1.A Case of Cavernous Sinus Thrombophlebitis Secondary toAcute Isolated Sphenoid Sinusitis.

Cavernous sinus thrombophlebitis may occur as a complication of infectious and noninfectious processes. Septic thrombosis of the cavernous sinuses is a serious life threatening condition, and most commonly follows midfacial skin infections due to Staphylococcus aureus. Isolated sphenoid sinusitis is a rare and elusive clinical problem, and it is an uncommon cause of cavernous sinus thrombosis. We report a case of cavernous sinus thrombophlebitis of spheoid sinusitis origin. A good result has been achieved with immediate medical measures and surgical drainage.

2.Risk Factors of Retinopathy of Prematurity.

In order to investigate the risk factors of retinopathy of prematurity, we retrospectively reviewed the medical records of 153 premature badies alive and born in our hospital between October 1994 and January 1997. We performed analysis on birth weight, gestational age, duration of oxygen therapy, duration of oxygen therapy more than 40 percent, maxinmum and mean value of oxygen pressute and mean value of carbon dioxide pressure from arterial blood gas analysis in the first 2 weeks, the presence of multiple birth, surfactant therapy, corticosteroid therapy, and intraventricular hemorrhage. In univatiate analysis, birth weight, duration of oxygen therapy, value of oxygen pressure and mean value of carbon dioxide pressure from arterial blood gas analysis in the first 2 weeks, surfactant therapy, and corticosteroid therapy were statistically significant(p<0.05). Howeverm, in stepwise logistic regression analysis to correct the correlations in multiple variables, only birth weight and duration of oxygen therapy were significant risk factors in 95% confidence interval( 0.05, p<0.05). Birth weight, duration of oxygen therapy, and maximum value of oxygen pressure from arterial blood gas analysis in the first 2 weeks were significant risk factors in 90% confidence interval( 0.1, p<0.1).

3.Wernicke Encephalopathy Associated with Acute Wet Beriberi.

Wernicke encephalopathy is usually accompanied with peripheral neuropathy, known as dry beriberi. In contrast, wet beriberi presenting as cardiovascular symptoms rarely occurs. The acute type of wet beriberi can be fatal, if untreated quickly. It is reported that the cerebellar vermis has a role of the coordination and control of cardiovascular and autonomic reflex activities. We report a 58-year-old man showing acute wet beriberi in Wernicke encephalopathy with cerebellar vermis lesion.

4.Clinical characteristics of CHARGE syndrome.

CHARGE syndrome, first described by Pagon, was named for its six major clinical features. They are: coloboma of the eye, heart defects, atresia of the choanae, retarded growth and development including CNS anomalies, genital hypoplasia and/or urinary tract anomalies, and ear anomalies and/or hearing loss. We experienced three cases of CHARGE syndrome who displayed ocular coloboma, heart defects, retarded growth and development, and external ear anomalies, and we also review the previously reported literature concerning CHARGE syndrome.

6.Simulation of the Effects of Long-term Implantation of Biventricular Assist Device on the Hemodynamic Parameters in Heart Failure.

BACKGROUND AND OBJECTIVES: Ventricular assist device(VAD) was developed for the bridge to cardiac transplantation, but the current research trends proceed to the purpose of bridge to cardiac recovery. We investigated the effects of long-term VAD implantation on the hemodynamic parameters related to the prognosis of heart failure by simulation to provide the preclinical and clinical applicability. MATERIAL AND METHODS: A moving-actuator type artificial heart developed by Seoul National University Artificial Heart Laboratory was used as a model of biventricular assist device. We set initial values of hemodynamic parameters according to the guideline of VAD implantation, and performed simulation of the change of hemodynamic variables related to successful device weaning and the prognosis of heart failure. RESULTS: Cardiac indices (CIs) at 1 hour and 6 months after VAD implantation were 2.98 l/min/m2 and 2.60 l/min/m2, respectively. Systolic/diastolic/mean aorta pressure were 121/84/99 mmHg at 6 months after VAD implantation. During pump-off state at 6 month, each value of hemodynamic parameters were as follows: CI 2.53 l/min/m2, pulmonary capillary wedge pressure 10 mmHg, left ventricular end-diastolic volume 105 ml, left ventricular ejection fraction 0.58, mean aorta pressure 84 mmHg, end-systolic wall stress 108 kdyn/cm2. Peak rate of change of power(peak dPWR(t)/dt) was 5.62x108 dyneXcm/s2 after 6-month VAD implantation. In a real VAD-implanted patient, simulation data were partly compatible with real hemodynamic data, especially in the aspects of predicting VAD weaning. CONCLUSION:Long-term VAD implantation partially improved the values of hemodynamic parameters related to the prognosis, and this simulation results will provide the basic concept and applicability of clinical trial for end-stage heart failure.

7.Positive Rate of HCV RNA in Anti-HCV Immunoblot Indeterminate Samples from Blood Donors and Analysis of Reactivity of Enzyme Immunoassay Kits to Indeterminate Samples.

BACKGROUND: Positive rate of HCV RNA in anti-HCV immunoblot indeterminate samples from Korean blood donors was investigated in this study. Futhermore, reactivity of enzyme immunoassay kits was evaluated according to patterns of band reactivity and RNA positivity of indeterminate samples. METHODS: HCV RNA was tested from a total of 180 samples that had been repeatedly reactive to LG HCD 3.0 or DONG-A HCV 3.0 and further confirmed to be indeterminate by LG HCD CONFIRM at 14 Korean Red Cross Blood Centers in 1997. Correlation between HCV RNA and reactivity to each recombinant antigens was analysed. Three kinds of EIA kits such as HCD 3.0, DONG-A HCV 3.0 and Cobas Core Anti-HCV EIA kits were tested of their reactivity to 56 samples that had shown single band positivity on LG HCD CONFIRM. RESLUTS: HCV RNA was detected in 11 (6.1%) of 180 samples. Positivity of 897 band showed positive correlation with HCV RNA positivity (p<0.001), while Core518 showed negative correlation (p<0.001). DONG-A HCV 3.0 showed negative reaction to two samples with anti-897 specificity and HCV RNA, while LG HCD 3.0 and Cobas Core HCV showed negative reaction to one sample with anti-EIE2NS4 and HCV RNA. CONCLUSION: Positive rate of anti-HCV immunoblot indeterminate samples from Korean blood donors was 6.1%. In case of indeterminate results by LG HCD CONFIRM, positive reaction to 897 band highly suggests the presence of HCV RNA, while Core518 the absence of HCV RNA. It is highly recommended that specificity of Core518 antigen be increased in LG HCD 3.0 or LG HCD CONFIRM and sensitivity of recombinant antigen from the 3rd non-structural region of HCV genome be increased in DONG-A HCV 3.0.

8.Evaluation of Vironostika HIV Uni-Form II Ag/Ab Enzyme Immunoassay Kit for Simultaneous Detection of Antigen and Antibody of HIV.

BACKGROUND: It has been estimated that 1 unit per 493,000 blood donations could transmit HIV by infected persons in seroconversion window in America. The 4th generation enzyme immunoassay (EIA) kits which are designed to detect HIV antigen and antibody simultaneously, could reduce the period of seroconversion window and thereby increase the safety of donated bloods without additional expenses. METHODS: Sensitivity and specificity of Vironostika HIV Uni-Form II Ag/Ab (Organon Technika, Boxtel, Netherlands) were evaluated using 53 samples (11 western blot positive samples and 42 sera from BBI panel) and 391 samples (282 healthy donors, 20 anti-HCV positive sera and 89 FANA positive sera), respectively. Seroconversion window was compared with that of COBAS CORE Anti-HIV-1/HIV-2 EIA DAGS (Roche Diagnostics, Basel, Switzerland) using 6 kinds of BBI seroconversion panel. RESLUTS: Sensitivity was 100% (53/53). Aggregated specificity was 99.5% (389/391), while all the samples from the healthy donors showed negative (282/282). COBAS CORE Anti-HIV-1/HIV-2 EIA DAGS also showed 100% sensitivity (53/53) and 100% specificity (391/391). Vironostika HIV kit detected HIV-1 infection earlier about 9.8 days than COBAS CORE Anti-HIV-1/HIV-2 EIA DAGS. CONCLUSION: Sensitivity and specificity of Vironostika HIV kit were as good as those of COBAS CORE kit. Vironostika HIV kit, however, could get more safety of donated bloods than the 3rd generation EIA such as COBAS CORE Anti-HIV-1/HIV-2 EIA DAGS, in that it can reduce seroconversion window of HIV infection.

Focal subarachnoid hemorrhage occasionally presents as transient focal neurologic episodes mimicking transient ischemic attack (TIA). Unless properly diagnosed, it may aggravate cerebral hemorrhage by administering antithrombotic agents. Therefore, clinicians need to be aware that such focal subarachnoid hemorrhage sometimes cannot be detected on noncontrast computed tomography and blood-sensitive magnetic resonance imaging can detect even a small amount of hemorrhage. We describe an 85-year-old woman with focal subarachnoid hemorrhage and possible cerebral amyloid angiopathy who presented transient left arm weakness recurrently, which mimicked TIA.

10.Brain Hypometabolism in Cerebellar Ataxia, Neuropathy, Vestibular Areflexia Syndrome

Cerebellar ataxia with neuropathy and vestibular areflexia syndrome (CANVAS) is a slowing progressive ataxic disorder characterized by bilateral vestibulopathy, cerebellar ataxia and somatosensory impairment. Autonomic dysfunction is recently considered as a core feature in CANVAS in addition to these symptoms. In most cases, patients with CANVAS show cerebellar atrophy in brain imaging, but some cases show minimal or no atrophy of cerebellum. Brain (18F)-fluoro-2-deoxy-D-glucose positron emission tomography (¹⁸F-FDG PET) study can be a complimentary tool to diagnosis CANVAS in cases of no structural abnormality such as cerebellar atrophy. Hereby, we present a case of CANVAS with minimal atrophy of cerebellum but showing a prominent hypometabolism in cerebellum, thalamus and posterior cingulate cortex in ¹⁸F-FDG PET.
