1.Two cases of atypical fibroxanthoma of the skin.

Atypical fibroxanthoma(AFX) occurs most on sun-exposed area of the head and neck of elderly person. It has an excellent prognosis after conservative, but complete, excision. However, because of its potential, albeit small, for metastasis, it is widely regarded as a low-grade sarcoma. We present herein two cases of atypical fibroxanthoma. The case 1 was a 86-year-old female who had a small egg-sized, dome shaped nodule with eroive surface on the left cheek. The other case was a 60-year-old male who had solitry bean-sized, nodulo-ulcerative lesion on the vertex. Immunohistochemical studies revealed positive reaction for vimntin and a-antichymo trypsin. These patients have received completely total surgical ecis on and remained free of recurrence for a period of about, 2 years follow up.

2.A Study of Personality Disorders in Alcoholics.

OBJECTIVES: The purpose of this study was to examine the prevalence of personality disorders among non-alcoholics and hospitalized alcoholics and the extent and nature of the overlap between different axis II disorders. METHODS: Subjects were 87 non-alcoholics in the general population group and 94 alcoholics admitted to psychiatric hospital. Alcohol dependence was diagnosed according to DSM-IV by the MAST, and general group and patients were assessed with the PDQ-4+. RESULTS: In the general population group, 52.3% of non-alcoholics had at least one personality disorder and the average number of personality disorders was 1.23 per person. In the alcohol group, 88.3% of the patients had at least one personality disorder and the average number of personality disorders was 4.26 per patient. CONCLUSION: In our study, there is no single "alcoholic personality" emerged.

6.A Case of Cutaneous Mucormycosis.

Mucormycosis occurs primarily in patients with severe underlying illness, especially leukemia, lymphoma, and uncontrolled diabetes mellitus. Cutaneous mucormycosis is somewhat less frequently associated with systemic illness than other forms of mucormycosis. The associated mortality is significantly less than that related to rhinocerebral form. More recent reports have shown that primary cutaneous mucormycosis has emerged as an important form of the disease. It develops where a break in the integrity of the skin has occurred as a result of surgery, bum, or other forms of trauma. We report a case of primary cutaneous mucormycosis in a healthy person without systemic illness, which was successfully treated with amphotericin B therapy for 2 weeks.

7.A Case of Aneurysmal Benign Fibrous Histiocytoma.

Aneurysmal benign fibrous histiocytoma, which is a rare variant of benign fibrous histiocytoma, usually occurs as a solitary nodule on the limbs of young to middle aged adults. The clinical diagnosis of fibrous histiocytoma is seldom considered in the differential disgnosis, which may include malignant melanoma, hemangioma, neurorfibroma, and nonspecific cyst. Histopathologically, aneurysmal benign fibrous histiocytoma is characterized by the presence of large blood-filled tissue space which is surrounded by histiocyte, fibroblast, hemosiderin pigment, and capillaries. We report a case of aneurysmal benign fibrous histiocytoma in a 15-year-old boy, who had a solitary nodular lesion on the left knee for a 2 year duration.

8.Primary Lymphoma of the Cerebellum.

Primary lymphoma is rare lesion of the intracranial neplasm. We have recently experienced a case of primary lymphoma invoving cerebellar vermis and hemisphere. The patient presented with headache, vomiting, ataxia and dysmetria. The brain CT scan and MR imaging revealed round mass lesion involving the left cerebellar hemisphere and vermis with minimal surrounding brain edema. On vertebral angiography, tumor stain appeared during the late arterial phase. Preoperative CSF analysis showed no specific abnormal findings. The CSF cytology was normal. The mass was surgically removed and the histological feature was diffuse histiocytic lymphoma. In the postperative and postradiation period, the metastasis occurred to the head of the caucate nucleus and cervical spinal cord. A case of primary lymphoma of the cerebellum is presented with review of literature.

9.A Case of Penetrating Cranial Injury by a Wooden Foreign Body.

Many objects create penetrating cranial injuries. Metallic objects and low velosity missiles are the most common offenders. Wood as wounding agent of the brain, has some characteristics. It has profound risk of infection, a significant propensity to fragmentation that is virtually undetechable on routine X-ray screening and very low atteunation value in CT scan. Because of its characteristics and rarity, we report a case of penetrating cranial injury by a wooden foreign body with review of literature.

10.A Study of Nail-splinting Technique with Flexible Plastic Tube in Ingrown Nail.

BACKGROUND: Ingrown nail is a common disorder that occurs most frequently in the great toe and gives affected patients a great discomfort. Many treatments hae been described, such as nail extraction, partial matrix phenolization and wedge excision. However, these treatments modality may lead to damage of the nail and nail fold or to frequent relapses. OBJECTIVE: We attempted to determine the effectiveness of nail-splinting technique with flexible plastic tube in ingrown nails. METHODS: Twenty-nine consecutive patients were treated and enrolled in this study. Without local anesthesia, the lateral edge of the nail plate including the spicule is splinted with a lengthwise -incised small flexible plastic tube, for example, plastic drainage portion of scalp vein set. The plastic tube retained for 2 weeks without any medication. RESULTS: Among the 29 patients, 24 patients were treated successfully without recurrence. After splinting, the patients experienced relief of pain immediately or 1~2 days later, therefore patients are able to lead ordinary lives more qvldrly. CONCLUSION: Nail-splinting technique with flexible plastic tube in ingrown nails is an effective treatment with relatively low recurrence rate. This method demands little skill and carried out in general practice. It gives an excellent cosmetic result with immediate pain relief, thus fulfilling the main requirements for the primary treatment of choice.
