2.Clinical Observation of External Dacryocystorhinostomy.

The author performed dacryocystorhinostomy on 31 cases which were diagnosed of obstruction of the nasolacrimal duct and the chronic dacryocystitis from February, 1976. to July, 1978. A modified Dupuy-Dutemps method was applied as a surgical technique. To simplify the operation, the bony opening was created by Niedermeier-Weingeist trephine and no suture was used to unite the posterior flaps but instead, gelfoam was left in the newly created nasolacrimal channel. Thus the anterior flap of sac was sutured to the anterior border of the periosteum. Follow up study was done over 6 months after discharge from the hospital. The resu1ts of this observation were as fo1low: 1. The sex distribution showed a preponderance in female, the ratio ma1e to ferna1e being 1:2. 2. The age of the caseg ranged from 4 years of age to 78 years of age. The majority of the cases were from 40 to 5O years old (33.3 %). 3. Postoperative complications were few, but they consisted of mild nasal bleeding (1 case). fistula formation (1 case), abscess of skin incision (1 case), and granu1ation tissue formation (1 case). These complications could be easily treated by usual treatment and reoperation. 4. The success rate by auther's techinque was 93.5 %. The cause of 2 failures were organized granulation tissue and atrophic change of the lacrimal sac, the former failure wag treated by reoperation. In conclusion, this procedure is technically easy and the operating time is short within the range of any ophtha1mic surgeon. I am convinced that dacryocystorhinostomy is the best treatment of tearing and chronic suppuration of the lacrimal sac due to obstruction of the nasolacrimal duct, and is successful in a high percentage.

3.The Reconstruction of Injured Hands by Microsurgery

The human hand is a highly sophisticated tool of sensibility and prehension, and the reconstruction of injured hands are essential to not only reconstruct of hand function but also reconstruct of body. Seven cases of tendocutaneous flap, a case of osteocutaneous flap, nine cases of thumb reconstruction with a free neurovascular wrap-around flap from the big toe, two cases of second toe-to-index trander, and a case of left ring finger to right index finger transfer were performed at the Department of Orthopedic Surgery of Hanyang University Hospital, and satisfactory results were obtained both comsmetically and functionally.

5.Thyroid Acropachy.

Thyroid acropachy is a rare complication of Graves' disease, manifested by clubbing of the terminal phalanges, periosteal new bone formation and overlying soft tissue swelling, It may occur when the patient is hypothyroid, euthyroid or hyperthyroid. In most cases, it is a part of the syndrome, including exophthalmos and/or pretibial myxedema. The authors have experienced one case of thyroid acropachy and report with a review of the literature review. The patient a 56-year-old female with a characteristic feathery new bone formation on the medial side of the shaft of the left first metatarsal bone and overlying soft tissue swelling. However, there was no pretibial myxedema and clubbing of fingers. She was hypothyroid and treated with systemic corticosteroid for mild pain and persistent swelling. The treatment had temporarily improved the patient's condition.

6.A Case of Meibomian Gland Adenocarcinoma.

Adenocarcinoma of the Meibomian gland is very rare but lethal tumors. Clinically, most of the time sebaceous gland carcinoma is mistaken for a chalazion. Any recurrent chalazion should be submitted for histologic study, and any chronic, recalcitrant, atypical blepharitis should be biopsied. The treatment of the choice is excisional biopsy and followed by radiation therapy. A 30 year old Korean male had a hard, nontender mass on the left upper lid which had recurred after chalazion surgery. The tumor was removed surgically as repair with modified Wheeler direct full halving procedure, and found to be a adenocarcinoma of the Meibomian gland histopathologically. There was no evidence of recurrence and metastasis to remote site a year after surgery with radiation therapy.

7.A Histologic Study of Filtering Wounds After Fistulizing Surgery.

When a glaucoma filtering operation fails, the causes may be categorized as intraocular, scleral, or extraocular. Some authors have implicated that extraocular causes of failure are due to closure of the external opening by connective tissue. This histologic investigation was undertaken to examine the surgical wounds of 8 experimental subjects including 4 albino and 4 pigmented rabbits. Under general anesthesia by pentothal sodium, a single punch sclerectomy was performed on each right eye. A trabeculectomy was performed on the left eye. Seven and ten days following surgery, globes were removed and immediately fixed in Karnovsky's fixation solution. Following fixation, globes were bisected through surgical wounds, and one-half was studied using standard light microscopy. The other one-half specimens were blocked and processed for transmission electron microscopy. The results of the evaluations were as follows: 1. Full thickness sclerectomy wounds healed by ingrowth of fibroblasts from subconjunctival and episcleral tissue, but trabeculectomy wounds showed no communication with extrascleral tissue. 2. Quantitatively, proliferation of fibroblasts were more exuberant in the sclerectomied eyes compared to trabeculectomies. 3. There was no evidence of significant difference between albino and pigmented rabbits. 4. The granulation tissue composed mainly of fibroblasts resembles smooth muscle cell(myofibroblasts) and extracellular collagen constituents. Ultrastructurally, these cells showed massive bundles of intracytoplasmic microfilaments with scattered dense bodies, wrinkled and folded nuclei indicative cellular contraction, and abundant cisternae of rough endoplasmic reticulum informative activity of cellular proliferation. From those results, author conclude that smooth muscle antagonists may inhibit the proliferation of myofibroblasts after glaucoma filtering operations.

8.A Case of Surgical Repair of Ruptured Filtering Bleb After Filtering Operations.

Recently we have experienced a case of the ruptured filtering bleb after Posterior lip sclerectomy, which occured in the right eye of a 36-year old Korean male. Sudden blurred vision and tearing in the right eye after severe rubbing, were his chief complaints prior to his visit to hospital. Visual acuity waS 0.2 (0.9X + 0.75D. sph.) in the right eye and the right eyeball was soft with a flat of anterior chamber. A leaking of aqueous onto the surface of the right eye was observed, when Fluorescein(Seidel Test) was used. First, we applied a pressure bandage for about 36 hours, but the leaking of filtering bleb was continued. So, Surgical repair was performed with postoperative care. 5 months later after surgical repair by conjunctiva alone, or combined with its attached undissected Tenon's fascia, filtering bleb was reconstituted and well-functioning.

9.A Case of Steroid-induced Glaucoma.

The prolonged use of steroids, especially the topical administration of ccrticosteroid, will causes an intraocular pressure(IOP) devation in some patients with clinical finding that typically resemble primary open-angle glaucoma. The authors experienced a case of steroid-induced glaucoma following systemic and topical use of dexamethasone for seborrheic dermatitis of both eyelids 3 years previously. During the past few months patient had noted complete lloss of vision in right eye and decrease of vision considerably, in left eye and a trabeculectomy was done on the left eye. We report a case of steroid-induced glaucoma and the literatures were reviewed briefly.

10.Clavicula Pro Humero Procedure for an Osteosarcoma in the Proximal Humerus: A case report.

Osteosarcoma is one of the most common malignant bone tumors except for multiple myeloma. The current managements of the osteosarcoma patients include preoperative chemotherapy, definitive operation and postoperative chemotherapy with tremendous improved oncologic results by lots of Orthopedic Oncologists. The definitive operations are either limb-salvage operations or amputation. Limb-salvage operations include autograft, allograft, tumor prosthesis, or its combination. However, we have a difficulty to use allograft due to Korean traditonal customs. Authors managed the 6 year-old female patient with the osteosarcoma on the proximal metaphysis of the right humerus by wide-margin resection and reconstruction with a clavicula pro humero procedure with an excellent result, which technique was devised and developed by W. Winkelmann in Germany.
