2.Isolated Sphenoid Sinusitis with Orbital and Intracranial Complication.

Sphenoid sinus is adjacent to important intracranial structures, such as major intracranial vessels and some cranial nerves. However isolated sphenoid sinusitis is often misdiagnosed because presenting symptoms are initially subtle. Therefore, if the diagnosis of the isolated sphenoid sinusitis is delayed and complications occur, it will lead to disastrous results. We experienced a rare case in which a 51-year-old male patient developed both brain abscess and abuducens nerve palsy and report this case with a review of the case and related articles.

3.Multiple Pulmonary Benign Metastasizing Leiomyoma.

We report here a case of pulmonary benign metastasizing leiomyoma from the uterus in a 45 year old woman. The patient presented for investigation of multiple pulmonary nodules on a routine chest roentgenogram. The patient had undergone uterine myomectomy due to uterine leiomyoma 10 years earlier. We performed thoracoscopic wedge resection for definitive diagnosis. Histologically, spindle shaped smooth muscle cells appeared between collagen stroma, histology similar to that seen in uterine myoma. The tumor tissue tested positive for estrogen and progesterone receptors. The pathological findings were consistent with benign metastasizing leiomyoma. The patient was in pre-menopause. She received no specific treatment for lung tumors, and we did not found any changes in the lesions after one year follow up without any medication.

4.Statistical Classification of Alcohol Dependence by MMPI.

OBJECTS AND METHODS: The authors tried to classify 75 male patients with alcohol dependence by using cluster analysis of their MMFI data, to Identify the characteristics of each subtype classified through that process and to evaluate the discriminant validity of the classification. RESULTS: The subjects were divided into 3 subtypes by K-means cluster analysis of MMPI scores: a psychotic subtype of 6(Fa)-7(Ft)-8(Sc) profile(21.4%), a neurotic subtype of 2(D)-3(Hy)-1(Hs) profile(49.3%), a normal subtype(29.3%). But, there were no statistically significant differences of age, educational level, marital status, admission history due to alcoholic problems, number of drinking day per week, average amount of ethanol consumed per drinking occasion, age at onset of drinking and family history of alcoholism among these three subtypes. CONCLUSION: It seems that the subjects with alcohol dependence could be classified into three subtypes with statistical significance, but this statistical classification does not constitute any evidence of discriminant validity.

5.The Effect of Mometasone Furoate Cream on Skin Barrier Function in Patients with Allergic Contact Dermatitis.

BACKGROUND: It is unclear how the usage of topical steroid agents affects skin barrier function. OBJECTIVE: In order to follow up on previous research into this topic, we sought to investigate the effects of a 3-week application of topical mometasone cream on the alteration of skin barrier function. METHODS: Twenty-six patients who had been clinically diagnosed with allergic contact dermatitis were enrolled. Topical mometasone cream was applied to the skin lesions. Clinical symptoms, transepidermal water loss (TEWL), corneometer unit, and pH value were measured on the initial visit, 1 week after treatment, and 3 weeks after treatment, and their values were compared. RESULTS: Clinical symptoms showed improvement after topical mometasone cream was applied (p<0.05), but changes in TEWL, corneometer units, and pH values failed to show statistical significance (p>0.05). CONCLUSION: This study found that treatment with topical mometasone cream for 3 weeks has no effect on skin barrier function. We believe that this research will help determine the optimal duration and dosage of topical steroid agents used for treating allergic contact dermatitis.

6.A Case of Brown Tumor of the Hard Palate in Association with Primary Hyperparathyroidism.

Brown tumor, also referred as "Oteitis fibrosa cystica," is a focal lesion found within the areas of bone resorption as a result of diffused resorptive process of the bone resulting from hyperparathyroidism. Brown tumors are nonneoplastic tumors of multinucleated giant cells and fibrocellular proliferation. Brown tumors as a part of primary hyperparathyroidism are commonly multiple and occur in the ribs, clavicle, and pelvic girdle. However, brown tumor on the hard palate is extremely rare. We recently experienced a case of a 39-year old female patient with palatal brown tumor. We present this case of brown tumor that occurred after primary hyperparathyroidism with a brief review of literature.

7.Hydrogen Peroxide Production in Neutrophil after Tourniquet Release.

The use of lower extremity tourniquets for procedures of the lower leg is considered routine in orthopedic surgery, yet, lower extremity tourniquets are not benign. While the tourniquet is inflated, metabolic changes such as increased PaCO2, lactic acid, and serum potassium and decreased level of PaO2. and pH occur in the ischemic limb. Deflation of tourniquet results in release of anaerobic metabolic products during ischemia into systemic circulation. In this ischemia/reperfusion situation, oxygen free radicals could potentially be produced during the reperfusion period by several mechanisms. One of these mechanisms is release of intracellular superoxide or hydrogen peroxide by activated neutrophils in the area. These reactive oxygen species(ROS) could be a causative factor for the postreperfusion no-flow, lung injury, induction of tourniquet shock, etc, The purpose of this clinical study was to investigate the effect of tourniquet deflation on the hemodynamic changes, changes of blood gas analysis, and hydrogen peroxide production using flow cytometric analysis of fluorescent DCF(Dichlorofluorescein). Quantitative Analysis of fluorescent DCF was performed in resting and fMLP(N-formyl-methyonyl-leucyl-phenylalanine) or PMA(phorbol myristate acetate) stimuliated neutrophils. The results were as follows: 1)The hemodynamic changes (systolic and diastolic BP, pulse rate) did not show any significant difference before and after tourniquet release(P>0.05). 2)Arterial pH deceased significantly until 10min and PaC was increased significantly until Smin after toumiquet release(P<0.05). 3) Arterial PO2, bicarbonate, base excess showed no significant change before and after tour- niquet release(P>0.05). 4) Hydrogen peroxide production which was estimated by fluorescent DCF in neutrophils did not show any significant change before and after tourniquet release(P>0.05). These results indicate that tourniquet application(400mmHg, less than 2hours) could not release significant hydrogen peroxide during reperfusion period after tourniquet release.

8.Amorolfine Nail Lacquer 5 % Once Weekly in Onychomycosis of Fingers and / or Toes.

BACKGROUND: Amorolfine is a new topical antifungal drug of the morpholine class has broad spectrum fungicidal activity. Amorolfine nail lacquer 5% is a transungual delivery system which can penetrate well through the nail plate to the infected keratin and remain there for a prolonged period. OBJECTIVE: The aim of this study is to assess the efficacy and tolerability of 5% amorolfine nail lacquer given once weekly to the patients with onychomycosis. METHODS: 29 patients with onychomycosis affecting not more than 80% of the surface of nail were treated once weekly for up to 9 months with amorolfine nail lacquer 5%. Clinical and mycological examination were performed before treatment and 3, 6, 9 months after start of treatment. Final evaluation was done 3 months after the end of treatment. RESULTS: Average affected area(%) and average score of clinical signs-thickening, splitting, discoloration-were significantly decreased 3 months after start of treatment. Mycological cure rate was continuously increased according to the treatment duration reaching 75.9% at 3 months after treatment. Overall efficacy assessed by the investgators were cure in 31.0% and improvement in 41.4%. Tolerability, mode and frequency of treatment, and formulation were evaluated as good or excellent by most of the patients.No systemic or local side effect was observed. CONCLUSION: Amorolfine nail lacquer 5% used once weekly up to 9 months was relatively effective and safe for the treatment of onychomycosis.

9.Relationship between Carcinoembryonic Antigen (CEA) and Pulmonary Function in Middle-aged Male Smokers.

BACKGROUND: Carcinoembryonic antigen (CEA) is a glycoprotein on cellular surface, which is highly condensed in embryonic tissue and tumor of various kinds. Previous study found out that CEA may grow with various cancer or other diseases other than cancer as well. Besides, it is widely known that smoking also influences the rise in CEA. Among the same smokers, some of them show high CEA figures in serum when others remain in normal range. There are those whose pulmonary function is not influenced by smoking when that of others are susceptible to it. Therefore, this study was undertaken with an aim to study the relationship between serum CEA and pulmonary function by investigating how the change in pulmonary function caused by smoking influences serum CEA. METHODS: From Nov, 1997 to Feb, 2001, this study carried out tests on adult male smokers ages 35 to 64 who visited a hospital located in Kang Nung city. The subjects were divided into two groups: one group of 29 subjects with high CEA with over 6.0 ng/ml with normal colon study; the other group, which is the CEA normal group, consisted of 58 subjects selected through age adjusted random sampling. Data on personal information, smoking and clinical history was collected from a questionnaire. CEA was tested using radioimmunoassay of Abott. Pulmonary function was examined using Analyzer assembly Vmax 20C from Sensormedics Company. These examinations was limited to those who have been screened not to have cancer by chest X-ray, abdominal ultrasonography, and duodenofibroscopy. RESULTS: Smoking per day for the group with high serum CEA was 1.3 pack ( 0.4 pack), which was found to be significantly higher compared to that of normal group (P<0.01). Pack-years with high serum CEA group was 32.6 13.5 which was also comparatively higher than that of the normal group with 22.4 10.9 (P<0.01). Pulmonary function test indicated that FEV1 for the group with high serum CEA was 3.0 0.5 L, which marked lower than that of the normal group with 3.4 0.5 L (P<0.05). After compensating for age and pack years, FEV1 decreased in proportion to the rise in CEA. CONCLUSION: This study has established a link between serum CEA and daily smoking, pack years, and pulmonary function and found that FEV1 was inversely proportionate to the rise in CEA regardless of corrected pack years and daily smoking. Consequently, serum CEA alone is thought to be related to the pulmonary function. Therefore, it is advised that smokers with high serum CEA need to take heed of the influence on pulmonary function.
