2.Primary Lymphoma of the Cerebellum.

Primary lymphoma is rare lesion of the intracranial neplasm. We have recently experienced a case of primary lymphoma invoving cerebellar vermis and hemisphere. The patient presented with headache, vomiting, ataxia and dysmetria. The brain CT scan and MR imaging revealed round mass lesion involving the left cerebellar hemisphere and vermis with minimal surrounding brain edema. On vertebral angiography, tumor stain appeared during the late arterial phase. Preoperative CSF analysis showed no specific abnormal findings. The CSF cytology was normal. The mass was surgically removed and the histological feature was diffuse histiocytic lymphoma. In the postperative and postradiation period, the metastasis occurred to the head of the caucate nucleus and cervical spinal cord. A case of primary lymphoma of the cerebellum is presented with review of literature.

4.The Change of Fetal Liver Length and Liver Volume by Ultra-sonography according to Gestational Age in Normal Pregnancy.

OBJECTIVE: The measurement of liver size can be used for the diagnosis of the fetal growth abnormality (FGR, macrosomia etc.). The purpose of this study was to evaluate a mathematical relationship between the fetal liver size(liver length or volume) and the gestational age in the normal pregnancies. Brief comparisons were also tried on the base of the degree of the correlation between liver length and its volume. METHODS: We collected 54 singleton pregnancies of 20 to 36weeks of gestation for measuring fetal liver length and 57 singleton pregnancies for measuring fetal liver volume. We used Combison 530 utrasonic machine(Kreztechnik AG, Zipf, Austria). RESULTS: There was significant correlation between liver size( length and volume) and gestation age. And the liver volume better correlated with gestational age than liver length(r=0.93 : r=0.78, p<0.0001, p<0.0001). CONCLUSION: Ultrasonic measurement of fetal liver size is a reliable indicator of fetal growth, especially liver volume. Therefore these data may have a potential value for the prediction of abnormal fetal growth(FGR, macrosomia).

5.Comparison between Osteopontin Expression and Renal Function in Spontaneous Renal Failure Model.

Tubuolointerstitial inflammation and tubular injury account for most types of glomerulonephritis. The injury is characterized by an infiltration of mononuclear cells with atrophy and dilation of tubules and increased deposition of collagen in the interstitium. Despite the fact that the degree of tubulointerstitial injury in glomerular diseases may be the best predictor of overall outcome, the pathogenic mechanism by which the tubular injury develops remains unknown. Osteopontin, a highly acidic, phosphorylated, secreted glycoprotein, is up-regulated in renal cortex in many experimental models of tubulointerstitial fibrosis. In this study, we examined the expression of osteopontin in tubulointerstitium in experimental renal failure mouse, FGS/KIST. Mice were assigned three groups and sacrificed at 1 month, 2 months, and 3 months, in each. Proteinuria, GFR, the degree of tubulointerstitial inflammation, tubular atrophy, glomerulosclerosis and osteopontin expression were measured. Three-month-old group showed severely decreased GFR and marked tubulointerstitial inflammation and glomerulosclerosis compared with other groups. The expression of osteopontin increased with the severity of tubulointerstitial injury. These data suggest that osteopontin may act as a chemotactic or adhesive factor in the recruitment of the monocyte/macrophages and have a role in the pathogenesis of the tubulointerstitial injury.

7.The Outcomes of 17,744 Cervicovaginal Smears in Ihha University Hospital.

OBJECTIVE: Our purpose was to investigate the distribution of the result of 17,744 cervicovaginal smears and their pathologic results in Inha University Hospital to make a reference data. METHODS: During 28 months, May 1996 to August 1998, we performed 17,774 cervicovaginal smears and arranged them by the Bethesda system and compared them with their pathologic results. RESULTS: Among 17,744 smears, there were 93.2% of benign results, 2.9% of ASCUS, 0.9% of LSIL, 1.4% of HSIL, and 0.8% of SCC. The ASCUS/SIL ratio was 1.28. Among ASCUS, the high grade histologic outcome was composed 4.0%. Among LSIL, there were 15.7%, and HSIL 48.0%, SCC 56.1%, AGUS 9.3%, and Adenocarcinoma, the results was 69.2%. CONCLUSION: Among 17,744 smears, there were 93.2% of benign results, 2.9% of ASCUS, 0.9% of LSIL, 1.4% of HSIL, and 0.8% of SCC. The ASCUS/SIL ratio was 1.28. These results were very similar to other data and we would like to add our data to them as a reference. And the worse the smear results were, the worse the pathologic results were.

8.Expression of Heat Shock Protein 70 in Human Skin Cells as a Photoprotective Function after UV Exposure.

BACKGROUND: Human skin is exposed to various environmental stresses, such as heat, cold, and ultraviolet (UV) radiation. Heat shock proteins (HSPs) induced by temperature elevations, as a physiologic response to mediate repair mechanisms and reduce cellular damage. OBJECTIVE: The purpose of this study was to investigate the induction of HSPs in human skin cells after UV exposure. METHODS: We performed immunoblotting using a specific monoclonal antibody to the HSP70 family, one of the best-conserved stress proteins in humans, with cultured normal human keratinocytes, A431 cells, human melanocytes, SK30 cells, and human dermal fibroblasts (HDF). RESULTS: Our results indicated that high expression of HSP70 in the unstressed state was noted in epidermal cells, including normal human keratinocytes, A431 cells, human melanocytes, and SK30 cells, but epidermal cells showed no additional up-regulation of HSP70 after UV irradiation. On the other hand, HDF expressed very small amounts of HSP70 at baseline, but significantly higher amounts of HSP70 after UV exposure. CONCLUSION: These findings suggest that constitutive expression of HSP70 in epidermal cells may be an important mechanism for protection of the human epidermis from environmental stresses, such as sunlight exposure.

9.A Case Of Lateral Medullary Infarction Presenting as Isolated Dysphagia with Unilateral Vocal Fold Paralysis.

Unilateral vocal cord paralysis and dysphagia are not uncommonly seen in the ENT practice. Just less than 10% of unilateral vocal cord paralysis are caused by diseases of the central nervous system, and lateral medullary infarction (LMI) is a wellknown condition that causes dysphagia and unilateral laryngeal paralysis. However, cases of LMI with isolated dysphagia and dysphonia without other typical neurological signs have been rarely reported. We describe the case of a 67-year-old man with chronic hypertension who presented as dysphagia and unilateral vocal cord paralysis without other typical signs of LMI. He was finally diagnosed as LMI on MRI. Although cases of LMI presenting in the manner like this case are extremely rare, ENT physicians should consider LMI in differential diagnosis for this situation, especially when the patient has a previous episode or risk factors of stroke.
