1.A Case of Granular Cell Tumor.

We report a case of granular cell tumor in a 62-year- old female. She had a 1. 5 cm sized, asymptomatic, firm nodule with central depression on the lateral aspect cf the left orbit. Histologically, the dermis had many large, pale tumor cells arranged in clusters and strands. The cells had a distinct cellular membrane and a pale cytoplasm filled with faintly eosiinophilic,coarse granules. The characteristic faint granules within the tumor cells are PAS-positive and diastase-resistant. The overlying epidermiis showed pseudocarcinomatous hyperplasia. We treated it with total excision.

2.Changes in Blood Superoxide Dismutase Activities after Alcohol Withdrawal.

OBJECTIVES: The aims of this study were to evaluate changes in plasma superoxide dismutase(SOD) activities in alcohol depedence, to fine out variables to influence on the SOD activities, and finally to identify the correlation of SOD activities with the alcohol-associated cognitive disorders. METHODS: For 24 male alcoholics and 21 healthy male controls, plasma SOD activities were measured by spectrophotometry on 1-2 wks after alcohol withdrawal. Structured interviews and laboratory tests were also performed. RESULTS: 1) Upon comparing SOD activities between controls and alcoholics, the SOD activities were significantly(p<0.01) lower in alcoholics(0.308+/-0.140 units/mL) than in healthy controls(0.313+/-0.086 units/mL). 2) Upon comparing SOD activities according to the presence of alcohol-related cognitive disorders, the SOD activities were significantly(p<0.05) lower in alcoholics with cognitive disorders(0.247+/-0.049 units/mL) than in alcoholics without cognitive disorders(0.317+/-0.148 units/mL). 3) Upon comparing SOD activities according to the presence of alcoholic polyneuropathy or alcohol withdrawal seizure, the SOD activities showed no significant differences between alcoholics with polyneuropathy or epilepsy and those without. 4) Upon analyzing variables influencing on the SOD activities in alcoholics, the SOD activities had the negative correlation with hemoglobin(gamma=-0.433) and severity of alcohol withdrawal symptoms(gamma=-0.375). 5) Upon comparing variables according to the presence of alcohol-related cognitive disorders, the occurrence of alcoholic polyneuropathy(p<0.05) and blood phosphorus concentrations(p<0.01) were significantly higher in alcoholics with cognitive disorders than those without. 6) Upon analyzing an association between SOD activities and variables in alcoholics with cognitive disorders, the SOD activities were positively correlated with the onset age(gamma=0.995), and negatively correlated with the severity of alcohol withdrawal symptoms(gamma=-0.996). CONCLUSIONS: Lower SOD activities in alcohol dependence suggested alcohol-associated cognitive disorders and alcohol withdrawal symptoms might be caused by oxidative stress.

5.A Comparison of Level of the Injury, Sacral Cord Sign and Urodynamic Testing in the Evaluation of the Patients with Spinal Cord Injury.

To define if the signs of sacral cord involvement have any predictive values in the behavior of bladder and sphincter function after spinal cord injury, we analysed results of neurologic signs and urodynamic studies from 45 patients with spinal cord injuries. Patients were classified based on the anatomical level (suprasacral vs. infrasacral), and the presence or absence of sacral cord sign (SCS) (bulbocavemous reflex latency time, perineal sense, anal sphincter tone). Urodynamic findings were classified as either detrusor hyperreflexia (DH), detrusor sphincter dyssynergia (DSD), detrusor areflexia (DA) or normal. Results were as follows; 1) Of the 15 suprasacral cord lesioned patients 6 (40%) had DA, of the 30 infrasacral cord lesioned patients 9 (20%) had either DA or DH with DSD. 2) SCS was positive in 16 out of 30 infrasacral, and in 3 out of 15 suprasacral cord lesioned patients. Of the 26 SCS negative patients, suprasacral cord lesioned patients comprised 46%. 3) Of the 30 infrasacral lesioned patients, 21 had DA and 16 had positive SCS. Incidence of positive SCS in the patients with DA was 72%. 4) Incidence of negative SCS with DH or DH+DSD was 100% in suprasacral lesion. But of the 15 suprasacral lesioned patients, 6 had DA and 3 had positive SCS. 5) In all levels of the injury, positive predictive value for the DA in positive SCS was 95%. However, negative predictive value for DA or DA+DSD in negative SCS was only 62%. These results indicate that there were poor correlation between the level of the cord injury and types of urodynamic abnormalities. Also, the correlation of the SCS with anatomical level of the injury was relatively poor. Correlation of the Positive SCS with DA was very significant for both levels of the cord injury; whereas there were poor correlation of the negative SCS with DH or DH+DSD. In conclusion, the positive SCS in itself are thought to be valuable tool in predicting infrasacral lesion and/or detrusor areflexia However, the results of negative SCS may not exclude infrasacral lesion or detrusor areflexia completely. Thus, combination of the sacral cord sign and results of urodynamic evaluation will provide a more precise diagnosis and treatment plan for the patients of spinal cord lesions.

6.Primary Cutaneous B Cell Lymphoma.

A 7-year-old girl had a 7 × 6cm sized dark red colored tumor on the left elbow and several erythematous macules on the face, right upper arm, and both thighs. Her general health was good and all laboratory findings were within normal limit. Histopathologic examination revealed an infiltration of atypical mononuclear cells in the dermis and subcutis. L26 and leukocyte common antigen were positively stained in an immunohistochemical study. According to clinical, histological and immunohistochemical findings, we diagnosed the disease as primary cutaneous B cell lymphoma. The patient's skin lesions markedly improved after a surgical excision and chemotherapy.

7.Three Cases of Papular Elastorrhexis.

Papular elastorrhexis is a connective tissue nevus that occurs in the second decade of life as asymptomatic small white-creamy papules scattered over the chest, shoulder, abdomen, or back Histopathologically, there is a decrease in elastic fibers with focal homogenization of collagen. Recognition of this entity is important to prevent use of unnecessary diagnostic procedures and therapy. We present three cases of papular elastorrhexis. All of them occurred in children under the age 10 years.
