3.Peripheral Blood Stem Cell Collection with an Automated Blood Cell Separator.

Peripheral blood stem cells(PBSC) were collected for autologous transplantation for the first time in Korea wish Fenwal CS-3000 Plus blood cell separator. Twenty-nine procedures were performed for 7 patients without any incidents. GM-CSF(400ug) was administered daily for 5 days before PBSC collection and the collections were performed 3-6 successive days. A total of 3.5-11.4 x 109 mononuclear cells(MNCs) were collected from 7,000mL blood processed per procedure. The number of MNCs collected was mainly dependent on the concentration of MNCs in patient's blood(r = 0.68, p<0.01). We concluded that the PBSC trans plantation program could be set up without difficulty in Korea since the Fenwal CS-3000 Plus blood cell separator is already available in many institutions and the procedure used in this study is easy to follow.

4.MRI Findings of Intracranial Cavernous IV lalformations.

PURPOSE: To analyze the variable MRI features and clinical significance of intracranial cavernous realformation. MATERIALS AND METHODS: Forty patients(mean age 35.4) with cavernous malformation were evaluated by MRI. Eleven patients were surgically confirmed. Cavernous malformations were divided into four categories on the basis of the MR imaging characteristics, especially on T2-weighted image. Type I lesion was defined as an extralesional subacute hemorrhage outside the low signal rim, type II as an intralesional hemorrhage surrounded by low signal rim, type III lesion as an intralesional thrombosis with variegated central core surrounded by low signal rim, and type IV lesion as a focal old hemorrhagic core with small low signal intensity. Type IV was further divided into IVa and IVb, whether the lesion has small iso- or hypersignal central core (IVa) or not (IVb). Follow-up MRI was evaluated in 12 patients who were managed conservatively. Follow-up intervals ranged from 2 weeks to 29 months (mean 6months). RESULTS: Total 80 lesions were detected in 40 patients. Multiple lesions were noted in 10 patients. The topography of the cavernous malformations was supratentorial in 75% and infratentorial in 23%. There were 10 lesions in type I, 15 in type 11, 21 in type III, 14 in type IVa, and 20 in type IVb. Type I lesions mainly showed mass effect and edema. Type III lesions showed minimal contrast enhancements in 7 lesions on delayed images. Type II lesions showed the characteristics of both type I and type III lesions. On follow up images, decrease in size in 5, change of type in 7, rebleeding in 2 and no change in 12 lesions were demonstrated. Hemorrhage, edema and mass effect were combined in the cases of rebleeding. On follow-up study, the estimated risk of bleeding was 32.3%/person-year and 13.7%/lesion-year. CONCLUSION: Cavernous realformations show as variable appearance, on MR imaging suggesting variable stages of evolution. The MR morphologic classification and evaluation of secondary findings are helpful to predict natural course and possibility of rebleeding of the lesion.

5.Change in Medical Care Utilization over Time in Early Years of Insurance Coverage.

The purpose of this study is to observe the pattern of change in medical care utilization over time in early years of insurance coverage. The source of data is the benefit records file of a voluntary medical insurance society for covering the four-year period, from 1982 to 1985. The measure of medical care utilization used in this study is the age-sex standardized percentage of the enrollee who have visited a physician over total analytical population during a three-month period. For six cohorts by the year of enrollment (1979-1984), the relationship between the utilization and duration of insurance coverage was examined controlling for the calender year and season. In the analysis, logistic multiple regression and residual analysis were employed. It was observed that medical care utilization rapidly increased during the early stage of insurance coverage, and after then increased at a slower rate over time to become almost stable in about twenty months.

6.A Case of Meibomian Gland Adenocarcinoma.

The clinical resemblance of carcinoma of the Meibomian gland to the relatively frequent chaiazion should be kept in mind by anyone performing eye lid surgery. A 46-year-oId Korean house wife was first visited here in May 2nd '69 with complaints of localized swelling, ulceration and dull pain on the left upper lid. She had been taken the operation for chaiazion on the same side of the left upper eye lid a year ago. Examination revealed a reddened hard, tender lesion elevating the skin on the medial one third of the left upper lid with ulceration at its center. The tarsal conjunctiva was noted yellowish patches with ulceraion extended to its margin. Local resection with full thickness containing normal tissue around he lesion of upper lid was performed and graft was made in lower lid and rotatory transplantation was given, and pathologic examination revealed adeno-carcinoma of Meibimian gland. There was no evidence of recurrence and metastasis to remote site a year after surgery.

9.Burkitt's Lymphoma Developed Acute Leukemia: A report of two cases.

Burkitt's lymphoma with a frankly leukemic picture is an extremely rare condition, and usually has a very acute course with a poor prognosis. The authors experienced two cases of Burkitt's lymphoma developed acute leukemia. The first case was 8 years-old boy who had rapidly growing large abdominal mass, which revealed typical Burkitt's lymphoma with characteristic "starrysky" pattern. The second case was 20 year old male who had not definite tumor mass. Their peripheral blood and bone marrow aspiration revealed acute leukemia with multiple vacuolated lymphoblast of Burkitt's type. We report these cases with brief review of literatures.

10.Role of Angiogenesis and Expression of Vascular Endothelial Growth Factor in Mouse Skin Carcinogenesis .

BACKGROUND: Angiogenesis is crucial for many biological processes such as embryogenesis, cyclic changes in the endometrium and wound healing. It is also critical for the growth, invasion and metastasis of solid tumors. Vascular endothelial growth factor (VEGF) acts as a mitogen for endothelial cells and is expressed by the presence of various tumor cells. The objective of this study is to evaluate if angiogenesis is involved in the mouse skin carcinogenesis and if VEGF is related to angiogenesis. METHODS: We induced premalignant and malignant lesions on mouse (BALB/c) skin using the two stage chemical carcinogenesis moedl, DMBA (7,12-dimethylbenzanthracene) initiation and TPA (tetra decanoyl-phorbol-acetate) promotion. And we analysed the microvessel densities (MVD) and expression of VEGF in various stages of premalignant and malignant lesions by immunohistochemical studies. RESULTS: Squamous papillomas, keratoacanthoma, dermatofibroma, and squamous cell carcinomas were developed in 20 weeks. There were no differences in the incidence of benign and malignant tumors between 10-week and 20-week promotion groups. There were significant increases in MVD from normal and hyperplastic skin through premalignant lesion to invasive squamous cell carcinoma (p<0.0005). But the degree of VEGF expression neither correlated with neither MVD nor the tumor groups. CONCLUSIONS: Increased angiogenesis begins from the hyperplastic stage. VEGF produced by tumor cells may not play major roles in the angiogenesis in the two stage chemical carcinogenesis model of the mouse skin.
