1.Treatment of Unstable Intertrochanteric Fracture

Trochanteric fractures are seen in elderly patients and have high mortality and morbidity. Strong internal fixation and early ambulation is the best way to treat fractures. For this purpose, recently, compression sliding screw plate became popular. The surgical procedure of open reduction and internal fixation were carried on 36 intertrochanteric fractures at Depertment of Orthopedic Surgery, Seoul National University Hospital, from Jan. 1972 to Dec. 1977 and the following results were obtained. 1. Of 36 cases of intertrochanteric fracture, 20 cases were in man, with the peak age at the 5th decade, and 16 cases in woman mostly over the 7th decade. 2. Most common causes were fall or traffic accidents in man and fall in woman. 3. Interval from injury to operation was less than 1 week usually. 4. Unstable fractures were 86% and of these, Tronzo Type III was the most common. 5. The trend of use of internal fixation device was from S-P plate and screw, and Jewett nail to sliding compression plate and screw. 6. Reduction of medial displacement with Dimon-Hughston method was used commonly in Jewett nailing and anatomical reduction in sliding compression plate and screw. 7. Post operative cast immobilization were applied in a few cases of under 50 years old, Other-wise early ambulation were carried out. 8. Post operative complications were encountered in 8 cases (22%), but none were fatal.

The human hand is a highly sophisticated tool of sensibility and prehension, and the reconstruction of injured hands are essential to not only reconstruct of hand function but also reconstruct of body. Seven cases of tendocutaneous flap, a case of osteocutaneous flap, nine cases of thumb reconstruction with a free neurovascular wrap-around flap from the big toe, two cases of second toe-to-index trander, and a case of left ring finger to right index finger transfer were performed at the Department of Orthopedic Surgery of Hanyang University Hospital, and satisfactory results were obtained both comsmetically and functionally.

4.Two human cases of tick bite caused by Ixodes nipponensis.

We report two human cases of tick bite. A 63-year-old male had a pruritic pea-sized brownish nodule on the left popliteal area. Another 41-year-old male had an asymptomatic bean-sized black nodule in the pubic area. The ticks were identified as Ixodes nipponensis, which are the 18th and the 19th cases in Korea.

Steroid cell tumors, not otherwise specified, are infrequently encountered ovarian neoplasms, which constitute <0.1% of all ovarian tumors. Most of these tumors are unilateral, and almost one-third of all cases are reportedly malignant. However, because most of these tumors are diagnosed in the early stage, and do not recur or metastasize, little is known about their response to therapies such as chemotherapy or radiation. Here, we present a rare case of recurrent steroid cell tumor, not otherwise specified that showed a complete response after debulking surgery, radiofrequency ablation, and adjuvant chemotherapy.

7.Extrahepatic Metastasis of Peripheral Cholangiocarcinoma: CT Findings.

PURPOSE: To evaluate the extrahepatic spread of peripheral cholangiocarcinoma with CT. MATERIALS AND METHODS: We retrospectively reviewed CT findings of extrahepatic spread in thirty-six patients with peripheral cholangiocarcinoma confirmed adenocarcinoma at surgery(n=5) or percutaneous biopsy(n--31) without primary leion in other organs, and analysed the frequency and distribution of enlarged lymph nodes, direct invasion of adjacent solid organs, and thrombosis of major vessels. RESULTS: Among the 36 cases, enlarged metastatic lymph node was noted in twenty-four(66.7%), direct invasion into adjacent organs in six(16.7%), portal and inferior vena caval thrombosis in five(13.9%). Enlarged lymph nodes were seen in hepatic nodes(n=16, 44.4%), portocaval nodes(n=15, 41.7%), interaortocaval nodes (n=10, 27.8) and celiac nodes in two(5.6%). Direct invasion was detected in the greater omentum(n=6), gallbladder(n:3) and stomach(n=2). Portal vein thrombosis(n:5) and thrombosis of inferior vena cava(n=2) were noted. CONCLUSION: Lymph node metastasis was seen in 67% in patients with peripheral cholangiocarcinoma. Direct invasion to adjacent organs and thrombosis in portal vein or inferior vena cava were occasionally associated.

10.Superior Mesenteric Artery Syndrome: Report of three cases.

Superior mesenteric artery syndrome is a rare clinical disease in Korea. The authors experienced three cases of the superior mesenteric artery syndrome. All of patients complained of continuos bilious vomiting, epigastric discomfort, epigastric fullness, and weight loss. The superior mesenteric artery syndrome was diagnosed preoperative, by using physical examination, gastrofiberscopy, upper gastrointestinal series, abdominal computerized tomography (CT), and spiral CT angiogram. We performed a Roux-en-Y duodenojejunostomy. We measured the angle between the aorta and that superior mesenteric artery by using a spiral CT angiogram. The angles were 10o, 11o, and 11o. Postoperatively they were improved. A spiral CT angiogram was a noninvasive method of diagnosing the superior mesenteric artery syndrome compared with a femoral angiography. The Roux-en-Y duodenojejunostomy was the proper method of treatment for the superior mesenteric artery syndrome.
