1.Treatment of Patella Fracture

The results of treatment upon 56 cases of fracture of the patella, during the period from July 1975 to October 1978 at the department of orthopedic surgery, Soon Chun Hyang Medical College have been analysed. In sex distribution, 47 were male and 9 were female cases. The incidence was remarkably higher in male and the age group of 20-40. 2. 39 cases were fractured by direct force and 17 cases were indirect force. The most common cause of fracture was traffic accident. 3. The most common site of fracture was midpole and 27 cases were communited type. 4. Our methods of treatment were simple cast, circlege wire loop, tension band technique, partial and total and total patelletomy, etc. as indicated. 5, The mean duration of rehabilitation in the group of immobilized in 10 of flexion was shorter than that of full extension group.

2.The Incidence of Allergic Disease among the Pediatric Outpatient.

The incidence of allergic disease in pediatric outpatient department of a hospital, where primary care is undertaking, was studied from Apr. 1, 1989 to Mar. 32, 1990. The results were as follows, 1) The incidence of respiratory tract disease was the highest with the value of 57.2% of the total patients, followed by gastrointestinal tract disease(13.9%), infectious disease (6.7%), visit for preventive medicine (6.7%), allergic disease (3.0%), neurologic disease (2.9%), cardiovascular disease (2.8%) etc. 2) The allergic patients were 3.0% of total patient and sex ratio was M:F=1.4:1. 3) The allergic patients were most prevalent in1~3 years of age group (46.6%)followed by under 1 year old group (22.4%), and 4~6 years of group (21.3%). 4) The allergic disease was most prevalent in fall (42.8%), followed by spring (20.4%), and winter(19.0%). 5) Among the allergic disease, asthma was most frequent (67.4%), followed by allergic rhinitis(11.8%), atopic dermatitis (10.6%), urticaria (9.3%). 6) Bronchial asthma was most prevalent among 1~3 years of age group (51.9%) and in fall (50.0%) 7) Allergic rhinitis was most frequent among 1~3 years of age group (40.9%) and in fall (34.8%). 8) Atopic dermatitis was most frequent among under 1 year of age group(64.4%)and in spring and winter(28.8%). 9) Urticaria was most frequent among 1~3 years of age group (36.5%) and in summer (32.7%). 10) The percentage of each allergic disease to the total outpatient was as follows; asthma (2.0%), allergic rhinitis(0.40%), atopic dermatitis(0.3%), urticaria (0.3%). 11) Sex ratio of each allergic disease was as follows; asthma (1.5:1), allergic rhinitis(1.6:1), atopic dermatitis (1.1:1), urticaria (1.4:1). With these findings, we ovserved that allergic disease is the fifth most common disease group among the pediatric out patients, covering 3.0% of total patients. Recently, we have had the idea that the prevalence of allergic disease is increasing remarkably. The findings of this study can be used as one of basic data for the study of prvalence and increase of allergic disease with further similar trials afterward.

3.The epidemiological studies on the filariasis in Korea I. Filariasis in Cheju-Do(Quelpart Island).

A night blood survey was carried out among inhabitants aged over 1 year from the fifteen villages throughout Cheju-Do (Quelpart Island). Blood films from 2,139 persons were examined and 183(8.6 percent) showed microfilariae, the incidences varying according to geographical sources are from 0.8 to 19.5 per cent. All the microfilariae found in this survey were of the nocturnal periodic Brugia malayi. The microfilarial density was 1.9 per cent of blood. The age and sex distributions of microfilaria rate in Cheju-Do were not distinctly different. On the other hand, the intradermal test using Dirofilaria antigen (FPT antigen) and clinical survey of filariasis were also undertaken in same areas of microfilaria survey. Out of 2,449 inhabitants examined 1,434(58.6 percent) persons showed positive reaction of skin test, 503(20.5 percent) persons have clinical manifestations and 112 (4.6 percent) persons showed elephantiasis . It is assumed that Aedes togoi may be the most probable vector of B. malayi in the areas of Cheju-Do.

4.Resting and Acetazolamide-Challenged Technetium 99m-ECD SPECT in Transient Global Amnesia.

BACKGROUND AND OBJECTIVE: Transient global amnesia(TGA) is a neurological syndrome affecting preferentially middle-aged or elderly people and characterized by sudden onset of transient impairment of antegrade amnesia with variably retrograde amnesia. TGA was defined over 30 years ago, but the etiology remain unclear. Several mechanisms have been proposed, including arterial thromboembolic ischemic attacks in both posterior cerebral artery territories, epilepsy, and migraine. Although many studies have reported TGA, only a few reported cerebral perfusion studies using SPECT because of the brief duration of the episode. Technetium-99m-ethyl cysteinate dimmer(ECD) is superior to sensitivity of lesion detection and lesion to normal contrast, probably due to lower back diffusion from the brain to the blood and its excellent radiochemical stability. So we evaluate pathophysiologic mechanism of TGA by using 99mTC-ECD SPECT with or without acetazolamide(ACZ) stress. METHODS: We evaluated six patients with transient global amnesia, four women and two men, age ranges 56 to 78 years, mean 62.2 years. Measurements of CBF and vascular reserve using 99mTC-ECD with or without ACD stress were performed during or after TGA episode. RESULTS: One patient, who was evaluated in TGA episode showed that regional cerebral blood flow was decreased in both anteroinferior frontal, both temporal, right thalamus, both inferior parietal and left parietal region with impaired vascular reserve in left inferior temporal and right thalamus. Others, who were evaluated more than 6days in TGA episode showed that regional cerebral blood flow was decreased in left temporal (4), both temporal region (1), left thalamus (2) and both basal ganglia (1) with preserved vascular reserve except one, showed impaired vascular reserve in left thalamus. CONCLUSION: This result show that severe hypoperfusion of bilateral temporal region and impaired vascular reserve in left temporal region in TGA episode. Other patients who were recovered from TGA showed hypoperfusion of left temporal region and preserved vascular reserve. 99mTc-ECD with acetazolamide SEPCT that sensitively detects localized impaired cerebraovascular reserve should help elucidate these processes. But further study with more cases is necessary for evaluation of pathophysiology of TGA.

5.A case of hyperimmunoglobuline E syndrome.

The hyperimmunoglobulin E syndrome is a primary immunodificiency disorder characterized by recurrent staphylococcal infections and markedly elevated serum IgE level. Clinical features are coarse face and severe infections of the skin-furunculosis or chronically pruritic dermatitis and sinopulmonary tract infection from infancy by coagulase positive Staphylococcus aureus, or Candida albicans etc. The patients's serum IgE level is elevated but the basic immunologic pathogenesis not fully understood. We have experienced a case of hyperimmunoglobulin E syndrome in a 26/12-year-old who had suffered from recurrent staphylococcal pneumonias and abscesses and chronically pruritic dermatitis from 1 month of age with elevated serum IgE level. A brief review of the related literature is presented.

6.Clinical Consideration on the Treatment of Fractures by Hoffmann's Transfixation Method

Open long bone fracture is frequently difficult to reduce and maintain, especilly when there is extensive soft tissue damage. Infection, instability, malalignment and soft tissue complication often result in prolonged mobidity or failure. The Hoffmann's apparatus have merits, that can easily correct distracted, angular or rotational deformity of fracture site during early post-operative period, and stable fixation facilitates easily to the care of soft tissue injury. In addition, the affected limb can be elevated with balanced suspension, possible early exercise of neighboring joints. But we have exerienced some problems during treatment of fracture with Hoffmann's transfixation method. From February 1980 to December 1981, Hoffmann's transfixation method was employed in treatment of twenty-one patients that considered to have a poor prognosis with conversional forms of treatment. The results were obtained as follows; 1. For correction of distracted, angular, or rotational deformity, the fracture site should be reduced accurately. 2. Hoffmann pins should be transfixed as one plane and parrallel to each other. 3. Rigidity of fixation can be increased by increasing number of pins, actually three or more pins should be applied at each fragment of fracture. 4. Hoffmann's apparatus is more complex for management, more expensive and requires skillful technique compared with other external fixation apparatus. 5. The lateral view of fracture site could not be confirmed accurately due to overlapping of Hoffmann's adjustable connecting rod and bony shadow. 6. The main cause of injuries was due to traffic accidents in 17 cases (81%), involving tibia in 18 cases, femur in 2 cases and humerus in 1 case. 7. In the sixteen cases which could be assessed, the average times for external transfixation was 13.9 weeks and then followed by early weight bearing with P.T.B. cast or brace. The average time of bony union was 26.4 weeks. 8. The final result was excellent in 7 cases, good in 5 cases, acceptable in 3 cases, poor in 1 case.

7.Clinical Analysis of the 784 Traffic Accident Victims

A retrospective study has been made of 784 cases of road traffic casualties who were treated at the Soon-Chun-Hyang Hospital between July 1980 and December 1981. This study was conducted to find out the nature and pattern of the traffic accident from the clinical and epidemiological standpoints. In additions, the mode of injury from the road accidents were persued which might help to reduce the misdiagnosis rate. The results obtained were as follows: l. In this survey, male dominated in numbers (casualty rate, 70%). The highest incidence was in the age group between 31–40 year (26.5%). The predominance was found in younger casualties (ages between 21–50 years, 72.1% of all casualties). 2. The large number of casualties followed the rush hours. There was a peak between 10PM to midnight. The explanation for this high incidence during the hours was the hurry-up life due to midnight crufew, pedestrian's carelessness and hurry-scurry, and the negligence of traffic rules, the poor labor conditions and weariness due to drivers long continuous working hours, especially in taxi drivers. The highest seasonal incidence was in October. 3. The high incidences were in drivers and students; the drivers were the commonest victims in car passenger accidents and students were injured by pedestrians accidents respectively. The causes of high risk of injury in drivers were intimately related with their vocational aptitudes, the employment status, the driving attitudes and car maintainances; and in students, their carelessness and emotional instability, unsolved mass transportation system and their negligence of traffic rules were listed as the causative factors. 4. Over 51.8% of all casualties were injured by passenger car accidents, and 21.5% were by truck and other types of special vehicles. 5. Approximately 60% of victims were injuried in the road as pedestrian and other 40% were in the car as passengers. In general, the pedestrian injuries were more serious than those sustained in the car as passengers. 6. About 40% of total pedestrian casualties sustained injury at pedestrian's cross road. 7. About 80%of injured drivers were in the drunken state on arrival, and 20.4% of pedestrians were also heavily drunken. 8. The trauma risk was about twice higher in the front-seated passengers than the rear-seated ones in the car. In buses, the highest incidence was in the passengers seated in the front 1/3 of the seats. 9. Orthopaedic (40.5%) and head injuries (38.2%) were the commonest types of the injuries among the hospitalized casualties; about 35% of total patients had the tibial fractures. 10. Of the 784 cases, 479(61.6%) victims arrived hospital within 30 minutes after accident, and among them only 94 cases (12%) were transported by ambulance from the accident site to the hospital.

8.Radiological manifestations of tuberculosis of the spine

The classical Roentgenographic features of tuberculosis of the spine such as narrowing of the intervertebraldisc, collapse of the vertebral bodies, with or without the shadow of a cold abscess, present a specific pictureof the actural pathological process. However, the diagnosis of tuberculosis is not confirmed unless proven bybiopsy, or the finding of the tubercle bacillus. 120 cases of proven spinal tuberculosis, at Kwangju ChristianHospital during the period form Jan 1973 through Aug. 1980, were studied and analysed. The results were asfollows; 1. The ratio of male to female was 1:1.1. The age preference was under 30 years old. 2. The most frequentsite of involvement was the lumbar spine(44.1%) and next the thoracic(39.2%0. 3. The incidence of lytic type was50.8%, mixed type 36.7%, and sclerotic type 12.5%. 4. Associated pulmonary tuberculous lesions were observed in 94patients (78.3%). 5. The central type, with wedging or collapse of the vertebral body, was more common in theyounger age group and the intrevertebral articular type in older patients. 6. The incidences of typical radiologicfindings were : Collapse of vertebral body(90%), cold abscess (78.3%) and narrowing of intervertebral space(70%).7. Associated kyphosis was observed in 37 cases (30.8%). Among these 37 cases the range of angulation was between21 to 40 degrees in 40%.

9.Metabolism of C(14)-glucose by Ascaridia galli.

The fowl nematode Ascaridia galli employed in this experiment was obtained from the intestine of domestic fowls at the local market. The worms selected and washed several times in normal sterilized saline solution. Each about thirty of intact worms were incubated in 50 cc volume of special incubation flasks with incubation mixture consisting of 10 cc of Krebs-Ringer phosphate buffer (pH 7.4) to which were added universally labeled C14-glucose and non-radioactive carrier glucose so as to contain concentration of 200 mg per cent. The worms were allowed to incubation for 3 hours in Dubnoff metabolic shaking incubator at 38 C. After incubation period, respiratory CO(2) samples from central well of incubation flask were analysed for total CO(2) production rate and their specific activity of respiratory CO(2). Glycogen samples isolated from worms were analysed for uptake rate was determined by analyzing the difference of the glucose concentration in a medium before and after incubation period . Radioactivities of these series of experiments were counted by an endwindow Geiger-Muller counter as an infinitely thin samples. The quantitative analysis of C(14)-glucose utilized by Ascaridia galli was summarized as the following . The glucose uptake rate by A. galli was a mean value of 1.73+/-0.32 micro-mole per hour per gram of wet wt. and total CO(2) production rate by the worms averaged 8.44+/-1.11 micro-mole per hour per gram of wet wt. The relative specific activity of respiratory CO(2) (R.S.A CO(2)) averaged 2.68+/-0.38 per cent . Thus , a man of 2.68 per cent of total CO(2) production rate was originated from the glucose in the medium, therefore the rate of CO(2) production derived from medium glucose was a mean of 0.23+/-0.03 micro-mole per hour per gram of wet wt. Thus, the average value of 2.58+/-0.55 percent (R.G.D CO(2))of glucose utilized by the worms from the medium glucose was oxidized to respiratory CO2. The tissue concentration of glycogen in A. galli was a mean of 22.59+/-1.18 miligram per gram of wet wt or 2.26+/-0.123 percent per gram, and the turnover rate of glycogen pool yielded with a mean of 0.17+/-0.04 percent per hour or 0.037+/-0.006 miligram per hour per gram of wet wt. Therefore, a mean value of 16.37+/-4.04 per cent (R.G.D gly) of glucose was incorporated to the glycogen. These data account for that at least 18.95 per cent of the utilized glucose by the worms participated in furnishing the oxidation into respiratory CO(2) and the synthetic process into glycogen. According to the above data of the experiment, it is suggested in the metabolic process of glucose by Ascaridia galli that the synthetic process into the glycogen is more active than the oxidative process into the respiratory CO(2).

10.Normal Predicted values of Pulmonary function Test in Korean Primary School-Aged Children.

Pulmonary function test is one of the most important tools in the management of patients with respiratory tract diseases and allergic diseases. As with the difficulties in performing the test, it has been seldom used in pediatric field. But with the advent of simple, computerized tools, pediatric pulmonologist and allergist try to manage the patients using PFT. Still the normal predicted values are variable among the reports. So it is important to have normal predicted value in Korean children. From April to July 1992, pulmonary function test was performed in 965(male: 490, female: 475) primary school-aged children except <3 or >97 percentile of Korean-children Physical developemental standards with history of allergic diseases and respiratory tract diseases ay SungNam, KyongKiDo. We evaluated the predicted normal values of the FEV1.0, FVC and PEFR and logarithmic regression equation setting the predicted values by using the microspirometer of Micromedical Ltd. England that could that could be easily applied to children. 1) Predicted values of pulmonary function test items were generally higher in boys than those of girls. 2) Correlation coefficient to the parameters examined was the highest in height 0.78, then age 0.75, weight 0.70 and chest circumference 0.61.
