1.The Effects of Aerobic Exercise Program on BMI, Mood States, Psychiatric Symptoms in Male Patients with Chronic Schizophrenia.

PURPOSE: The purpose of this study was to examine the effects of an aerobic exercise program on body mass index, mood state, and psychiatric symptoms in men with chronic schizophrenia. METHODS: Non-equivalent control group pretest and posttest design was used. The participants were men with chronic schizophrenia hospitalized in N mental hospital located in N city (29 in the experimental group and 30 in the control group). The program was provided 3 times a week for 8 weeks, 24 sessions each lasting 45~60 minutes. Effects of this program were evaluated using BMI, K-POMS-B, PANSS. The data were analyzed using percentage, chi2-test, Kolmogorov-Smirnov test, paired t-test, t-test, and ANCOVA with SPSS 12.0 version. RESULTS: After the aerobic exercise program, statistically significant decreases in BMI, depression, fatigue score of the K-POMS, and psychiatric symptoms were found for the experimental group. CONCLUSION: These findings suggest that aerobic exercise programs are an effective psychiatric nursing intervention to decrease BMI, general psychiatric symptoms and fatigue, and also to improve mood states. Therefore aerobic exercise program are suggested as a potential approach for reducing re-hospitalization and facilitating social readjustment for patients with schizophrenia.

2.Radiological Study of Interphalangeal Sesamoid Bones on Hallux in Korean Subjects.

PURPOSE: Frequency of sesamoid bone on hallucal interphalangeal joint has been described to be low probability in orthopaedic and anatomical literature. We have, however, experienced two cases of interphalangeal joint dislocation giving difficulty to usual manipulative reduction because of presence of sesamoid bone recently. In order to ascertain existence of sesamoid bone on interphalangeal joint of hallux in Korean adults, radiological study have been performed with feet of patients MATERIALS AND METHODS: Between May 2003 and October 2006, 974 patients with 1098 radiographs of feet which were reached skeletal maturity over 18-year-old were examined. Unilateral or bilateral anteroposterior, lateral and oblique radiographs were observed by one same person and presence was recorded if there was sesamoid in films. Distance of long and short axes were measured in lateral view and cases of two sesamoids in interphalangeal joint were recorded. Statistical differences between left and right side or between men and women were evaluated by chi-square test. RESULTS: Frequency of sesamoid was 980 cases (89.3%) and no occurrence in 118 cases (10.7%). Two sesamoids were observed in 3 cases. Average distance of long axis was 4.9 mm (range, 0.5-11.4) and average distance of short axis was 3.5 mm (range, 0.3-9.3). Unilateral sesamoid was observed in 7 patients (5.6%), bilateral absence was 7 patients (5.6%) and bilateral sesamoids in 110 patients (88.8%) out of 124 patients who took bilateral feet radiographs. Men has less frequency than women significantly (p=0.014) while there was no significant difference in frequency according to side(p>0.05). CONCLUSION: Sesamoid bone was seen in 980 feet (89.3%) out of 1098 normal Korean radiological studies of feet. We report 3 cases of two seamoids which was extremely rarely reported in literature. Korean frequency is similar with Japanese, but much higher than Caucasians and black Africans.

5.Treatment of Lateral Malleolar Fractures using Minimally Invasive Plate Osteosynthesis Technique.

PURPOSE: To evaluate the efficacy and complication of minimally invasive plate osteosynthesis (MIPO) technique for lateral malleolar fracture. MATERIALS AND METHODS: From January 2001 to December 2003, we had treated 13 cases of lateral malleolar fracture (AO-OTA classification, type A: 4, type B: 9) by MIPO technique and followed them up more than 1 year. Operation time, union time, radiologic alignment, range of motion of the ankle joint, functional results according to criteria of Meyer and complication were evaluated. RESULTS: After the final follow-up, all the fractures were healed without any second procedure, full-weight bearing ambulation was started in average 10 weeks. Any malunion was not observed by the inadequate bending of plate. Two patients felt a discomfort of ankle because the location of plate was lower than the tip of the lateral malleolus, but all the patients had excellent or satisfactory ankle functions. No deep infection or soft tissue compromise were observed at the last follow up. CONCLUSION: Minimally invasive plate osteosynthesis technique is safe and worthwhile method in management of lateral malleous fractures while avoiding the complications associated with conventional open plating methods.

6.A Pathology-proven Case of Schilder's Disease.

Schilder's disease or myelinoclastic diffuse sclerosis (MDS) is a rare variant of multiple sclerosis. We report a 66-year-old woman with progressive motor weakness and diffuse white matter degeneration on MRI, which satisfies the Poser's restrictive criteria of MDS. As previously reported cases of probable Schilder's disease did not meet the criteria correctly, we consider our patient is the first pathology-proven case of MDS in Korea.

7.Comparison of Bone Mineral Density in Elderly Patients according to Presence of Intertrochanteric Fracture.

PURPOSE: To analyze difference in bone mineral density (BMD) between intertrochanteric fracture and control group and to explore the predictive value of BMD for intertrochanteric fracture. MATERIALS AND METHODS: 57 patients who were over 60-year-old with intertrochanteric fracture were examined. For control group, 110 patients who did not have any fracture were selected. Dual energy X-ray absorptiometry was studied at 1, 2, 3, 4 lumbar vertebrae, femoral neck, trochanter and Ward's triangle. BMD was compared at each site between two groups statistically. RESULTS: Fracture group consisted of 16 male, 41 female and was average 70.8 year old. Control group consisted of 21 male, 89 female and was average 68.1 year old. There was no differences in sex and age between two groups (p>0.05). BMD of L1, L2 and mean lumbar area were significantly less in fracture group than control group (p<0.05). There was no difference between two groups in BMD of another sites (p>0.05). CONCLUSION: BMD of L1, L2 and mean lumbar area in fracture group had lower value significantly, but had no differences between two groups at another sites. BMD of L1, L2 and mean lumbar area might be used as the most sensitive predictive indicator for risk of osteoporotic fractures including intertrochanteric fracture in elderly patient.

8.Relationship Between the Level of Methicillin Resistance and mecA, mecI, femA Genes Genes in Staphylococci.

BACKGROUND: About 60~70% of hospital isolates of staphylococci are resistant to methicillin. The level of resistance varies from low to high depending upon the genetic background of the strains. The purpose of this study was twofold : (i) to understand the relationship between beta-lactamase and methicillin-resistance genes(mecA, mecI, mecR1, femA) and the level of resistance and (ii) to survey the distribution of mec regulator genes(mec I, mecR1) among methicillin-resistant staphylococci. METHODS: Eighty-three isolates of Staphylococus aureus and 29 of coagulase-negative staphylococci(CNS) at Catholic University Hospital were examined. The level of methicillin resistance was studied using disk diffusion test and agar dilution test. MecA, mecI, mecR1, and femA genes detected by polymerase chain reaction. RESULTS: beta-lactamase production was significantly high in S. aureus and CNS isolates with low-level resistance. MecA and mecR1 genes amplification correlated with the level of resistance in S. aureus and CNS isolates. There was no correlation between the level of resistance and mecI and fem A genes amplification in S. aureus and CNS isolates. Methicillin- resistant S. aureus isolates showed more variety in mec regulator region than methicillin-resistant CNS isolates. CONCLUSION: From this study, we conclude that mecR1 gene could be considered as one of the important factors influencing the level of methicillin resistance in staphylococcal strains.

9.Squamous Cell Carcinoma of Temporal Bone: Four Case Reports.

We report the CT findings of four cases of squamous cell carcinoma, paying special attention to the epicenter of the lesion and the pattern of bony destruction. All four patients had a past history of chronic otitis media. Squamous cell carcinoma affected mainly the hypotympanum and inferior wall of the external auditary canal, and in all cases revealed an irregular pattern of bony destruction. Irregular destruction of the tegmen tympani occurred in two cases. In cases of squamous cell carcinoma, CT findings suggesting involvement of the promontary are usually noted.

10.Prevalence of Thyrotoxicosis and Hypothyroidism in the Subjects for Health Check-Up.

BACKGROUND: The prevalence of ovat hyperthyroidism ar hypothyroidism has been estimated up to 5% in the general populatian. Subclinical hyperthyroidism and subclinical hypothyroidism have pevalences of approximately 1% and 6%, ectively. The prevalence of hypothyroidism may be associated with excessive intake of iodine in iodine sufficient areas. Therefore, we assumed the prevalence of thyroid dysfunction in Karea might be different from those af Western cauntries. However, thete have been no surveys to examine the prevalence of thyroid dysfunction in Karea. We performed the study to investigate the prevalence of thyrotoxicosis and hypothyroidism in Korean adults. METHODS: This study was performed in 15019 subjects (8275 men, 6744 women; between 17 and 87 years of age) visited in health promotion center of Samsung Medical Center for 12 months in 1996. Serum T3, T4, and TSH concentrations were measured with RIA or IRMA using commercial kits. History of thyroid dysfunction and current medication were obtained from medical records. The criteria for thyrotoxicosis were TSH level below than 0.30 mU/L and increased T3 or T4 levels (T3 > 3.1 nmol/L or T4 > 152 nmol/L). Patients who had TSH level above than 5.0 mU/L and T4 level below than 77 nmol/L met the criteria for hypothyroidism. RESULTS: The prevalence of thyrotoxicosis was 5.5/1000 population (men 3.6/1000, women 7.7/1000) with peak prevalence in fifth decade. The prevalence of previously undiagnosed thyrotoxicosis was 4.0/1000 (men 2.9/1000, women 5.3/1000). The prevalence of hypothyroidism was 2.8/1000 population (men 1.1/1000, women 4.9/1000) with peak prevalence in seventh decade. The prevalence of previously undiagnosed hypothyroidism was 1.6/1000 (men 0.6/1000, women 2.S/1000). The prevalence of subclinical thyrotoxicosis was 12.4/1000 population (men 11.8/1000, women 13.0/1000). The prevalence of subclinical hypothyroidism was 18.2/1000 population (men 11.2/1000, women 26.7/1000) which frequency was increased with age. CONCLUSION: Although the prevalence of thyrotoxicosis and hypothyroidism was not significantly different from those of other countries, it was lower than expected and female preponderance is not significant. The prevalence of subclinical thyrotoxicosis and subclinical hypothyroidism was lower than those of other countries. The prevalence of subclinical hypothyroidism was higher in women and old ages.
