgery, Kang-nam General Hospital Public Corporation. RESULTS: There were 40 benign polypoid lesions including 30 cholesterol polyps, 5 adenomas, 4 inflammatory polyps and 1 adenomyomatous hyperplasia and 8 malignant polypoid lesions. Data analysis showed that neoplastic polypoid lesions of gallbladder correlated significantly with size and malignant polypoid lesions of gallbladder correlated significantly with size and age. CONCLUSION: The risk factor for neoplasm was the size of the polypoid lesions of gallblaldder (> or =10 mm) and the risk factors for malignancy were the size of the polypoid lesions of gallblaldder (> or =10 mm) and age (> or =50). Surgical intervention should be considered when a polypoid lesion of the gallbladder is larger than 10 mm and patient is older than 50 years.

6.Laparoscopic Colorectal Resection after Endoscopic Stent Insertion in Cases of Malignant Colorectal Obstruction: the Experience of a Single Center.

PURPOSE: Laparoscopic surgery has been considered to be contraindicated for obstructive colorectal cancer. However, endoscopic stent insertion for obstructive colorectal cancer has recently allowed elective laparoscopic surgery. The aim of this study is to evaluate the feasibility and the short-term clinical outcomes of laparoscopic surgery following endoscopic stent insertion for management of malignant colorectal obstruction at a single center. METHODS: The medical records of patients who had undergone endoscopic stent insertion for colorectal cancer obstruction, followed by laparoscopic colorectal resections, from August 2004 to August 2008 were reviewed. To evaluate the surgical and clinical outcomes, we analyzed the clinical and pathologic data. RESULTS: Thirty-six endoscopic stent insertions were successfully performed during the study period. Of those 36, the 28 treated by laparoscopic surgery were enrolled in this study. The mean interval between stent insertion and surgery was 7.4+/-2.3 days. Two cases were converted to open surgery. The mean operative time was 185.5+/-53.1 min, and the mean blood loss was 77.0+/-72.9 mL. Flatus was passed on the mean 2nd postoperative day, and patients started to eat on the mean 4th postoperative day. The mean postoperative hospital stay was 11.2+/-4.4 days. Anastomosis leakages occurred in two cases and were treated by a secondary operation with a transient ileostomy. There were morbidities in five cases, but no mortalities. CONCLUSION: A combined endoscopic stent insertion and laparoscopic surgery is an effective and safe, minimally invasive operation for malignant colorectal obstruction.

Pulmonary siderosis is one kind of pneumoconiosis, occurs from chronic inhalation of iron or iron oxide. Inhaled iron dust is deposited in the intra-alveolar spaces, which leads to radiological changes and respiratory symptoms. It is diagnosed by iron exposure history, radiological changes, and the evidence of intra-alveolar iron deposit. We have experienced a case of pulmonary siderosis which was confirmed by bronchoalveolar lavage and transbronchial lung biopsy, so report it with a review of literature.

Rheumatoid arthritis(RA)is occasionally associated with variable extra-articular involvement and the chronic inflammatory process can affect the gastrointestinal system.The gastrointestinal involvement in RA may present in many causes:drug induced colitis,vasculitis and amyloidosis involved in the gut,association with certain bowel diseases such as collagenous colitis or infectious colitis. Ulcerative colitis(UC)is a chronic inflammatory bowel disease and is commonly associated with peripheral joint disease which correlates with the disease activity and extent of the bowel inflammation.The arthritis is usually presented in pauciarticular,generally asymmetric,transient,and nondestructive pattern.However,the chronic and destructive peripheral arthritis has been reported in a few cases and RA has not been observed in association with UC. We experienced a case of RA patient with recurrent abdominal pain,hematochezia,and tenesmus who was diagnosed as UC by endoscopic and histologic finding.We herein report the case with literature.

9.Clinical Analysis of Basal Skull Fracture.

BACKGROUND AND OBJECTIVES: Basal skull fractures are different from cranial vault fractures in several aspects. These fractures are difficult to diagnose by ordinary X-ray, routine head CT and are frequently inferred from clinical signs. It involves more commonly the cranial nerves, makes fistulae of cerebrospinal fluid (CSF) and lead to central nervous system infections. Despite the clinical significance of basal skull fracture in otorhinolaryngology, there have been only a few clinical studies. So we began this research for better treatment of basal skull fracture (BSF). MATERIALS AND METHOD: The authors analyzed 100 cases of basal skull fracture treated in the department of neurosurgery, from January 1998 to December 2000. We reviewed the clinical features, radiologic findings, and rate of delayed diagnosis. RESULTS: The basal skull fractures were more common in men than women. In the decreasing order of cause in basal skull fractures were traffic accidents (64%), fall down and assault. Clinical features were nasal bleeding (42%), ear bleeding (41%) and hearing loss (36%). Most of CSF leakages were noted within 24 hours after injury and had ceased by conservative management. The degree of facial palsy had an influence on the recovery of facial nerve function. The types of hearing loss had a role in the recovery of hearing. Six cases (6%) were delayed in the diagnosis of BSF. CONCLUSION: In the BSF patient's treatment, many parts needed otorhinolaryngological treatment. But in the patient's treatment, otorhinolaryngological problems had been ignored by the neurosurgical concerns. From the otorhinolaryngoloical aspect, BSF patients should be given an active treatment.
