2.Introduction And The Current Status Of Hospital Information Systems.

The purpose of this research was to understand the current status of hospital information system(HIS) in Korea. Our main interests were when hospitals adopted each component of HIS and how they developed and managed the system. Structured questionnaires were applied to the department of each hospital in charge of managing information system. All hospitals, 276 in 1997, were included, and among them 93.5% responded. The HIS has been rapidly developed to include medical record management system, order communication system, laboratory information system, and picture archiving and communication system. The software for HIS was developed extramurally in more than half of all hospitals, and usually hardware was secured by buying than leasing them. Recently more hospitals has separated departments for His as independent units in hospital.

3.The Diagnostic Significance of 67 Gallium Lung Scan and High Resolution Computed Tomography in Patients with Pulmonary Tuberculosis.

OBJECTIVE: The activity of pulmonary tuberculosis has been usually detarmined by smear and culture of AFB in sputum and serial chest X ray. However it is sorntimes difficult to determine the activity of pulmonary tuberculosis by either. We have studied Gallium lung scan 4 HRCT in the patients with pulmonary tuberculosis to determine its activity and compared their diagnostic significance. METHODS: 67Gallium lung scan and HRCT were examined in 63 patients with pulmonary tuberculosis suspected by chest X ray and were compared each other with their possible diagnostic significance in determination of activity. The active pulmonary tuberculosis was defined by positive AFB smear and/ or culture in sputum and changes of findings in serial chest X ray. RESULTS: 1) Of 63 patients with pulmonary tuberculosis, 37 patients has been diagnosed active 9 patients were diagnosed by positive AFB smear in sputum, 20 patients by positive AFB culture in sputum and 8 patients by changes of findings in serial chest X ray. 2) In the 37 patients with active pulmonary tuberculosis, chest X ray revealed active pulmonary tuberculosis findings in 28 patients, inactive in 4 patients and undetermined in 5 patients. On the contrary, In the 26 patients with inactive pulmonary tuberculosis, the chest X ray revealed active pulmonary tuberculosis findings in 6 patients, inactive in 16 patients and undetermined in 4 patients. 3) 67Gallium lung scan showed positive uptake in 34 patients of the 37 patients with active pulmonary tuberculosis, but did not show any uptake at all in 26 patients with inactive pulmonary tuberculosis. 4) HRCT showed active findings in 36 patients of the 37 patients with pulmonary tuberculosis. And it showed active findings in 4 patients of the 26 patients with inactive pulmonary tuberculosis. CONCLUSION: The Gallium lung scan showed 82% of sensitivity and 8% of false negativity. And HRCT showed 97% of sensitivity and 15% of false positivity. In the aspect of laboratory examinations for the determination of activity in pulmonary tuberculosis the 67Gallium lung scan and the HRCT appeared useful.

4.The Impact of Order Communication System on Changes of User's Work Patterns, Adaptation and Satisfaction.

Order communication system(OCS) could change involved users' work pattern profoundly. Also adaptation to OCS and satisfaction with it influence the efficiency of management in hospital. This study applied self-administered questionnaire and individual interview to evaluate changes of users' work pattern, adaptation and satisfaction after the introduction of OCS. The survey measured the experience of computer use, user's evaluation on OCS education, degree of usage, change of work patterns, relation with co-workers or other departments, user's adaptation and satisfaction of physicians, nurses and pharmacists who worked in two university hospitals in Seoul that had introduced OCS. The major findings of this study were as follows; 1. Changes of users' work patterns were different between two hospitals. In a hospital major business time to issue and confirm order was decreased but in another hospital increased . 2. Relations of doctors with nurses were also different between two hospital. Frequency of contacts in a hospital was increased and doctor-nurse relationship grew worse, while in another hospital decreased and grew better. These situations were observed on the relationship with other departments, such as pharmacy and admission/discharge service. 3. There were no significant variables that explained user's adaptation, but again there was a difference between hospitals. 4. User's satisfaction was significantly affected by hospital and job catergories classified. Level of satisfaction of a hospital was higher than that of another, and highest in pharmacists, and the next was nurses and doctors in order. 5. There was o difference in satisfaction level by the type of hardwares, operations and accuracy between hospitals, but was difference by the contents of information, convenience of use and timeliness between hospital. The hospital factor consistently influenced users' work pattern, adaptation, and satisfaction. It implied that, if OCS was designed familiarly and conveniently for users, users' resistance will not trouble the implementation of OCS.

5.A Case of metastatic lid carcionoma of stomach cancer.

Eyelid tumors, including metastatic lid tumors, present clinically as recurrent chalazia, multicentric nodules, ul cerated lesions, diffuse swelling, or chronic blepharocon juntivitis. Lid tumors consist of benign lesions(69%) and malignant lesions(31%). Basal cell carcinoma and squa mous cell carcinoma account for 92% of the malignant lesions. Metastatic lid disease was found to be of rare occurence, accounting for 0.1% of all lid lesions and 0.3% of malignant lid lesions. We report a case of metastatic lid carcinoma of sto mach cancer. A 60-year-old man developed a painless, slow-growing nodular lesion of his right lower eyelid. Excisional biopsy revealed metastatic undifferentiated ade nocarcinoma. We believe this is the first case in Korean literature.
