Coronary restenosis is still regarded as Achilles' Hill of interventional cardiology despite relentless efforts of many investigators. Recent experimental and clinical studies have suggested that both gamma and beta radiation can be reduce restenosis after angioplasty. Currently, intracoronary brachytherapy for the prevention of restenosis has become a new evolving treatment modality in interventional cardiology. This report discusses a physical aspect of gamma and beta radiation, initial clinical results and delivery systems used in intracoronary brachytherapy. We shall take a brief overview of methods and their advantages in intracoronary brachytherapy. Future work should provide further insight for the best way of treating restenosis.

7.A Case of Burkitt's Lymphoma with Bilateral Renal Enlargement.

Burkitt's lymphoma is a distinct pathologic entity characterized as a diffuse undifferentiated malignant lymphoma of B-lymphocyte origin. We experienced a case of Burkitt's lymphoma with bilateral renal enlargement in which a 3year-old male patient was admitted with complaints of abdominal distention and facial edema. Laboratory examination revealed positivity for CD 19, CD 10, CD 20 and c-myc on flow cytometry, bilateral renal enlargement(10x12cm in longitudinal length) on abdominal ultrasonogram and CT, malignant lymphoma of Buritt's type with prominent nucleoli and cytospin of cerebrospinal fluid. Initially he showed tumor lysis syndrome and clinical stage D by Zigler(stage IV by Murphy) with CNS involvement. After initial management with hydration, urine alkalinization and allopurinol, combination chemotherapy had been applied with the craniospinal radiotherapy according to the CCG 503 II regimane, with achievement of complete remission. Thus we report a case of Burkitt's lymphoma with bilateral renal enlargement with a biref review of literatures.

8.Characteristics of Developmentally Delayed Infants and Young Children Who Demonstrated Normal Myelination by Brain MRI.

PURPOSE: To evaluate the clinical characteristics of infants and young children who had developmental delay without delayed myelination and dysmyelination. METHODS: We retrospectively reviewed 59 cases of developmental disability between July 1996 and June 2001 at Inha University Hospital. Twenty-eight patients showed normal myelination(Group I), while thirty-one patients showed delayed myelination(Group II) by brain MRI. The following clinical records and diagnostic procedures were performed; birth history, head size, neurological examination, developmental assortment test, brain stem auditory evoked potential, visual evoked potential, electroencephalogram, chromosomal study, metabolic screening tests, and clinical psychologic tests. RESULTS: There were no significant differences of sex, age, gestational period, birth weight, etiology, and seizures between group I and II. The incidences of cerebral palsy and abnormal background activity of EEG in group II were significantly higher than those in group I. In group I, mental retardations or speech disorders were higher than motor handicaps. In group II, motor handicaps were higher than mental retardations or speech disorders. In both groups, cases accompanied with seizures, have a tendency of intractability. CONCLUSION: Characteristics of developmentally delayed patients with normal myelination by brain MRI were not significantly different from characteristics of patients with delayed myelination. Even though infants and young children with the developmental delay showed normal findings by brain MRI, they are likely to be accompanied by severe development retardation.

9.Comparison of the Success Rate of Recanalization According to Morphologic Patterns in Proximal Obstructed Fallopian Tube.

PURPOSE: To compare the success rate of recanalization in three types of the proximal fallopian tube obstruction. MATERIALS AND METHODS: Transcervical fallopian tube catheterization (TFTC) was performed in 141 infertile patients with 239 fallopian tube obstruction. Three-coaxial catheter and 0.018 inch guide wire(Terumo, Tokyo, Japan) were employed for the recanalization. The obstructed proximal fallopian tubes were divided into three groups according to the appearance of the distal end of obstructed fallopian tubes : smooth tapering, concave, or convex. RESULTS: Recanalization was successful in 151 of 239 tubes(63.2%) ; success rates were 82.7%(62/75) in the smooth tapering group, 80.2%(69/86) in the concave, and 25.6%(20/78) in the convex. In theproximal portion, success rates were 80.3%(53/66) in the smooth tapering group, 82.6%(38/46) in the concave, and 48.5%(16/33) in the convex. Eighty-nine patients were successfully recanalized. Sixty-four of 89 patients were followed-up for more than one year and 29 patients(45%) became pregnant. Complications occurred in 37 tubes, tube perforation in 32, re-obstruction in five, and ectopic pregnancy in one. CONCLUSION: The success rate of recanalization of an obstructed proximal fallopian tube was higher in smooth tapering and concave types than inthe convex type.

10.Short-term outcomes after laparoscopic surgery following preoperative chemoradiotherapy for rectal cancer.

PURPOSE: The safety and the feasibility of performing laparoscopic surgery for rectal cancer after preoperative chemoradiotherapy (CRT) have not yet been established. Thus, the aim of this study was to evaluate the efficacy and the safety of laparoscopic rectal cancer surgery performed after preoperative CRT. METHODS: We enrolled 124 consecutive patients who underwent laparoscopic surgery for rectal cancer. Of these patients, 56 received preoperative CRT (CRT group), whereas 68 did not (non-CRT group). The patients who were found to have distant metastasis and open conversion during surgery were excluded. The clinicopathologic parameters were evaluated and the short-term outcomes were compared between the CRT and non-CRT groups. RESULTS: The mean operation time was longer in the CRT group (294 minutes; range, 140 to 485 minutes; P = 0.004). In the non-CRT group, the tumor sizes were larger (mean, 4.0 cm; range, 1.2 to 8.0 cm; P < 0.001) and more lymph nodes were harvested (mean, 12.9; range, 0 to 35; P < 0.001). However, there was no significant difference between the two groups in time to first bowel movement, tolerance of a soft diet, length of hospital stay, and postoperative complication rate. CONCLUSION: Performing laparoscopic surgery for rectal cancer after preoperative CRT may be safe and feasible if performed by a highly skilled laparoscopic surgeon. Randomized controlled trials and long-term follow-up studies are necessary to support our results.
