Acute alcoholic myopathy presented with paraparesis, myoglobinuria. And acute renal failure is an uncommon complication of alcoholism. A 34-year-old man had been a heavy alcohol drinker for a year, complained of sudden lower extremity weal; ness and swelling after heavy drinking. On examination, lower extremity was swollen and unable to move. Laboratory data showed markedly elevated GOT, LDH, CK, and myoglobinuria. Initial electrodiagnostic study was indefinite for diagnosis of myopathy and biopsy of the quadriceps muscle showed slight increased eosinophilia of sarcoplasm. But in followup study, myopathic findings and degeneration becarne apparent in electromyography and blopsy.

4.A Clinical Study of Right Middle Lobe Syndrome.

PURPOSE: Right middle lobe symdrome is characterized by a spectrum of disease from recurrent atelectasis and pneunomitis to brobchiectasis of the Right middle lobe symdrom. It was first reported gy Grahm describing 12 patients with middle loge atelectasis and bronchiectasis due to enlarged lymph nodes. The incidence of Right middle lobe syndome seems tobe increasing in children, byt there have been only a few studies of right middlelobe syndrome in Korea. METHODS: Twenty-five children with RMLS who had been admitted during the last 10 years were evaluated with particular attention to clinical features, laboratory results, bronchographic findings, and treatment RESULTS: All patients were symptomatic and complained of chronic cough(25), sputum(20), fever(16), dyspnea(3), vomiting(2), and foreign body in the bronchus(2). Most of the patients had recurrent pneumonia: 6 patients had Mycoplasma pneumonia, and 6 patients had ashma and allergic disorders. Only 5 out of the 25 patients showed sufficient obstruction on bronchography and 6 patients took computed tomography scans. Chest radiography, bronchography and computed tomography scans were evaluated for review in 25 patients showing consolidation(17), patchy infiltration(14), atelectasis(12), hyperinflation(5), bronchiectasis(2), and air bronchogram(2). Most patients were improved by conservative medical management and only 2 patients had closed thoracostomy. CONCLUSION: These 25 patients who had been diagnosed as Right middle lobe syndrome were improved after 2 week treatment of antibiotics and conservative management and their prognosis were good during the follow-up period.

5.Identification and characterization of buckwheat allergen.

BACKGROUND AND OBJECTIVE: Buckwheat is considered one of the most important food allergens in Korea. Although a very small amount is ingested or inhaled, it can cause serious allergic reactions. However, the major allergens of buckwheat still remain to be elucidated. The aim of our study was to identify and characterize the major allergen of buckwheat seed. MATERIAL AND METHOD: Dialysis membrane with a cut-off MW 1kD was used for the preparation of crude buckwheat seed allergen extract. SDS-PAGE under reducing conditions and IgE immunoblotting were performed using sera from 15 buckwheat sensitive subjects. Isoelectric focusing and lectin blotting assay were done. RESULT: Western blot analysis showed more than 15 IgE-reactive buckwheat proteins. Among them, a 24kD protein was shown to be the most frequently bound to sera from allergic subjects (54%). Isoelectric point of 24kD protein was around 5.9. In lectin blotting assay, 24kD protein did not bind to Con A nor five other lectins. CONCLUSION: A 24kD protein was the most frequently recognized allergenic component in buckwheat seed. Isoelectric point was around 5.9. Glycosylation was not detected in 24kD of buckwheat protein.

6.Stent Insertion in Patients with Malignant Biliary Obstruction: Problems of the Hanaro Stent.

PURPOSE: To investigate the problems of the Hanaro stent (Solco Intermed, Seoul, Korea) when used in the palliative treatment of patients with inoperable malignant biliary obstruction. MATERIALS AND METHODS: Between January 2000 and May 2001, the treatment of 46 patients with malignant biliary obstruction involved percutaneous placement of the Hanaro stent. Five patients encountered problems during removal of the stent's introduction system. The causes of obstruction were pancreatic carcinoma (n=2), cholangiocarcinoma (n=2), and gastric carcinoma with biliary invasion (n=1). In one patient, percutaneous transhepatic cholangiography and stent insertion were performed as a one-step procedure, while the others underwent conventional percutaneous transhepatic biliary drainage for at least two days prior to stent insertion. A self-expandable Hanaro stent, 8-10 mm in deameter and 50-100 mm in lengh, and made from a strand of nitinol wire, was used in all cases. RESULTS: Among the five patients who encountered problems, breakage of the olive tip occourred in three, upward displacement of the stent in two, and improper expansion of the distal portion of the stent, unrelated with the obstruction site, in one. The broken olive tip was pushed to the duodenum in two cases and to the peripheral intrahepatic duct in one. Where the stent migrated during withdrawal of its introduction system, an additional stent was inserted. In one case, the migrated stent was positioned near the liver capsule and the drainage catheter could not be removed. CONCLUSION: Although the number of patients in this study was limited, some difficulties were encountered in withdrawing the stent's introduction system. To prevent the occurrence of this unusual complication, the stent should be appropriately expansile, and shape in the olive tip shoud be considered.

7.Two Cases of Successful Use of Urokinase in Continuous Ambulatory Peritoneal Dialysis Patients with Relapsing Peritonitis.

Peritonitis is a frequent and serious complication in continuous ambulatory peritoneal dialysis (CAPD) patients. Recently, due to educational promotion in general hygiene and development of laboratory technique for bacterial cultures and sensitivity test, proper use of antibiotics, the incidence of CAPD peritonitis has gradually decreased. However, CAPD peritonitis is still one of the most common causes of peritoneal dialysis failure and of removal peritoneal catheter. It has been suggested that the formation of biofilm on the inner surface of peritoneal catheter leads to relapsing peritonitis and removal of the peritoneal catheter in CAPD patients. The biofilm is a kind of protecting coat which consists of fibrin inhibiting the penetration of antibiotics. It surrounds and covers the bacteria, making them to survive from the attack of antibiotics. Therefore thrombolytic therapy, urokinase modifies the structure of biofilm, and helps the antibiotics penetrating the fibrin coat, eventually amplify the bacteriocidal effect. We experienced two cases of successful treatment with urokinase and antibiotics in CAPD peritonitis patients. The combination of thrombolytic agents and antibiotics might be one of the strategies for the treatment of CAPD patients who experienced it frequently.

Solin(R) (oral sodium phosphate) is a commonly used osmotic laxative solution that has replaced polyethylene glycol in bowel preparation for colonoscopy as it has equal or greater efficacy and patient compliance. However, its use has been associated with several cases of acute renal failure and electrolyte imbalance, especially hyperphosphatemia and hypocalcemia. Those at higher risk of complications are patients who are older and have intestinal obstruction, inflammatory bowel disease, renal failure, or congestive heart failure. We report the case of a 61-year-old woman presenting with paresthesia in both hands after using Solin(R) for bowel preparation for colonoscopy. The patient had hypocalcemia and hyperphosphatemia combined with acute renal failure. She recovered from the renal failure and electrolyte imbalance with intravenous hydration and treatment with phosphate-binding agents.

9.Identification and Characterization of German Cockroach Allergen.

PURPOSE: Cockroaches have been reported one of the major causes of allergic disease such as bronchial asthma and allergic rhinitis. About 10% of children and 20% of adult respiratory allergy patients have positive results of allergy skin tests to cockroach allergen. This finding suggests that cockroach is also important allergen in Korea. Among 4,000 species, German cockroach seems predominant species in the Korean peninsula. Identification and charactrerization of the major allergen is the first step in German cockroach allergen research. But, there have been controversies. It may depends upon different identifying methods. The aim of our study was to identify the major allergen in German cockroach using Westernblot. METHODS: Crude German cockroach allergen extracts with or without protease inhibitor cocktail mixture were prepared, and protein patterns of these two extracts were compared using SDS-PAGE and Coommssie stain. Sera from 32 atopic asthmatic patients with positive allergy skin test against cockroach allergen were used for IgE immunoblot. RESULTS: Coomassie stain after SDS-PAGE demonstrated some discordance between the differently prepared crude allergen extracts. In comparison with the allergen extract which abolished endogenous protease activity, the untreated extract revealed new 32, 55 and 78 kD protein bands and 41 kD protein band was disappeared. About 60% of sera with positive allergy skin test revealed IgE binding bands in Westernblot. Among them, 10, 21, 25, 36 and 52 kD protein band might seemed important German cockroach allergen. CONCLUSION: Finding above may suggest that endogenous protease could denature allergenic proteins in the process of crude cockroach allergen extract. The 10, 21, 25, 36, and 52 kD might be the important allergens in German cockroach.

10.Marginal fit related to margin types of glass infiltrated alumina core fabricated from aqueous-based alumina tape.

STATEMETN OF PROBLEM: In-Ceram system is one of the all-ceramic crowns that can be used in anterior 3 unit fixed partial dentures and posterior single crowns. The alumina core used in In-Ceram system is manufactured using slip-casting technique. The slip-casting technique is difficult and technique sensitive. To improve this problem, tape-casting method was introduced into dentistry. There were no studies to examine the effect of margin design on the margin fit-ness of all-ceramic crowns fabricated from alumina tape. PURPOSE: The purpose of this study was to compare the marginal fitness of glass infiltrated alu-mina core fabricated from aqueous-based alumina tape according to different margin types (90 .,110 ., 135 .shoulder margin). MATERIAL AND METHOD: Three upper central resin incisors were prepared with 90 ., 110 .and 135 .shoulder margins for all-ceramic crowns, respectively. The resin teeth were duplicated and master die and special plaster die were made as usual. After alumina cores were fabri-cated from aqueous-based alumina tape, cores were cemented to each 15 epoxy dies replicated from three resin teeth with resin cement. These cemented cores were embedded in epoxy resin. Specimens were cut mesiodistally and buccolingually. Marginal gap and discrepancy were measured under microscope. RESULTS: The marginal gap and discrepancy of 90.marginal angle was 75.1 mu m, 86.6 mu m, 110 .marginal angle was 41.5 mu m, 50.7 mu m and 135 .marginal angle was 51.7 mu m, 54.2 mu m, respectively. The smallest value was seen in 110 (angle, which was statistically significant com-pared to that of 90 .angle (p<0.05). CONCLUSION: Marginal fitness of alumina cores made of alumina tape with 110 .shoulder mar-gin was best and others were clinically acceptable.
